Trois cancers sont des candidats au dépistage : cancer colorectal, mammaire et cancer du col de l'utérus qui reste le seul à ne pas être soumis à un dépistage organisé. Actuellement, ce dépistage est individuel la participation atteint difficilement 60% de la population. Le dépistage organisé reste le plus efficient en termes de coût/efficacité. Mais encore faut-il que les patientes trouvent des professionnels de santé pour réaliser cet examen simple qu'est le frottis cervico-utérin : les effectifs baissent et le manque de médecins est de plus en plus important en zone rurale. Je me suis intéressée au dépistage dans le canton rural de Montret où je compte m'installer prochainement. J'ai interrogé les patientes du canton et les médecins du territoire de santé par des questionnaires anonymes, j'ai collecté et analysé les données des caisses d'assurance maladie et des laboratoires d'anatomopathologie. Les patientes se pensent bien dépistées mais les données chiffrées montrent que le taux de couverture est loin d'être suffisant, inférieur à la moyenne nationale. Du côté des soignants, l'envie de plus s'impliquer est présente mais le manque de temps et la multiplication des tâches qui leur incombent est un obstacle. Face à cette situation, le système de soin doit changer en facilitant la tâche des médecins mais aussi en améliorant l'organisation des soins notamment au travers de nouvelles stuctures comme les maisons de santé pluridisciplinaires, qui permettent des actions de proximité plus adaptées aux spécificités des population.

Book excerpt: Notre travail a pour but de connaître les réticences des femmes âgées en milieu rural à propos du dépistage du cancer du col de l'utérus, afin d'adapter la campagne à leur attentes. Nous avons interrogé par le biais d'un questionnaire, 42 femmes vivant en milieu rural et ayant participé aux joumées organisées en petits groupes sur l'alimentation et la prévention en matière de santé de façon générale. Au cours de ces réunions , féminines exclusivement, a été abordé le dépistage du cancer du col de l'utérus, dépistage organisé dans le Doubs depuis 1993. En général, les femmes de notre enquête sont informées sur le dépistage du cancer du col de l'utérus ainsi que sur le cancer du sein. Les affiches, la MSA (par l'intermédiaire des invitations nominatives et des réunions) et le médecin généraliste leur permettent une bonne transmission de l'information. Toutes les femmes enquêtées ont un suivi gynécologique régulier et ont un rythme de dépistage satisfaisant. Certaines ne désiraient pas en parler pendant les réunions mais celles-ci se sentaient suffisamment informées. La plupart du temps, c'est le gynécologue qui réalise le frottis cervico-vaginal, mais les femmes mettent très souvent en avant le rôle primordial du médecin généraliste dans l'information et la prévention. Il est difficile de mesurer l'impact des différentes actions dans les campagnes, mais notre étude révèle l'intérêt du médecin généraliste pour ces femmes en milieu rural. De même, les groupes de paroles organisés par la MSA apportent une bonne sensibilisation de ces femmes au dépistage

Selon elle, la prévalence des lésions de haut grade était de 2.5% alors qu'elle était de 1.6% selon les frottis conventionnels. Aucun cancer n'a été détecté sur 750 patientes. L'anatomopathologie a confirmé les résultats dans 85% des cas pour la technique en milieu liquide et dans 60% pour la méthode conventionnelle. Le dépistage cytologique est de qualité insuffisante dans cette région et d'autres stratégies doivent être envisagées.
