Book excerpt: Introduction : La présence de personnes âgées aux urgences pour des problématiques médico-sociales n'est pas rare. Souvent considérée comme inappropriée, elle pose des difficultés aux urgentistes. Méthode : Nous avons mené une étude transversale concernant les patients de 75 ans ou plus aux urgences du CHU Sud, adressés par un médecin pour un motif médico-social. Les facteurs déterminant cet adressage ont été recueillis auprès des médecins adresseurs puis étudiés quantitativement. Résultats : L'étude a permis une évaluation des passages de patients de 75 ans ou plus en médecine au CHU de Saint Pierre, environ 10 patients par jour, dont 19% pour des motifs médico-sociaux, 28 % adressés par un médecin de ville et 45 % environ seront hospitalisés. Sur 82 dossiers éligibles, 30 ont été inclus. Les réponses des médecins participants montraient des préoccupations surtout somatiques, à la fois de pathologies aiguës et de syndromes gériatriques, ainsi que la difficulté d'organiser des entrées directes. Dans les analyses en sous groupes, le statut de médecin traitant et l'observation de troubles de la marche (nouveaux ou aggravés) étaient significativement protecteurs contre le recours inapproprié. Discussion : Derrière un motif médico-social se cache souvent une préoccupation somatique, d'autant plus pertinente si c'est le médecin traitant qui constate l'évolution défavorable. Les critères de jugement du caractère approprié du recours aux urgences restent assez subjectifs. L'utilisation massive des ressources de l'hôpital est souvent en contradiction avec l'évaluation que la situation aurait été mieux gérée en ambulatoire.
