As a consequence, a wealth of information and novel data has been produced during the last 3-4 years, particularly in the areas of taxonomy, biology, pathogenesis, epidemiology - particularly zoonotic and water borne transmission, and treatment. It is thus very timely to bring together in this book the international research community involved to review the major advances in research and identify the important research priorities for the future, thus enabling as wide an audience as possible to benefit from and share in this comprehensive look at Cryptosporidium and cryptosporidiosis.

The same properties of this protozoan parasite that mean that water is an excellent transmission vehicle are also important for foodborne transmission. These include the low infective dose, the high number of oocysts that are excreted, and the robustness of these transmission stages. However, many more outbreaks of waterborne cryptosporidiosis have been reported than foodborne outbreaks. This is probably partly due to epidemiological tracing being much more difficult for foodborne outbreaks than waterborne outbreaks, and the number of persons exposed to infection often being fewer. Nevertheless, the potential importance of foodborne transmission is gradually being recognized, and a wide range of different foodstuffs have been associated with those outbreaks that have been recorded. Additionally, various factors mean that the potential for foodborne transmission is becoming of increasing importance: these include the growth of international food trade, a current trend for eating raw or very lightly cooked foods, and the rise in small-scale organic farms, where there the possibility for contamination of vegetable crops with animal feces may be greater.

Book excerpt: The enteric protozoans Cryptosporidium spp. and Giardia duodenalis are the most common diarrhoea-causing protozoan parasites worldwide. Cryptosporidium spp. is a leading cause of diarrhoea morbidity and mortality in children younger than 5 years of age in poor-resource settings in sub-Saharan Africa and South Asia. Although infection by G. duodenalis is rarely a fatal condition, giardiasis is commonly associated with childhood growth faltering and cognitive impairment. Because of their significant socioeconomic impact, particularly in low-income countries, giardiasis and cryptosporidiosis joined the "Neglected Disease Initiative" launched by the World Health Organization in 2004. Both Cryptosporidium spp. and G. duodenalis are ubiquitous in the environment and can infect a wide range of hosts with different specificities, meaning that humans may acquire the infection via waterborne, foodborne, or zoonotic transmission. Recent advances in detection and molecular epidemiology have indicated that certain species/genotypes of Cryptosporidium and G. duodenalis have an expanded range of suitable hosts, suggesting that their transmission pathways are more intricate than previously thought, challenging our current notion of host specificities. Therefore, there is a clear need for more studies aiming at investigatingthat aim to investigate the frequency and molecular diversity of these parasites in humans, production and companion animals, and wildlife species. This information would be extremely useful to elucidate the transmission dynamics of cryptosporidiosis and giardiasis, and to investigate the exact contribution of zoonotic events to human infections.
