Notre objectif était d'analyser les déterminants de l'impact d'un programme d'éducation thérapeutique au diabète au sein d'une population caribéenne à haut risque cardio-vasculaire. Méthode : Il s'agit d'une étude observationnelle longitudinale mono centrique réalisée à la MSP de Ducos en Martinique. Tous les patients diabétiques consultant sur les périodes d'inclusion ont été inclus après information éclairée et ont reçu une ETP pluridisciplinaire en groupe. Les données anthropométriques, biologiques (GAJ, Hba1c) ainsi que les comorbidités et habitus ont été recueillis lors des entretiens ou à partir des dossiers médicaux. L'analyse statistique s'est dichotomisée en analyse descriptives, réalisées à l'aide du test de Student ou de Wilcoxon selon la normalité, puis régressions logistiques. Résultats : Sur les 232 patients inclus, 204 patients ont constitué l'échantillon final soit 145 femmes et 59 hommes. La moyenne d'âge est de 64.55ans (ET=11.91) et la durée d'évolution du diabète à la date d'inclusion est de 9.99 ans (ET=8.08). On observe une baisse significative de l'Hba1c les deux premières années mais celle-ci n'est plus que tendancielle au bout de 3ans. Le poids des sujets diminue de façon significative à tous les temps d'étude. Le fait de pratiquer une activité physique plus de 3h par semaine, de reconnaître une étiologie alimentaire au diabète ou d'avoir un moral satisfaisant est prédictif d'une meilleure Hba1c après ETP. Le modèle fait ressortir que les patients ayant un diabète plus ancien, un ATCD familial de diabète chez leurs parents, des difficultés (financières, familiales ou de santé) et qui attribuent leur diabète à un choc émotionnel ou un stress ressenti ont une Hba1c plus élevée. Conclusion : L'ETP a montré une baisse significative de l'Hba1c jusqu'à 2 ans après le programme. Il serait opportun de renouveler l'intervention passé ce délai. L'action éducative doit cibler les patients les plus vulnérables.
