Book excerpt: Minimally-invasive laparoscopic procedures have proven efficacy for a wide range of surgical procedures as well as benefits such as reducing scarring, infection, recovery time, and post-operative pain. While the procedures have many advantages, there are significant shortcomings such as limited instrument motion and reduced dexterity. In recent years, robotic surgical technology has overcome some of these limitations and has become an effective tool for many types of surgeries. These robotic platforms typically have an increased workspace, greater dexterity, improved ergonomics, and finer control than traditional laparoscopic methods. This thesis presents the designs of both a four degree-of-freedom (DOF) and 5-DOF miniature in vivo surgical robot as well as a software architecture for development and control of such robots. The proposed surgical platform consists of a two-armed robotic prototype, distributed motor control modules, custom robot control software, and remote surgeon console. A plug-in architecture in the control software provides the user a wide range of user input devices and control algorithms, including a numerical inverse kinematics solver, to allow intuitive control and rapid development of future robot prototypes. A variety of experiments performed by a surgeon at the University of Nebraska Medical Center were used to evaluate the performance of the robotic platform.

Many procedures that were once predominantly performed as open procedures are now routinely performed using minimally invasive surgery with improved patient outcomes. Recently, there has been a renewed interest in accessing the surgical environment through a single incision or a natural orifice to further reduce invasiveness. The limitations in visualization and manipulation experienced by surgeons performing traditional laparoscopy through multiple small incisions are intensified when working through a single, constrained insertion point. Many of the current methods for improving triangulation are focused on improving the articulation of existing laparoscopic tools, the use of the da Vinci® Surgical System, or on the development of more compact prototype surgical robots, also based on an externally-actuated platform. ^ Miniature in vivo robots represent a fundamentally different approach to minimally invasive robotic surgery where the entire robot is inserted into the abdomen to perform a surgical intervention. A second generation of planar robots for Natural Orifice Translumenal Endoscopic Surgery, NOTES, were developed that attempt to emulate the use of two laparoscopic tools while using a NOTES platform. These robots have been prototyped and demonstrated in two animal model cholecystectomies. ^ Cholecystectomy is an example of a surgical procedure that is currently performed routinely using a laparoscopic approach. Other, more complicated procedures, such as colon resection, have a more limited acceptance and remain predominantly performed as open procedures. To better understand the design requirements in developing a robotic system for performing a colectomy, a task decomposition for sigmoid colectomy has been performed. This decomposition formed the basis for the design of a dexterous robot prototype for performing this procedure. The design of this prototype robot is presented and results of benchtop testing and two animal model procedures are discussed. The development of this prototype robot represents a step towards the development of dexterous miniature in vivo robots that can help extend the benefits of a minimally invasive approach to more complicated surgical procedures.

Book excerpt: Advancements in minimally-invasive surgical (M.I.S.) techniques continue to combat the major drawbacks of open surgery. Lengthy recovery times, increased chance of postoperative infection, and distinct cosmetic remnants are a few of the many issues that implore the engineering community for technological intervention. Conversion of open surgery to minimally invasive laparoscopic surgery has gained popularity in recent years. Laparoscopic surgery consists of the insertion of thin profiled tools and accessories into a gas-inflated abdominal region, eliminating the need for a large incision; however, these procedures have introduced an entirely new set of limitations such as increased duration of surgery, reduction in visibility of the surgical site, and greater dexterity requirements of the surgeon. Accordingly, robotic platform integration has been introduced to the surgical field of medicine and looks to challenge these limitations. The goal of this thesis is to examine the accumulation of past designs for miniature in vivo robotics and to expand and innovate upon the strengths that each design has to offer. Various novel designs are presented to address numerous limitations of current techniques in miniature in vivo surgical robotics, including the insertion interface, mobility, and dexterity of the robot. This thesis also investigates the contribution to increased wound morbidity by larger, single-incision techniques to further strengthen the case for smaller, multiple incisions. The JoeyBot prototype design features a 3 degree-of-freedom compact, in-line joint, allowing for a slimmer overall profile of the robot. It is an independently fixated manipulator capable of insertion through a custom prototyped trocar. This is advantageous in that trocars are a proven technology, offering various advantages in comparison to devices such as the GelPort®, used in previous designs. The design also offers increased mobility with independently actuated gross positioning systems. Also presented is an additional design for implementing a 3 degree-of-freedom end-effector wrist joint to increase dexterity in performing complex surgical tasks..

