Book excerpt: The current study utilized a sample of 138 adolescents from a short-term inpatient chemical dependency recovery hospital. Analyses utilized scales from the Global Appraisal of Individual Needs (GAIN), a semi-structured interview that was administered multiple times over the two years after treatment intake. Analyses examined the persistence of substance abuse, conduct disorder symptomatology, delinquent behavior, and mental distress following treatment. A primary goal of the study was to examine whether adolescents with severe conduct disorder (i.e., those engaging in behaviors causing considerable harm to others) followed different trajectories than their mild/moderate counterparts. This inquiry was predicated largely on research that suggests two different pathways for delinquent behavior, namely life-course persistent versus adolescence-limited offending. Substance abuse was measured in terms of both frequency of use, as well as associated problems. The study also examined the presence of "minimal" (i.e., 10% or less of days) or "non-problem" use (i.e., active users reporting no symptoms of abuse or dependence). Overall, fewer mean substance-related problems were reported at each successive follow-up despite the fact that average frequency of substance use increased steadily from the around one-fifth of days at the 3-month follow-up to nearly half of all days at year-two. Results of multilevel negative binomial regression indicated that estimated number of conduct disorder symptoms changed significantly over time, with estimates for average number of conduct disorder symptoms approximating six at intake, but approaching only one symptom by year-two. Although results indicated that male and female participants differed significantly for mean number of conduct disorder symptoms at treatment entry, both groups reported mean symptoms exceeding the 3 required for diagnosis. By the year-two follow-up, no significant sex differences were demonstrated. The study also examined service utilization in the year following initial treatment exposure. Analyses sought to examine potential relationships between treatment duration or treatment focus (e.g., substance abuse treatment versus mental health services) and outcomes. In general, the results of the correlations and regressions showed that the nature or amount of treatment received failed to predict or account for differences in frequency of substance use, symptoms of mental distress, and conduct disordered or criminal behaviors.

Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. Book excerpt: The purpose of this collection is to provide a forum to integrate pre-clinical and clinical investigations regarding the long-term consequences of adolescent exposure to drugs of abuse. Adolescence is characterized by numerous behavioral and biological changes, including substantial neurodevelopment. Behaviorally, adolescents are more likely to engage in risky activities and make impulsive decisions. As such, the majority of substance use begins in adolescence, and an earlier age of onset of use (<15 yr) is strongly associated with the risk for developing a substance use disorder later in life. Furthermore, adolescent drug use may negatively impact ongoing neurological development, which could lead to long-term cognitive and emotional deficits. A large number of clinical studies have investigated both the acute and long-term effects of adolescent drug use on functional outcomes. However, the clinical literature contains many conflicting findings, and is often hampered by the inability to know if functional differences existed prior to drug use. Moreover, in human populations it is often very difficult to control for the numerous types of drugs, doses, and combinations used, not to mention the many other environmental factors that may influence adult behavior. Therefore, an increase in the number of carefully controlled studies using relevant animal models has the potential to clarify which adolescent experiences, particularly what drugs used when, have long-term negative consequences. Despite the advantages of animal model systems in clarifying these issues, the majority of pre-clinical addiction research over the past 50+ years has been conducted in adult animals. Moreover, few addiction-related studies have investigated the long-term neurocognitive consequences of drug exposure at any age. In the past 10 years of so, however, the field of adolescent drug abuse research has burgeoned. To date, the majority of this research has focused on adolescent alcohol exposure using a variety of animal models. The results have given the field important insight into why adolescents are more likely to drink alcohol to excess relative to adults, and the danger of adolescent alcohol use (e.g., in leading to a persistence of excessive drinking in adulthood). More recently, research regarding the effects of adolescent exposure to other drugs of abuse, including nicotine, cocaine, and cannabinoids has expanded. Therefore, we are at unique point in time, when emerging results from carefully controlled pre-clinical studies can inform the sometimes confusing clinical literature. In addition, we expect an influx of prospective clinical studies in response to a cross-institute initiative at NIH, known as the ABCD grant. Several institutes are enrolling children prior to adolescence (and the initiation of drug use), in order to control for pre-existing neurobiological and neurobehavioral differences and to monitor the age of initiation and amount of drug used more carefully than is possible using retrospective designs.

