Adopting a public health approach, the report responds to both the enormity of health problems associated with obesity and the notorious difficulty of treating this complex, multifactorial disease. With these problems in mind, the report aims to help policy-makers introduce strategies for prevention and management that have the greatest chance of success. The importance of prevention as the most sensible strategy in developing countries, where obesity coexists with undernutrition, is repeatedly emphasized. Recommended lines of action, which reflect the consensus reached by 25 leading authorities, are based on a critical review of current scientific knowledge about the causes of obesity in both individuals and populations. While all causes are considered, major attention is given to behavioural and societal changes that have increased the energy density of diets, overwhelmed sophisticated regulatory systems that control appetite and maintain energy balance, and reduced physical activity. Specific topics discussed range from the importance of fat content in the food supply as a cause of population-wide obesity, through misconceptions about obesity held by both the medical profession and the public, to strategies for dealing with the alarming prevalence of obesity in children. Les médecins généralistes sont les premiers intervenants pour dépister et prendre en charge cette pathologie complexe. Ils sont cependant peu nombreux à s'investir et font face à des difficultés. Notre travail a cherché à identifier les besoins des médecins généralistes pour réaliser le dépistage et la prise en charge des enfants et adolescents en surpoids ou obèses. Méthode : Une enquête qualitative suivant la méthodologie des focus group a été réalisée entre décembre 2014 et avril 2015 auprès de 14 médecins généralistes des Alpes Maritimes répartis en 3 groupes. Résultats : L'analyse des données recueillies a mis en évidence quatre besoins nécessaires pour optimiser le dépistage et la prise en charge du surpoids et de l'obésité en pédiatrie. Les médecins généralistes ont besoin de motivation pour débuter une prise en charge et affronter les différentes difficultés. Ne pouvant agir seul, ils ont besoin d'un investissement de la famille du patient. Pour mener à bien la prise en charge, une certaine organisation pratique est nécessaire qui regroupe notamment, la nécessité d'avoir du temps, et certains outils facilement accessibles et exploitables. Dans les cas les plus difficiles la collaboration avec des professionnels spécialisés est intéressante mais doit être facilitée. Perspectives : Depuis notre travail, il a été créé une adresse mail dédiée à disposition des médecins généralistes pour communiquer avec le centre spécialisé de l'obésité. Ce document pourra être complété par différents outils au cours d'un prochain travail de recherche puis testé auprès des médecins généralistes afin de définir s'il correspond à leurs attentes.