Robotics-assisted MIS procedures have been conducted on over 3 million patients to date — primarily in the areas of urology, gynecology and general surgery using the FDA approved da Vinci® surgical system. The significant commercial and clinical success of the da Vinci® system has resulted in substantial research activity in recent years to reduce invasiveness, increase dexterity, provide additional features such as image guidance and haptic feedback, reduce size and cost, increase portability, and address specific clinical procedures. The area of robotic MIS is therefore in a state of rapid growth fueled by new developments in technologies such as continuum robotics, smart materials, sensing and actuation, and haptics and teleoperation. An important need arising from the incorporation of robotic technology for surgery is that of training in the appropriate use of the technology, and in the assessment of acquired skills. This volume covers the topics mentioned above in four sections. The first section gives an overview of the evolution and current state the da Vinci® system and clinical perspectives from three groups who use it on a regular basis. The second focuses on the research, and describes a number of new developments in surgical robotics that are likely to be the basis for the next generation of robotic MIS systems. The third deals with two important aspects of surgical robotic systems — teleoperation and haptics (the sense of touch). Technology for implementing the latter in a clinical setting is still very much at the research stage. It covers the various types of nanorobotic systems, their applications and future directions in this field. The volume is divided into three themes related to medical applications. The first theme describes the main challenges of microrobotic design for propulsion in vascular media. Such nanoscale robotic agents are envisioned to revolutionize medicine by enabling minimally invasive diagnostic and therapeutic procedures. To be useful, nanorobots must be operated in complex biological fluids and tissues, which are often difficult to penetrate. In this section, a collection of four papers review the potential medical applications of motile nanorobots, catalytic-based propelling agents, biologically-inspired microrobots and nanoscale bacteria-enabled autonomous drug delivery systems. The second theme relates to the use of micro and nanorobots inside the body for drug-delivery and surgical applications. A collection of six chapters is presented in this segment. The first chapter reviews the different robot structures for three different types of surgery, namely laparoscopy, catheterization, and ophthalmic surgery. It highlights the progress of surgical microrobotics toward intracorporeally navigated mechanisms for ultra-minimally invasive interventions. The first section addresses an important activity in our daily lives: walking, where the neuromuscular system orchestrates the gait, posture, and balance. Conditions such as stroke, vestibular deficits, or old age impair this important activity. However, even in the best cases the level of performance of a surgeon in Minimally Invasive Surgery is still a fraction of the con ventional surgery. With these procedures, surgeons use long, slender tools which pass through several small incisions. Performing surgery in this fashion has shown many benefits including reduced pain and recovery times, lower costs, and less scarring post-recovery. The use of surgical robotics has shown several key advantages over MIS techniques. Minimally invasive surgeries typically require unnatural movements, have limited visibility, greatly reduce dexterity, and provide little tactile feedback. Through robot kinematics and specialized sensors, surgical robots can resolve many of these limitations, especially in terms of creating intuitive controls that can be mastered quickly, without losing many of the benefits of MIS. Because of these properties and their relatively small size, surgical robots could be viable options for use during space flight emergencies. This thesis presents the design and assembly of a parabolic flight payload to evaluate these robots in microgravity where the robot performance and the operator capability is unknown. The structure supports all required hardware and is compliant with all NASA requirements and guidelines for microgravity research. Through future experiments using the payload, completion metrics such as experimental time-to-completion and robot positioning accuracy will be used to define the challenges with working in microgravity as well as propose possible solutions to create a surgical system for space.
