Book excerpt: Introduction : Dans le contexte démographique actuel des professionnels de santé, nous avons voulu savoir qui assure le suivi gynécologique des femmes et quels critères intervenaient dans leur choix entre gynécologue, médecin généraliste et sage-femme. Méthode : Une étude observationnelle transversale descriptive a été menée auprès de 116 femmes majeures, par le biais de questionnaires distribués dans les cabinets libéraux de la ville de Strasbourg à la fois de sages-femmes pratiquant du suivi gynécologique de prévention, de gynécologues libéraux et de médecins généralistes. Résultats : Parmi les femmes étudiées, 39% sont suivies par leur sage-femme, 31% par leur gynécologue et 30% par leur médecin traitant. Les femmes jeunes consultent préférentiellement une sage-femme, préférant s'adresser à un professionnel de santé de sexe féminin, se sentant plus à l'aise pour aborder leur santé génésique. La sage-femme est également choisie pour la clarté de ses explications et la possibilité d'aborder de multiples sujets avec elle sans tabou. Les femmes choisissant d'être suivies par un gynécologue mettent en avant les compétences de ce dernier ainsi que l'habitude de le consulter. Les femmes préférant consulter leur médecin traitant mettent en avant la rapidité d'obtention d'un rendez-vous et accordent également une plus grande importance au tarif de la consultation. Conclusion : La conjugaison de l'ensemble des acteurs de santé, dont les sages-femmes, doit permettre de faire face à la carence en gynécologue pour offrir un suivi de qualités aux femmes. Ainsi, les sages-femmes vont devoir informer et convaincre les femmes de leurs compétences en matière de gynécologie.

Il a été proposé à 157 patientes ayant récemment accouché à l’hôpital de SENS de répondre à une enquête sur le suivi de leur grossesse. Nous souhaitions connaitre les critères de choix pour le praticien qui les a suivies tout au long de leur grossesse et déterminer l’influence ou non de certaines caractéristiques socio-démographiques. Les femmes choisissaient leur médecin généraliste dans 8,6% des cas, essentiellement pour sa disponibilité et sa connaissance du dossier médical. Deux critères influençaient le choix des patientes : l’âge et la parité. On remarquait également que les patientes confiaient leur grossesse au médecin qui réalisait leur suivi gynécologique habituel. Enfin, cette enquête a mis en évidence l’ignorance des patientes concernant les compétences de leur médecin traitant pour des actes courants de gynécologie-obstétrique. Les médecins généralistes suivent peu de grossesses et réalisent peu de suivis gynécologiques. Ils informent peu leurs patientes sur leurs champs de compétences et proposent rarement de suivre leur grossesse. Les données de santé publique et l’évolution de la démographie médicale incitent à prendre des mesures pour donner au médecin généraliste sa place, entre autres dans le suivi des femmes enceintes.

Notre travail consistera donc à mettre en place un outil d'aide à l'orientation des patientes, lors d'un entretien dédié au moment de leur inscription à la maternité, afin de les guider dans le choix du professionnel responsable du suivi des six premiers mois de leur grossesse. L'outil a été développé selon le guide méthodologique édité par la HAS en 2008 concernant l'élaboration d'un document écrit d'information à l'intention des patients et des usagers du système de santé. Une première version a été établie à partir du rapport de la HAS de 2007 sur le suivi et l'orientation des femmes enceintes en fonction des situations à risque identifiées. Deux groupes de travail pluridisciplinaire constitués de sages-femmes, médecins généralistes et gynécologues obstétriciens ont ensuite permis de l'améliorer. Deux types de suivi ont été définis. Le suivi par une sage-femme ou, un médecin généraliste ou, un gynécologue médical ou obstétricien dans le cas des situations à risque modéré et le suivi par un gynécologue-obstétricien de l'équipe responsable de l'accouchement dans le cas des situations à risque élevé. Chacun de ces suivis pouvant être complété d'un avis spécialisé. Ces avis complémentaires ont ainsi été étayés et il a notamment été précisé à quel moment ceux-ci devaient intervenir. La surveillance des grossesses, essentiellement assurée jusqu'ici par les gynécologues-obstétriciens, devrait évoluer, notamment vers une responsabilité croissante des sages-femmes et des médecins généralistes dans le suivi des grossesses dites physiologiques. Le défi à relever pour tous les professionnels de la périnatalité est donc d'apprendre à travailler en réseau pour un accompagnement à la fois médical, psychologique et social des futurs parents

Matériel et méthodes : Nous avons mené une étude transversale descriptive auprès des femmes asccouchées dans deux maternités du Finistère (une maternité de niveau III d'un CHRU et une clinique de niveau IIa) du 22 août au 14 septembre 2011. Un autoquestrionnaire était proposé aux patientes présentant une grossesse physiologique deux jours après leur accouchement dans le service des suites de couche. Les données collectées concernaient : les caractéristiques sociodémographiques des femmes, le début de grossesse, le suivi prénatal, le déroulement des consultations mensuelles, les connaissances des compétences de la sage femme et le suivi gynécologique. Une compariason des patientes suvies par une sage-femme ou par un gynécologue obstétericien a été établie.Résultats : au total 170 femmes ont accepté de remplir l'autoquestionnaire dont 88 (51.8%) ont accouchés dans la maternité de CHRU et 82 (48.2%) dans la clinique, 138 (81.2%)étaient suivi pour leur grossesse par un gynécologue obstétricien, 28 (16.5%) par une sage-femme et seulement 4 (2.3%) par un médecin généraliste. L'analyse des résultats a mis en évidence la place prépondérante des gynécologues obstétriciens dans le suivi de grossesse physiologique. Le critère de choix principal pour s'orienter vers une sage-femme ou un gynécologue obstétricien est la compétence du professionnel. Les patientes suivies par une sage-femme bénéficiaient plus souvent d'une couverture sociale. Le choix du professionnel a été conditionné par le suivi gynécologique antérieur. Les médecins généralistes font peu de suivi de grossesse mais gardent un rôle important dans le bon déroulement de la grossesse en intervenant pour le diagnostic de grossesse et en dehors des consultations prénatales. Conclusion : Au terme de ce travail nous constatons une méconnaissance des patientes sur les compétences des sages-femmes. Une difffusion plus large des compétences de la sage-femme, ainsi qu'une collaboration accrue entre professsionnels dans le suivi de grossesse physiologique s'appuyant sur les recommandations de la Haute autorité de Santé pourrait peut-être permettre d'augmenter le nombre de suivi de grossesse effectué par les sages-femmes.
