Book excerpt: Objectif : Une séquence T1 injectée sensible et fiable est nécessaire pour la détection des lésions cérébrales rehaussées de sclérose en plaques (SEP) lors du diagnostic mais aussi lors du suivi de la maladie. Les séquences injectées 3D écho de gradient (GRE) T1 et 3D turbo spin echo (TSE) T1 sont toutes les deux disponibles en pratique clinique et n'ont jamais été comparées concernant la détection des lésions de SEP. Notre objectif était donc de comparer les séquences 3D TSE T1 et 3D GRE T1 après injection de gadolinium pour la détection des lésions cérébrales rehaussées de SEP. Méthodes : Cette étude prospective monocentrique a inclus des patients adultes ayant une SEP selon les critères de McDonald révisés en 2017 et nécessitant une IRM cérébrale injectée, réalisée notamment avec les séquences 3D FLAIR, 3D TSE T1 et 3D GRE T1, entre août 2017 et avril 2021. L'ordre des séquences T1 après injection pour chaque patient a été établi par randomisation. Deux radiologues indépendants ont compté les lésions rehaussées pour les deux séquences T1 injectées qui étaient étudiées séparément de manière randomisée, en aveugle des autres données en dehors du 3D FLAIR et de la séquence étudiée sans injection. Ces lectures ont été comparées à une référence établie par un neuroradiologue expert ayant accès à l'ensemble du dossier clinique et radiologique antérieur et postérieur à l'examen étudié du patient. Le critère de jugement principal était le nombre de patients ayant au moins une lésion rehaussée et le critère de jugement secondaire était le nombre de lésions rehaussées par patient. La sensibilité, la spécificité, les valeurs prédictives positive et négative de chaque lecteur pour chaque séquence ont été calculées. La reproductibilité inter-observateur, le contraste ratio (CR) lésionnel et le contraste sur bruit (CNR) ont été calculés pour les deux séquences. Résultats : Un total de 158 patients a été inclus (âge moyen 40 +/- 11 ans, 95 femmes). Les deux lecteurs ont rapporté significativement plus de patients ayant au moins une lésion rehaussée en 3D TSE T1 qu'en 3D GRE T1 (61/158 (38,6%) versus 48/158 (30,4%) et 60/158 (38,6%) versus 47/158 (29,7%), p

This variant, often in the guise of a chronic progressive myelopathy or, less commonly, progressive cerebellar or bulbar dysfunction, usually responds poorly to corticosteroids and rarely seems to benefit to a significant degree from intensive immunosuppressive treatments. In recent years, most randomized clin ical trials have excluded PPMS patients on two counts. Clinical worsening devel ops slowly in PPMS and may not be recognized during the course of a 2-or 3-year trial even in untreated control patients. This factor alone adds to the potential for a type 2 error or, at the very least, inflates the sample size and duration of the trial. In addition, there is mounting evidence that progressive axonal degeneration and neuronal loss (rather than active, recurrent inflammation) may be important components of the pathology in this form of the disease. Although contemporary trials are evaluating whether PPMS patients may benefit from treatment with the ~-interferons and glatiramer acetate, preliminary, uncontrolled clinical experi ence suggests that the results may not be dramatic.
