Book excerpt: "[...]article in a scientific paper from the pen of a jackass who showed a Corliss engine card, and then blackguarded the railroad mechanics of America for being satisfied with the link because it was handy. I started in to design a motion to make a card, but-well, you know how good-for-nothing those things are to pull loads with. "After my first attempt, I put in many nights making a wooden model for the Patent Office. I was subsequently informed that the child of my brain interfered with about ten other motions. Then I commenced to think-which I ought to have done before. I went to studying what had been done, and soon came to the conclusion that I just knew a little-about enough [...]".

Book excerpt: The main aim of this practical Handbookis to strengthen counselling and communication skills of skilled attendants (SAs) and other health providers, helping them to effectively discuss with women, families and communities the key issues surrounding pregnancy, childbirth, postpartum, postnatal and post-abortion care. Counselling for Maternal and Newborn Health Careis divided into three main sections. Part 1 is an introduction which describes the aims and objectives and the general layout of the Handbook. Part 2 describes the counselling process and outlines the six key steps to effective counselling. It explores the counselling context and factors that influence this context including the socio-economic, gender, and cultural environment. A series of guiding principles is introduced and specific counselling skills are outlined. Part 3 focuses on different maternal and newborn health topics, including general care in the home during pregnancy; birth and emergency planning; danger signs in pregnancy; post-abortion care; support during labor; postnatal care of the mother and newborn; family planning counselling; breastfeeding; women with HIV/AIDS; death and bereavement; women and violence; linking with the community. Each Session contains specific aims and objectives, clearly outlining the skills that will be developed and corresponding learning outcomes. Practical activities have been designed to encourage reflection, provoke discussions, build skills and ensure the local relevance of information. There is a review at the end of each session to ensure the SAs have understood the key points before they progress to subsequent sessions.

This book was released on 2017-09-18 with total page 207 pages. Available in PDF, EPUB and Kindle. Book excerpt: The immune system detects "danger" through a series of what we call pathogen-associated molecular patterns (PAMPs) or damage-associated molecular pattern molecules (DAMPs), working in concert with both positive and negative signals derived from other tissues. PAMPs are molecules associated with groups of pathogens that are small molecular motifs conserved within a class of microbes. They are recognized by Toll-like receptors (TLRs) and other pattern recognition receptors. A vast array of different types of molecules can serve as PAMPs, including glycans and glycoconjugates. Bacterial lipopolysaccharides (LPSs), endotoxins found on the cell membranes of Gram-negative bacteria, are considered to be the prototypical class of PAMPs. LPSs are specifically recognized by TLR4, a recognition receptor of the innate immune system. Other PAMPs include bacterial flagellin (recognized by TLR5), lipoteichoic acid from Gram-positive bacteria, peptidoglycan, and nucleic acid variants normally associated with viruses, such as double-stranded RNA, recognized by TLR3 or unmethylated CpG motifs, recognized by TLR9. DAMPs, also known as alarmins, are molecules released by stressed cells undergoing necrosis that act as endogenous danger signals to promote and exacerbate the immune and inflammatory response. DAMPs vary greatly depending on the type of cell (epithelial, mesenchymal, etc.) and injured tissue. Some endogenous danger signals include heat-shock proteins, HMGB1 (high-mobility group box 1), reactive oxygen intermediates, extracellular matrix breakdown products such as hyaluronan fragments, neuromediators, and cytokines like the interferons (IFNs). Non-protein DAMPs include ATP, uric acid, heparin sulfate, and DNA. Furthermore, accumulating evidence supports correlation between alarmins and changes in the microbiome. Increased serum or plasma levels of these DAMPs have been associated with many inflammatory diseases, including gastric and intestinal inflammatory diseases, graft-versus-host disease (GVHD), sepsis and multiple organ failure, allergies particularly in the lungs, atherosclerosis, age-associated insulin resistance, arthritis, lupus, neuro-inflammation/degeneration and more recently in tumors, which is particularly interesting with the emergence of immunotherapies. Therapeutic strategies are being developed to modulate the expression of these DAMPs for the treatment of these diseases. A vast number of reviews have already been published in this area; thus, in an effort to not duplicate what has already been written, we will focus on recent discoveries particularly in disease models that are epidemic in Western society: intestinal chronic inflammatory diseases including GVHD and its relationship with the microbiome, chronic infectious diseases, allergies, autoimmune diseases, neuroinflammation and cancers. We will also focus on the basic cellular roles of macrophages, T cells and B cells. This research topic brings together sixteen articles that provide novel insights into the mechanisms of action of DAMPS/alarmins and their regulation and subsequent immunologically driven responses.

Book excerpt: John Alexander Hill (1858-1916) was a co-founder of The McGraw-Hill Companies. In the 1880s he worked as an editor at Locomotive Engineer. Over the next few years he would produce several technical and trade publications, eventually forming his own company, The Hill Publishing Company, in 1902. He had known James H. McGraw, who had established The McGraw Publishing Company in 1899, for some time and the two men shared similar interests. In 1909 they agreed upon an alliance and combined the book departments of their publishing companies into The McGraw-Hill Book Company. He wrote Danger Signals (1898) with Jasper Ewing Brady. Colonel Jasper Ewing Brady was the author of: Tales of the Telegraph (1899), The Little Angel of Canyon Creek (1914), The Tangle (1914), Private Dennis Hogan (1914), From the Dregs (1915), Lifting the Ban of Coventry (1915), The Island of Regeneration (1915), Hearts and the Highway (1915), The Hero of Submarine D-2 (1916), The Surprises of an Empty Hotel (1916), Britton of the Seventh (1916), The Island of Surprise (1916), The Ninth Day (1917), The Case of Mary Sherman (1917), The Divorce Trap (1919) and The Necessity of Original Photoplay Material (1920).

Until Tierney uncovered a fact he couldn't deny. Someone was watching Wade—someone who might be connected to the recurring nightmare he'd had since childhood. And as he and Tierney both came into the killer's sights, Wade knew he'd face down death to keep this woman by his side.

Book excerpt: Warning Signs provides practical methods to reduce harm to and by children. Doctors Johnson and Berdahl present the most common risk factors and warning signs, along with practical parenting advice and strategies for raising strong, safe kids and protecting them from becoming either perpetrators or victims. They explain how the "old" risk factors—child abuse and neglect, domestic violence, sexual assault, and poverty—have been compounded by new ones in the past 20 years, such as violent media, kids' entitled attitudes, parents' fear of intervention, and increases in childhood mental illness, disrupted families, substance use, bullying, access to weapons, and social media. They provide sample language for tough conversations with kids and with other adults. Full of specific, practical ideas, this book will appeal to parents who want to raise kind and compassionate children.