This document, Treatment of Adolescents With Substance Use Disorders, is a revision and update of Treatment Improvement Protocol (TIP) 4, published in 1993 by the Center for Substance Abuse Treatment (CSAT) of the Substance Abuse and Mental Health Services Administration (SAMHSA). Like TIP 4, this document aims to help treatment providers design and deliver better services to adolescent clients with substance use disorders. In 1992, CSAT convened a Consensus Panel of experts on adolescent substance use disorder treatment to produce guidelines for treatment programs on designing and delivering effective services to adolescent clients. The clients addressed in the TIP included, among others, young people involved with the juvenile and criminal justice systems. CSAT also intended for the Panel's guidelines to help governmental agencies and treatment providers establish, fund, operate, monitor, and evaluate treatment programs for substance-using adolescents. The result of that Panel's work was TIP 4, Guidelines for the Treatment of Alcohol- and Other Drug-Abusing Adolescents. In July 1997, CSAT convened a small Revision Panel to review TIP 4. The Panel recommended changes and developed content for this revised TIP. Since the publication of TIP 4, the understanding of substance use disorders and its treatment among adolescents has advanced. More is known today about the immediate and long-term physiologic, behavioral, and social consequences of use, abuse, and dependency. New research attention has begun to examine the effectiveness of various treatment methods and components that meet the specific treatment needs of substance-using adolescents, and this literature is reviewed. However, the literature is small. Fortunately, a large multisite, national study on the effectiveness of treatment for adolescent substance users is underway with funds from the National Institute on Drug Abuse. CSAT is also conducting studies on adolescents, focusing on marijuana treatment, diversion programs in the juvenile justice system (JJS), and exemplary treatment programs. The field will likely mature greatly by the knowledge advanced from these studies. The structure of the earlier TIP of separate inpatient and outpatient treatment chapters, which represented a continuum of service intensity, was viewed by the Revision Panel to be less central to treatment decisions than a continuum based on the severity of the substance use disorder. This shift in focus better reflects clinical experience, extant treatment research, and the recent changes regarding reimbursement by health care payors for treatment. However, the Revision Panel retained a broad definition of treatment. Treatment is defined in this TIP as those activities that might be undertaken to deal with problem(s) associated with substance involvement and with individuals manifesting a substance use disorder. Although the Panel recognizes that primary or secondary prevention of substance use are included in expanded definitions of treatment, the Panel limited the continuum of interventions to what is traditionally viewed as acute intervention, rehabilitation, and maintenance. The elements of the continuum primarily reflect the treatment philosophies of providers, with less emphasis on settings and modalities. In addition to defining the treatment needs of adolescents and providing a full description of the use of the severity continuum, the Revision Panel focused attention on three common types of treatment for adolescents today: 12-Step-based treatment, treatment in the adolescent therapeutic community, and family therapy. The 12-Step model lies at the heart of many adolescent treatment programs. Therapeutic communities (TCs) are an intensive type of residential treatment that is attracting attention as a preferred approach for substance-using juveniles incarcerated in the justice system. Clinicians have found that effective treatment of the adolescent almost always involves the family, and the effectiveness of family therapy has been documented extensively, particularly among those substance-using adolescents who are normally the most difficult to treat. This revision of the earlier TIP, then, offers guidelines for using the severity continuum to make treatment decisions and for providing three common models of treatment for adolescents with substance use disorders. Recommendations of the Revision Panel, supported by extensive clinical experience and the literature, are summarized below. The organization of this TIP reflects the core facets of initiating, engaging, and maintaining the change process for youths with substance use disorders. Chapter 1 details the scope and complexity of the problem. Chapter 2 covers factors considered in making treatment decisions, and Chapter 3 details the features of successful programs. Chapters 4, 5, and 6 introduce and describe the treatment approaches used in 12-Step-based treatment, therapeutic communities, and family therapy, respectively.
