Book excerpt: Challenges which the Department of Defense (DOD) faces in restructuring its health care system to managed care include: (1) significant budget constraints as the size of the military forces is reduced, (2) building a consensus of support among various parties for managed care, and (3) making key operational decisions in the face of little data from the Department's demonstration projects. GAO believes that DOD should reconsider certain features of its Coordinated Care program to address the following problems: differences in cost-sharing by enrollees depending on whether they are directed to military facilities or private network providers for their medical care, higher levels of cost-sharing by enrollees compared to those in health maintenance organizations, and use of negative incentives to encourage enrollment such as eliminating nonenrollees access to military health facilities. GAO believes that DOD should reconsider the range of beneficiary cost-sharing options available for Coordinated Care including (1) instituting small charges for care provided to beneficiaries in military facilities and (2) a system of beneficiary premiums. DOD should postpone its plans to "lock out" nonenrollees from military health care facilities.

Through TRICARE, its new managed health care program, DOD is trying to improve access to care among its 8.3 million beneficiaries while containing costs. How well DOD implements and operates TRICARE may define and shape military medicine for years to come. Because of TRICARE's complexity, scale, and impact on beneficiaries, GAO reviewed the program, focusing on (1) whether DOD's experiences with early implementation yielded the expected results, (2) how early outcomes may affect costs, and (3) whether DOD has defined and is capturing data needed to manage and assess TRICARE's performance. GAO concludes that despite initial confusion among beneficiaries arising from marketing and education problems, as well as problems with the compatibility of computer systems, early implementation of TRICARE is progressing consistent with congressional and DOD goals. However, the success of DOD's efforts to implement resource-sharing agreements and utilization management is critical to containing health care costs. DOD also needs to gather enrollment and performance data so that it and Congress can assess TRICARE's success in the future.

By mid-1998, DOD expects to have implemented seven TRICARE managed care support contracts at an estimated cost of $17 billion. Since the award of the first TRICARE contract in 1994, 357 change orders have been made to the five TRICARE contracts now in place. Cumulatively, these change orders have increased tasks and overall costs. DOD has settled 134 of the orders at a cost of about $336,000. DOD estimates costs for the 223 orders that are yet to be settled at $38 million. However, DOD's initial cost estimates differ markedly from contractor estimates. GAO found that although there have been many change orders, DOD has not adequately managed the process. For example, rather than separately budgeting for the costs of individual change orders, DOD has used funds budgeted for other Defense Health Program activities to pay for them--an approach that could potentially create a need for supplemental funding. In addition, DOD's initial cost estimates for new orders have not been sound. As a result, DOD has not developed a reliable estimate of the total federal liability for the contract changes.

Book excerpt: Under TRICARE, the Department of Defense's (DOD) managed care program, military-operated hospitals and clinics are supplemented by contracted civilian services. Since the inception of TRICARE, DOD has made many changes to these contracts via contract change orders. Since July 1997, when GAO reported that DOD was trying to improve its change order process, the backlog of change orders has continued to grow. This report evaluates (1) the status of the change order backlog and how DOD addressed it, (2) factors that contributed to the growth of the backlog, and (3) DOD's new initiative to improve the change order process. GAO found that as of June, 2000, the number of change orders issued had almost tripled, while the number of unsettled change orders had more than doubled since July 1997. Despite recommendations to devote high-level attention to managing improvements to the change order process, this was not done. Until recently, none of TRICARE Management Activity's (TMA) many initiatives significantly improved the process or reduced the backlog. The current small backlog is the result of recent concerted effort, not better management over time. TMA's new Change Management Process (CMP) appears to address many of TMA's problems with change orders. However, past initiatives have made similar promises and delivered little when they were abandoned or eclipsed by higher priorities. The high volume of change orders now or soon to enter the new CMP makes it imperative that TMA management closely monitor the process to prevent future backlogs.
