Book Synopsis Casualty Investigation Code by : International Maritime Organization
Download or read book Casualty Investigation Code written by International Maritime Organization and published by IMO Publishing. This book was released on 2008 with total page 36 pages. Available in PDF, EPUB and Kindle. Book excerpt: The MSC adopted a new Code of International Standards and Recommended Practices for a Safety Investigation into a Marine Casualty or Marine Incident (Casualty Investigation Code). Relevant amendments to SOLAS Chapter XI 1 were also adopted, to make parts I and II of the Code mandatory. Part III of the Code contains related guidance and explanatory material. The Code will require a marine safety investigation to be conducted into every marine casualty involving the total loss of the ship or a death or severe damage to the environment. The Code will also recommend an investigation into other marine casualties and incidents, by the flag state of a ship involved, if it is considered likely that it would provide information that could be used to prevent future accidents. The new regulations expand on SOLAS Regulation I/21, which requires administrations to conduct an investigation of any casualty occurring to any of its ships when it judges that such an investigation may assist in determining what changes in the present regulations might be desirable.

Book excerpt: “By far the most thorough and well-written investigative book on RMS Titanic’s short life and tragic sinking that this reviewer has read . . . fascinating.” —Choice Reviews The sinking of the Titanic on her maiden voyage in April 1912 was one of the defining moments of the twentieth century. Books and films about the disaster that befell the iconic liner are commonplace, and it seems almost inconceivable that anything fresh can emerge. But there is one angle that has not been covered, and Titanic examines the events of April 1912 from that completely new perspective. John Lang brings the standards of a twenty-first-century accident investigation to bear on the events of April 1912, using his expertise and his investigator’s instinct to determine exactly what happened a century ago, and what important lessons still need to be learned. “A fascinating account . . . this book looks at the facts from a new perspective and uncovers some unexpected findings. A worthwhile read.” —Royal Naval Sailing Association “Provides genuine insight into what almost certainly happened before, during and after the ice sliced through the five compartments on the liner’s starboard bow.” —Viewpoint “Lang, with his experience of marine accident investigation has found an angle in examining events of April 1912 from the perspective of an MAIB officer. His analysis is supported by detailed background material enabling the reader to understand the events of the collision and sinking more fully as he brings the standards of a 21st century investigation to bear on the events in determining exactly what happened and why.” —Flash

This book was released on 2014-12-23 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: The course is essentially balanced between theory (lecture material) and practical group exercises and case studies. It consists of a series of exercises structured around the requirements of the international legislation, IMO conventions and instruments, and the required common and homogeneous approach, application of consistent methodology and no blame culture to adopt in the conduct of marine safety investigations into marine casualties and marine incidents; as well as to provide reports to the Organization. The group exercises and case studies are controlled by an instructor and, initially, allow the trainees to become familiar with applicable international legislation and IMO instruments on this matter as well as guidelines and procedures to be used.
Book Synopsis A Human Error Approach to Aviation Accident Analysis by : Douglas A. Wiegmann
Download or read book A Human Error Approach to Aviation Accident Analysis written by Douglas A. Wiegmann and published by Routledge. This book was released on 2017-12-22 with total page 174 pages. Available in PDF, EPUB and Kindle. Book excerpt: Human error is implicated in nearly all aviation accidents, yet most investigation and prevention programs are not designed around any theoretical framework of human error. Appropriate for all levels of expertise, the book provides the knowledge and tools required to conduct a human error analysis of accidents, regardless of operational setting (i.e. military, commercial, or general aviation). The book contains a complete description of the Human Factors Analysis and Classification System (HFACS), which incorporates James Reason's model of latent and active failures as a foundation. Widely disseminated among military and civilian organizations, HFACS encompasses all aspects of human error, including the conditions of operators and elements of supervisory and organizational failure. It attracts a very broad readership. Specifically, the book serves as the main textbook for a course in aviation accident investigation taught by one of the authors at the University of Illinois. This book will also be used in courses designed for military safety officers and flight surgeons in the U.S. Navy, Army and the Canadian Defense Force, who currently utilize the HFACS system during aviation accident investigations. Additionally, the book has been incorporated into the popular workshop on accident analysis and prevention provided by the authors at several professional conferences world-wide. The book is also targeted for students attending Embry-Riddle Aeronautical University which has satellite campuses throughout the world and offers a course in human factors accident investigation for many of its majors. In addition, the book will be incorporated into courses offered by Transportation Safety International and the Southern California Safety Institute. Finally, this book serves as an excellent reference guide for many safety professionals and investigators already in the field.
