Book excerpt: Abstract The historical treatment of African Americans with events such as the Tuskegee experiment and Jim Crow laws combined with distrust of the healthcare system may be affecting health outcomes in African Americans with hypertension. Cultural appropriate education by a provider of the same race is a method that can be utilized to bridge the trust gap between African Americans and providers and assist African Americans in managing their hypertension. A community based culturally appropriate educational intervention by a provider of the same race was implemented over the course of eight weeks to assist African Americans with hypertension to improve their lifestyle behaviors and lower their blood pressure. Twenty African American participants were recruited from a church and within the community to participate. Blood pressures were obtained from participants and they completed the High Blood Pressure Self-care profile (HBP SCP) Behavior Scale pre intervention and post intervention at week nine. Participants were given the culturally appropriate education and a brochure at the start of the project and received a phone call every other week to provide verbal persuasion, motivate, and answer questions. Book excerpt: "Purpose: The purpose of the project is to increase the knowledge level about hypertension (HTN), to increase healthy behaviors, and to decrease blood pressure in adult African Americans through education, behavioral changes. Background: Thirty three percent of adult African Americans have HTN, the highest prevalence of HTN among ethnic groups in the United States. Moreover, there is a high prevalence of undiagnosed and uncontrolled hypertension, especially among minority groups. Data Sources and Outcomes: The Health Promotion Lifestyle II and the American Heart Association Hypertension Quiz, were the tools used to obtain data from the project. A comparison of pre and post score revealed an increase in the HPLP-II score indicated an increase in knowledge. A score for overall health-promoting lifestyle is obtained by calculating a mean of the individual's responses to all 52 items; six subscale scores are obtained similarly by calculating a mean of the responses to subscale items. The use of means rather than sums of scale items is recommended to retain the 1 to 4 metric of item responses and to allow meaningful comparisons of scores across subscales. And the American Heart Association's Hypertension Quiz was scored and evaluated by a Likert scale. Conclusions: There was a significant improvement in the percentage of participant's blood pressure pre stage II (30%) and post stage II group (17%) blood pressures. Pre stage I systolic blood pressures (SBP) decreased from the range of 148-150 to the SBP range 140 post Stage II. A decrease was noticed in the diastolic blood pressures (DBP) range, pre stage I from 94-96 to 86-90 post stage I. The decrease was related to an improvement in medication compliance and self-monitoring of blood pressures (SMBP). Implications for practice: Culturally sensitive education is the key in hypertension education to prevent cardiovascular complications in the adult African American population. Also the church is the cornerstone of the African American community and health care providers should use the church to access this population to improve their health. This understanding should make it easier to get patients to adhere to important medical treatments like hypertension. The purpose of this directed project is to compare the ratings of healthcare provider's cultural competency made by adherent (those who follow their hypertension drug regimen) versus nonadherent patients in middle adult African-Americans who are 35-65-year-olds. The adherent group gave significantly higher ratings for their healthcare provider than the nonadherent group when computing the scores of the Perceived Cultural Competency for their health providers' awareness of their culture as a patient.

Book excerpt: Objective: To review methods that could contribute to improved knowledge of high blood pressure, also known as hypertension (HTN), among African immigrants at a Californian congregation. The difference in the management and outcomes of HTN between African Americans and Whites is one of the highest in cardiovascular diseases (Lackland, 2014). Lack of education is a major contributor to uncontrolled HTN among African Americans, especially among African immigrants (AIs). This brief report will describe a Doctor of Nursing Practice project to address lack of HTN knowledge among AIs. Setting: A church in Southern California. Participants: Twenty adult AIs diagnosed with or self-reported high blood pressure. Methods: Survey questions will focus on country of birth, HTN knowledge, culture, and barriers and enablers of self-management of HTN. As a large part of African culture, HTN education incorporated as storytelling will be used in the delivery of the education. Pre- and post-intervention surveys will be used to assess and evaluate knowledge of HTN. Results: Participants will report HTN knowledge and consequences of uncontrolled HTN before and after the education; as well as barriers to self-care, and facilitators that encourage self-care. Conclusion: Findings may demonstrate the importance of evidence based HTN education among African Americans, especially AIs, in a cultural setting like a church. The findings also may identify the barriers and facilitators and discuss ways to mitigate and utilize them respectively. The result may suggest a need for redesign of HTN education strategies in AIs, including consideration of culture like storytelling.

African Americans differ slightly in culture, health, and migration history from Africans who are 21st century immigrants from Africa (African immigrants); however, there are more similarities than differences between them (Terrazas, 2009). At a family care clinic with a large African American population in Maryland, patients sought help with their hypertension management. An educational program was developed to combat the antihypertensive medication adherence issues among African Americans. Literature review supported an educational intervention to increase medication adherence for African American adults with hypertension. The Rosswurm and Larrabee Model was the guiding framework. The desired outcomes were a) An increased knowledge associated with hypertension medication adherence; and b) Increased adherence to using hypertension medication, which improved patient outcomes. A Hypertension Knowledge Questionnaire was administered. The pre-intervention mean score was 61.875%, and post-intervention mean score was 96.775%. The Hill Bone Medication Adherence Scale was administered to all project participants pre-and post-intervention to assess increased adherence to using hypertension medications. Once all participants' scores were calculated, the scores were aggregated to create a pre-intervention mean (60.925) and post-intervention mean (98.625) for comparative analysis. The post-intervention mean was higher, indicating higher medication adherence upon completion of the education program.
