Book excerpt: Le développement de la compétence à poser un jugement clinique est essentiel pour l'ensemble des infirmières de soins critiques afin qu'elles offrent des soins sécuritaires et de qualité. Pour ce faire, l'American Association of Critical Care Nurses (2019) recommande l'intégration d'activités d'apprentissage coopératif au sein des programmes de formation continue. Ces activités seraient bénéfiques pour les infirmières novices, mais les connaissances sur le processus d'apprentissage à l'occasion de ces activités et leur apport pour les infirmières expertes demeurent limitées. Présentée sous forme de manuscrit, cette étude qualitative descriptive avait pour but d'explorer, selon la perspective d'infirmières expertes en soins critiques, la contribution d'activités d'apprentissage coopératif à leur compétence à poser un jugement clinique. Elle visait à comprendre le processus et les caractéristiques de ces activités d'apprentissage. Le cadre de référence combinait le modèle du jugement clinique de Tanner (2006) et le modèle d'apprentissage coopératif de Slavin (2014). Des entrevues semi-dirigées ont été réalisées auprès de sept expertes en soins critiques ayant fait l'expérience d'activités d'apprentissage coopératif avec des infirmiers et des infirmières de différents niveaux de développement. L'analyse des données s'est inspirée de l'analyse thématique de Braun et Clarke (2006). L'analyse des données a révélé quatre thèmes. Les deux premiers correspondent aux caractéristiques des activités soit « l'établissement d'un environnement sécuritaire favorise la participation » et « l'hétérogénéité du groupe d'apprenants favorise les échanges ». Les deux autres correspondent aux mécanismes lors des activités : « comparer son raisonnement clinique afin de le remettre en question et prendre conscience de ses automatismes » et « expliquer son raisonnement clinique pour faire l'état de ses connaissances et les approfondir ». À la lumière de ces résultats, des recommandations ont été élaborées afin d'améliorer les programmes de formation continue offerts aux infirmières, dont les expertes, et du fait même, contribuer au développement de leur compétence à poser un jugement clinique.

Book excerpt: Le réseau de la santé québécois a connu, au cours des vingt dernières années, des transformations majeures qui ont eu des répercussions concrètes sur les soins aux patients. Plusieurs facteurs dont la pénurie de l'effectif infirmier ont souvent été au centre des discussions, des préoccupations et de certaines interventions sans toutefois contrer la dégradation des services sur les unités de soins critiques. Les infirmières, qui oeuvrent actuellement sur ces unités, ont très peu d'expérience et doivent s'appuyer sur une faible proportion d'infirmières expertes. La complexité des soins prodigués sur ces unités est telle qu'elle amène les infirmières y oeuvrant à développer leur compétence et ce, de différentes façons. Reconnaissant que certains facteurs peuvent influencer le développement de la compétence, la recherche proposée a pour but de décrire ceux qui facilitent ou qui contraignent le développement de la compétence des infirmières qui oeuvrent en soins critiques. C'est en utilisant un devis descriptif avec méthodes de collecte et d'analyse de données qualitatives que la recherche a eu lieu. De là, un cadre conceptuel mixte basé sur les savoirs infirmiers de Carper (1978) revisités par Lechasseur, Lazure, et Guilbert (2008) ainsi que sur les stades de compétence de Benner (1995) a été utilisé. Ainsi, c'est à partir d'un échantillonnage non probabiliste par choix raisonné que 10 infirmières (n=10) ont accepté de participer à une entrevue individuelle semi-dirigée. Après avoir fait émerger 448 facteurs, l'analyse et la synthèse des données ont démontré que 27 facteurs individuels, contextuels et organisationnels influencent le développement de la compétence des infirmières qui oeuvrent aux soins intensifs. À la lumière des écrits recensés, quelques facteurs avaient préalablement été exposés mais plusieurs facteurs décrits dans l'étude actuelle ont été explicités pour une première fois. Finalement, des recommandations pour la formation, la recherche et la pratique infirmières sont exposées.

It is now widely recognized that human resources for health (HRH) are a key enabler for the attainment of universal health coverage, and for the achievement of Sustainable Development Goal (SDG) 3 - Ensure healthy lives and promote well-being for all at all ages. As is stressed in the Global Strategy on Human Resources for Health: Workforce 2030, there can be no viable national, or global, health system without an effective health workforce. The Global Strategy, adopted at the Sixty-ninth World Health Assembly in May 2016, challenges the erroneous narrative of health workers as a unit of cost in the production of health. The evidence instead presents an intersectoral agenda on the pre-condition of equitable access to health workers in the attainment of universal health coverage, along with a dynamic labour market understanding of the substantive impact on education, employment, jobs and innovation in the health and social care economy. The Global Strategy, therefore, enables governments and other relevant stakeholders to adopt a holistic, rather than fragmented, approach to ensuring that the health workforce contributes both to improved health and to broader socioeconomic development.
