Book excerpt: The demand for reconstructive orthopaedic implants continues expanding at a reasonable pace as the incidence of fracture injuries and infectious diseases rises. There has been an increase in the clinical need for more effective synthetic bone graft materials due to the drawbacks of autogenous grafts. Since the 1980?s calcium phosphate cements (CPC?s) have attracted a great deal of interest due to their chemical similarities to natural bone; chemical, physical and mechanical characteristics have been investigated and manipulated to maximise osteoconductivity and osteointegration of these CPC?s since the start of their commercialisation. Here in this thesis, a series of investigation are complied to demonstrate novel and inventive approaches to expand the application of CPC?s: (1) limiting the liquid phase in the setting reaction of a brushite cement to produce monetite (dehydrated brushite) based cement, with increased solubility to overcome the problems faced by long term stability of hydroxyapatite (HA) cements; (2) manipulating the cement formulation to produce a cement that can set on a change in temperature, upon implantation, increasing handling time during surgeries; (3) incorporating therapeutic molecules to eliminate secondary surgeries following infectious diseases; (4) to enhance osteointegration of CPC?s by synchronising the degradation to natural bone formation. Results exhibit compressive strength appropriate for the application of cranioplasty; long term ageing studies demonstrates that the novel cement formulations do not hydrolyse to HA, eliminating the risk of catastrophic brittle failure that is commonly associated with CPC?s.

Book excerpt: The use of commercially available powder-liquid acrylic bone cements in vertebral compression fracture augmentation can lead to procedural complications. These limitations of these cements are the source of complications with such techniques. A premixed cement system was developed as an alternative material for spinal procedures to overcome limitations such as poor handling and pseudoplasticity, and bioinertness of the powder-liquid cements. Spinal augmentation procedures such as vertebroplasty require high viscosity cements that can be injected through small cannulas. The cement must also be able to interdigitate and osseointegrate with the surrounding trabecular bone in order to provide desirable fixation. Premixed acrylic-calcium phosphate composite formulations were prepared through customizing the handling properties of the cement. Furthermore, bioactivity was introduced through calcium phosphate phase addition. The goal of this study was to develop composite cements tailored for use in vertebroplasty. Cement properties were characterized and the formulations were tested in vertebroplasty models. Cell and animal studies were performed to analyze the osseointegrative ability of the cements. In summary, novel premixed composite formulations were successfully developed, demonstrating desirable characteristics for use in vertebral augmentation.

Book excerpt: "Synthetic bone graft substitutes are frequently used to fill defects with significant bone loss to prevent fibrous tissue ingrowth and loss of function. Bone grafts are often used to augment deficient alveolar ridges allowing placement of dental implants subsequent to prior bone loss. Although autografts, allografts and xenografts have been used and researched extensively, they have inherent limitations. To overcome these, synthetic alternatives, such as calcium phosphate cement-based biomaterials are being developed. Brushite and monetite are acidic calcium phosphates having similar chemical composition. However, their in vivo behavior differs in terms of resorption and bone response. Although brushite and monetite have been shown to resorb faster in vivo than hydroxyapatite (HA), a significant reduction in the rate of resorption occurs following phase conversion of brushite to insoluble HA. As such differences between bone grafts are attributed to material composition even though physical properties such as surface area and porosity invariably differ. This thesis focuses on determining the factors and mechanisms that makes these chemically similar materials behave differently in vivo. We produced brushite cement grafts and converted them to two types of monetite by using wet (autoclaving) and dry heat (under vacuum) dehydration which resulted in materials with differing physicochemical properties. These grafts were then aged in vitro using bovine serum and phosphate buffered saline (PBS) solutions, implanted subcutaneously, in femoral condyles and onlay grafted on calvaria. The dissolution, resorption and bone formation potential of these dicalcium phosphate cement grafts was assessed and compared. Also, we developed a new alkali ion (sodium and potassium) substituted calcium phosphate cement, reinforced with silica and set using phytic acid. The set cement blocks were aged in PBS solutions and their in vitro dissolution and degradation along with other physico-chemical properties was assessed.Results presented in this Ph.D thesis discuss and shed light on the fundamental yet not understood questions regarding the relative contributions from chemico- vs physico-biomaterial properties in graft resorption and bone regeneration. This will allow in future the preparation of improved calcium phosphate-based bone substitute grafts with potential to achieve higher clinical efficiency. " --
