The population sample was non-clinical, drawn from The Chicago School of Professional Psychology. All volunteers completed the Beck Anxiety Inventory (BAI) and the Beck Depression Inventory-II (BDI-II) to exclude participants with mild and severe symptoms of anxiety and depression. A baseline IVA+Plus and POMS assessed for attention and mood prior to any CES treatment. Participants were control-matched to a sham-CES (placebo) condition or treatment-CES condition, while controlling for female participant's phase in menstrual cycle. After a one-hour application of CES at 100 uA, 0.5 hz, participants were administered a second IVA+Plus and second POMS. Results show a statistically significant increase in auditory attention for the treatment group as compared to the placebo group. This research may have significant implications for the use of CES to enhance attention.

Book excerpt: This is a double-blind, placebo-controlled, exploratory study examining the efficacy of Cranial Electrotherapy Stimulation (CES) in the treatment of depression. Twenty participants who met criteria were selected from a rural community. Ten were randomly assigned to the active/experimental group, and 10 were randomly assigned to the placebo/sham group. A pre-test Beck Depression Inventory-II (BDI-II) was administered prior to administration of CES to measure baseline of depressive symptoms. The study protocol consisted of 21 consecutive days of treatment with the participants coming to the Research Center for administration of CES. The devices used in this study were supplied by Electromedical Products International, Inc. (EPI) and were locked at 0.002 amperes with the timer set at 60 minutes. The outcome measure for this study was the self-administered Beck Depression Inventory-II (BDI-II). Following the three-week protocol, the post-test scores on the BDI-II were significantly lower for the active/experimental group when compared to the pre-test scores. Likewise, the post-test scores on the BDI-II were significantly lower for the placebo/sham CES group compared to the pre-test scores. As there was no statistically significant difference between the post-test scores for the active/experimental group versus the placebo/sham CES group, the efficacy of CES cannot be confirmed by the present study. Limitations of the study were small sample size and failure to include a second, untreated group which may have contributed to obscuring the effects of CES that may have been prevalent.

Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.

Although the use of anxiolytic medication reduces the anxiety associated with surgical procedures, the need is not ubiquitous and the side effects are often undesirable. As minimally invasive procedures are utilized, the necessity of anesthesia for patients has been replaced by the need for temporary relief of anxiety and pain. Cranial electrotherapy stimulation, the passing of electrical currents transcranially using ear clip electrodes, has been established for use of a regular basis for anxiety in psychiatric and substance abuse populations. However, its effectiveness when used as a one-time intervention to decrease state anxiety has only recently been examined in several studies, however; the studies have lacked power. This study contributes to the developing body of knowledge regarding the one-time use of CES prior to and during interventional pain management procedures. This study aimed to assess the efficact of a single, cranial electrotherapy stimulation intervention to reduce anxiety prior to and during fluoroscopically-guided spinal and pelvic injections for pain management.

Book excerpt: The caregiver population has recently been recognized in society as a population highly susceptible to problems related to increased anxiety, burden, and abnormal psychological well-being. This experimental pretest post-test 2-group double-blind study was designed to measure the efficacy of cranial electrotherapy stimulation (CES) on anxiety and depressive symptoms of caregivers. Changes in anxiety and depressive symptoms were identified using pre- and posttest measures of State-Trait Anxiety Inventory and Beck Depression Inventory. Caregivers were assigned a pre-coded CES device. Neither the researcher nor the caregiver knew whether a device was active or sham until the completion of the entire study. This experimental design used a repeated measure t-test for quantitative statistics. Following an analysis of the data, the researcher’s hypotheses that CES would help reduce symptoms of anxiety and depression more significantly than a placebo were not supported. Both the treatment and control groups experienced decreased anxiety and depressive symptoms, but the treatment group’s decrease was not significantly greater than the control group’s decrease.

The treatment consisted of half-hour sessions given five days per week during a four-week period. The variables analyzed included age, socio-demographic status, duration of alcohol dependence, frequency and volume of consumption, craving, and duration of sleep. Michigan Alcoholism Screening Test (MAST), Alcohol Dependence Scale (ADS), Drinking Behavior Inventory (DBI), Symptoms Checklist-90 Revised (SCL-90R), Hamilton Psychiatric Rating Scale for Depression, Hamilton Anxiety Scale and Diagnostic Interview Schedule concerning organic brain syndrome, were used. Laboratory tests included serum Gamma-Glutamyl Transferase (GGT) and Mean Corpuscular volume (MCV). Significantly greater improvements were obtained in the active treatment group in the depression subscale and in the Positive Symptoms Distress Level of SCL-90R. The active stimulation reduced alcohol consumption significantly during weekends, but no significant difference in the average weekly consumption was observed between active and sham treatment groups. Consistently more favorable results were observed in the active treatment group in other dependent variables." --

Book excerpt: Cranial electrical stimulation (CES) is a non-invasive method of applying low-intensity electrical current to the head. It is related to but distinct from other forms of transcranial electrical stimulation including electroconvulsive therapy, transcranial direct current stimulation (tDCS), and high-definition transcranial direct current stimulation. The different versions of transcranial electrical stimulation vary in the placement of electrodes, the intensity of the current, and the waveform of the current. According to Guleyupoglu and colleagues, CES evolved from the concept of "electrosleep," first investigated at the beginning of the 20th century. Most of the early research and applications occurred in Russia. Beginning in the 1960s, the concept of electrosleep became more popular in the USA. Because of the belief that the treatment did not actually induce sleep, but rather the sleep was a side effect of the relaxing effect of the current stimulation, the name was changed from "electrosleep" to "cranial electrical stimulation." Other proposed names, which have not persisted, included "transcerebral electrotherapy" and "NeuroElectric Therapy." The latter is noteworthy because it gave its name to an early CES device, the Neurotone 101, which was the first device approved by the FDA. All subsequent CES devices have been cleared for marketing by FDA based on the concept of claiming equivalency to the Neurotone 101. The status of cranial electrical stimulation devices and FDA regulation remains a matter of some controversy.
