Great achievements have been made recently in neuroimaging techniques and computer technology for neuronavigation, from frameless, armless systems to robotic microscopes. Contained in the present volume are all the papers presented at the International Symposium on Computer-Assisted Neurosurgery and selected papers presented at the 6th Annual Meeting of the Japanese Society of Computers in Neurosurgery, which were held in Kobe, Japan, on January 24-26, 1997. This volume is a comprehensive description and review of current technical ad vancements in computer-assisted neurosurgery, with a special focus on advanced intraoperative neuroimaging, various neuronavigation system, robotic microscopes, and strategies for preoperative and intraoperative surgical planning using high-power workstations with three-dimensional software. We express our thanks to the contributors for their participation and cooperation, and to Springer-Verlag for personal and technical assistance in publishing this work. We sincerely hope that this volume will contribute to improving neurosurgical technology and outcomes.

Book excerpt: In Computer-Integrated Surgery leading researchers and clinical practitioners describe the exciting new partnership that is being forged between surgeons and machines such as computers and robots, enabling them to perform certain skilled tasks better than either can do alone. The 19 chapters in part I, Technology, explore the components -- registration, basic tools for surgical planning, human-machine interfaces, robotic manipulators, safety -- that are the basis of computer-integrated surgery. These chapters provide essential background material needed to get up to speed on current work as well as a ready reference for those who are already active in the field. The 39 chapters in part II, Applications, cover eight clinical areas -- neurosurgery, orthopedics, eye surgery, dentistry, minimal access surgery, ENT surgery, craniofacial surgery, and radiotherapy -- with a concluding chapter on the high-tech operating room. Each section contains a brief introduction as well as at least one "requirements and opportunities" chapter written by a leading clinician in the area under discussion.

Available in PDF, EPUB and Kindle. Book excerpt: The visualization of human anatomy for diagnostic, therapeutic, and educational pur poses has long been a challenge for scientists and artists. In vivo medical imaging could not be introduced until the discovery of X-rays by Wilhelm Conrad ROntgen in 1895. With the early medical imaging techniques which are still in use today, the three-dimensional reality of the human body can only be visualized in two-dimensional projections or cross-sections. Recently, biomedical engineering and computer science have begun to offer the potential of producing natural three-dimensional views of the human anatomy of living subjects. For a broad application of such technology, many scientific and engineering problems still have to be solved. In order to stimulate progress, the NATO Advanced Research Workshop in Travemiinde, West Germany, from June 25 to 29 was organized. It brought together approximately 50 experts in 3D-medical imaging from allover the world. Among the list of topics image acquisition was addressed first, since its quality decisively influences the quality of the 3D-images. For 3D-image generation - in distinction to 2D imaging - a decision has to be made as to which objects contained in the data set are to be visualized. Therefore special emphasis was laid on methods of object definition. For the final visualization of the segmented objects a large variety of visualization algorithms have been proposed in the past. The meeting assessed these techniques.

Book excerpt: A unique how-to guide for spine surgeons on state-of-the-art computer-assisted navigation and robotic surgery techniques The past decade has seen major advances in image-guided spine surgery techniques, with robotically assisted approaches emerging in the last five years. While early adopters of this technology paved the way for more widespread use of navigated and robotic systems, barriers still exist. Navigation and Robotics in Spine Surgery by master spine surgeon Alexander Vaccaro and esteemed co-editors Jaykar Panchmatia, I. David Kaye, and Srinivas Prasad addresses existing issues such as the perception of increased upfront costs, intrusion on current workflow, and a lack of understanding about the potential ways these technologies can enhance the surgical experience and improve patient outcomes. Organized into six sections, the book starts with evidence-based fundamentals of navigated spine surgery and robotics including discussion of instrumentation and mechanics. Sections 2-5 serve as a surgical handbook for spine surgeons who wish to introduce these technologies into practice or augment their current repertoire with more complex techniques. Topics range from more routine procedures such as navigated and robotic minimally invasive TLIF to complex approaches like intraoperative ultrasound guided intradural spinal tumor resection. The final section looks at future directions and potential new applications for these technologies. Key Highlights An impressive group of international spine surgeons who pioneered navigation and robotic surgery techniques share invaluable tricks of the trade Discussion of fluoroscopy- and intraoperative CT-based platforms, applications for intraoperative sonography, and radiation exposure and minimization strategies Special topics include OR set-up and workflow, surmounting the learning curve, artificial intelligence, and lessons learned from other industries Procedural videos demonstrate the benefits of computer-assisted navigation and robotic techniques This book is essential reading for orthopaedic surgery and neurosurgery residents and spine fellows who wish to learn about and incorporate these technologies into practice. Seasoned spine surgeons seeking to expand the scope of their navigated/robotic practice will benefit from chapters detailing advanced approaches.

This book was released on 2015 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: "This thesis describes a novel computer-assisted method for planning deep brain stimulation surgery. The technique combines the use of multiple MRI contrasts - T1- weighted anatomy, susceptibility-weighted venography and time-of-flight angiography (T1w-SWI-TOF) - and automated risk assessment for planning probe trajectories that avoid key structures including fine vessels that cannot be detected using a standard gadolinium-enhanced contrast MRI protocol. The first aim of this thesis is to simplify the use of multi-contrast T1w-SWI-TOF protocol with the creation of a novel path-planning algorithm that can automatically analyze, within few seconds, thousands of candidate trajectories from brain surface to subthalamic nuclei. This study demonstrates the high aggregative power of the automatic algorithm for simultaneously optimizing multiple surgical constraints and evaluates feasibility of the computed trajectories based on neurosurgeons' feedback. The second aim is to assess the clinical validity of the T1w-SWI-TOF protocol for deep brain stimulation purposes. Results show that trajectories planned by neurosurgeons using T1w-SWI-TOF do not intersect the vasculature visible on standard gadolinium-enhanced T1w MRI. This study also demonstrates that computer-assisted T1w-SWI-TOF path planning can significantly improve the avoidance of finer vessels, comes with minimal effects on the imaging and surgical workflow, and provides a safe cost-effective alternative to injection of gadolinium contrast. The third aim is to improve the segmentation and visualization of the dense vasculature obtained with susceptibility- weighted venography. A new method based on Conditional Random Field (CRF) theory is presented and combines different potentials: appearance, shape, location and data- dependent interaction. Results show that the proposed CRF model yields consistent segmentation of deep and sub-cortical veins, and significantly improves segmentation of superficial veins, which is critical for minimizing risks of post-operative deficits.Overall, this thesis addresses key methodological challenges that will facilitate the application of susceptibility-weighted venography contrast in deep brain stimulation neurosurgery. The computer-assisted planning framework is currently being evaluated prospectively on a small cohort of patients. New extensions to the framework are also being investigated for computer-assisted targeting with multiple active contacts." --
