Available in PDF, EPUB and Kindle. Book excerpt: Medical and surgical care have made considerable progress in the last two decades in the care of the mother, fetus, and neonate. Most of the improvement in outcomes has been related to advances in diagnostic and therapeutic modalities. These advances have only been possible due to dramatic progress in our knowledge of the physiological mechanisms that determine the normal development of organ systems, as well as the alterations that can lead to disease states and their multifarious consequences. Because improved antenatal care has resulted in the survival of greater numbers of immature infants, they have become a major challenge for clinicians due to the immaturity of multiple organs and their susceptibility to long-term sequelae. The aim of this book is not to address all aspects related to the newborn lung, but to discuss those areas that are more novel or controversial or have been of greater relevance in the progress of maternal, fetal, and neonatal care. Where possible, we have attempted to meet the expectation that practice guidelines be based either on convincing available evidence or on consensus statements of authoritative organizations. Therefore, this book should be useful to obstetricians, perinatologists, and neonatologists.

Book excerpt: Neonatal-perinatal medicine has a strong history of evidence based practice but unfortunately there remain many areas of uncertainty and unproven hypotheses and treatments that may harm our highly vulnerable patients. When new ideas are introduced into the labor and delivery room or the NICU we have to evaluate these procedures and therapies before they become accepted as standards of care. We need to learn from the past lessons of grey baby syndrome and chloramphenicol and of kernicterus and sulfonamide antibiotics where therapeutic good intentions actually did more harm than good and increased the mortality rate of premature babies in the NICU.This proposed edition addresses a broad range of current topics in perinatal neonatal practice. The AAP has just issued new guidelines for the approach to a neonate exposed to maternal genital herpes virus infection. The recommendations were not evidence based and are highly controversial as well as confusing. These topics are addressed in a scholarly and objective manner to both address the controversy and help the practitioner make informed decisions.

Available in PDF, EPUB and Kindle. Book excerpt: This textbook addresses the themes that are at the forefront of neonatal clinical care and research, based on natural divisions in care during pregnancy, and postnatally by gestational age at birth. The book offers a unique approach, in that it proposes discussion of important general principles underpinning neonatal care that are not addressed in most general neonatology textbooks, such as ethical issues, counselling, effective training methods, quality and safety, among other subjects. These are fundamental aspects and challenges that need to be appreciated by senior clinicians. A chapter authored by parents describing their perspectives of neonatal intensive care is unique and will be highly educational, with the potential to influence the way in which individuals view and deliver neonatal care. The authors discuss common and important conditions, to promote adoption of sound evidenced based practice where this is available. However, where evidence is limited, as is the case in many areas of neonatal practice, the authors aim to encourage critical thinking and evidence appraisal, which are necessary skills for busy clinicians wishing to filter evidence to guide delivery of care. This text is suitable for senior trainees wishing to pursue a career in neonatal medicine, early career neonatologists and paediatricians with an interest in neonatology. It is also of interest to established neonatologists wishing to update their neonatal knowledge. The content is based on the RCPCH Level 3 curriculum, and addresses important topical and/or controversial aspects of neonatal care.

Available in PDF, EPUB and Kindle. Book excerpt: This book presents the latest evidence-based guidelines for perinatal management and is designed to help obstetricians and neonatologists minimize complications and offer patients the best possible care. Since 1960, there has been a significant increase in basic and clinical investigations on normal and pathological pregnancy in the developed world. This has provided insights into the physiopathology of pregnant women, fetuses and newborns and led to the development of new technologies, bringing about a new medical subspecialty: perinatal medicine. The book is divided into eight main sections: The first examines basic periconceptional care and discusses the ethical aspects of perinatology. The next section focuses on prenatal considerations, such as the nutritional aspects of gestation and puerperium, physical exercise during pregnancy, routine laboratory tests, prenatal care of multiple gestations and the role of the neonatologist in prenatal care. The third and fourth sections then explore fetal evaluation, and clinical intercurrences in pregnancy, respectively. The next section addresses pregnancy complications: prevention, diagnosis and management. The sixth section covers the basic aspects of congenital infections and the seventh examines labor and delivery aspects. Lastly, the final section includes chapters on neonatal assistance. Written by leading experts in obstetrics, neonatology, and perinatology, this thoroughly updated, comprehensive resource reflects the latest information in all areas, including genetics and imaging.
