Book excerpt: Pre-Accident Investigations: Better Questions - An Applied Approach to Operational Learning challenges safety and reliability professionals to get better answers by asking better questions. A provocative examination of human performance and safety management, the book delivers a thought-provoking discourse about how we work, and defines a new approach to operational learning. This is not a book about traditional safety. This is a book about creating "real" safety in your organization. In order to predict incidents before they happen, an organization should first understand how their processes can result in failure. Instead of managing the outcomes, they must learn to manage and understand the processes used to create them. Ideal for use in safety, human performance, psychology, cognitive and decision making, systems engineering, and risk assessment areas, this book equips the safety professional with the tools, steps, and models of success needed to create long-term value and change from safety programs.

Book excerpt: Many organizations tell us that work has never been as safe as it is today. They will show the lowest injury figures ever, and the rosiest incident counts in years. They want to be proud of these accomplishments, and perhaps they should be. But behind these results hides complexity and contradiction-a messiness that Todd Conklin takes us into with this book. For one, it is pretty obvious by now that trying to lower our incident and injury rates leaves the risk of process safety disasters and fatalities pretty much unaffected. Getting better at managing injuries and incidents doesn't help us prevent fatalities and accidents-we've known that for a long time (Salminen, Saari, Saarela, & Rasanen, 1992). The number of fatalities in, say, construction, or the energy industry, has remained relatively stable over the past decades (Amalberti, 2013; National-Safety-Council, 2004), even when many organizations proudly report entire years (or more) without injury. Lowering the injury or non- serious incident rate can actually put an organization at greater risk of accidents and fatalities. In shipping, for example, injury counts were halved over a recent decade, but the number of shipping accidents tripled (Storkersen, Antonsen, & Kongsvik, 2016). In construction, most workers lost their lives precisely in the years with the lowest injury counts (Saloniemi & Oksanen, 1998). And in aviation, airlines with the fewest incidents have the highest passenger mortality risk (Barnett & Wang, 2000).What lies behind these fatalities? Do they really happen because some people don't wear their personal protective equipment; that some don't wear gloves when rules say they should? WorkPlace Fatalities: Failure to Predict is the first book for the industry professional that speaks directly to this important challenge: If your organization is so safe - Why do we have fatal and serious events?

Book excerpt: To prevent future accidents, a complete examination of the causes and contributory factors of an accident is necessary. Stressing the need to correct these issues, Incident Investigation and Accident Prevention in the Process and Allied Industries strikes a balance between the theoretical and applied aspects of accident investigation while also addressing accident prevention. Based on the authors forty years of experience, this detailed work covers an extensive range of topics often encountered during an incident investigation. Since the scope and needs of investigations can widely vary, the author begins with an introduction that provides guidance on how to use the book. He supplies a "roadmap" of incident investigation, making the material accessible to novices yet also valuable to more seasoned investigators. Topics include: Responding to occurrence of incidents and emergency response Securing the site and handling eyewitness testimonies Notification of appropriate authorities, compliance requirements, and legal and insurance issues Internal/external incident investigation options and investigation team requirements Disassembly, gathering, screening, tagging, storing of evidence, and post-incident testing Establishing preliminary causal mechanisms and root cause determinations Multiple cause determination, incident modeling, and human error evaluation and reporting Remedial and preventative measures, lessons learned, and reconstruction and retraining Accident prevention through predictive methodologies, including pro-active measures Pre- and post-incident management Corporate structuring, attitudinal problems, and planning an accident prevention program

Book excerpt: In this book the author applies contemporary error theory to the needs of investigators and of anyone attempting to understand why someone made a critical error, how that error led to an incident or accident, and how to prevent such errors in the future. Students and investigators of human error will gain an appreciation of the literature on error, with numerous references to both scientific research and investigative reports in a wide variety of applications, from airplane accidents, to bus accidents, to bonfire disasters. Based on the author's extensive experience as an accident investigator and instructor of both aircraft accident investigation techniques and human factors psychology, it reviews recent human factors literature, summarizes major transportation accidents, and shows how to investigate the types of errors that typically occur in high risk industries. It presents a model of human error causation influenced largely by James Reason and Neville Moray, and relates it to error investigations with step-by-step guidelines for data collection and analysis that investigators can readily apply as needed. This second edition of Investigating Human Error has been brought up to date throughout, with pertinent recent accidents and safety literature integrated. It features new material on fatigue, distraction (eg mobile phone and texting) and medication use. It also now explores the topics of corporate culture, safety culture and safety management systems. Additionally the second edition considers the effects of the reduction in the number of major accidents on investigation quality, the consequences of social changes on transportation safety (such as drinking and driving, cell phone use, etc), the contemporary role of accident investigation, and the effects of the prosecution of those involved in accidents.

Available in PDF, EPUB and Kindle. Book excerpt: Considerable effort has been expended particularly over the previous decade in the area of examining pre-crash factors in an attempt at developing appropriate highway safety countermeasures. This report summarizes the review of this field by a select panel of highway safety researchers and includes recommendations for NHTSA-sponsored research in accident causation over the next several years. After attempting to clarify the significance of the “cause” of an accident, the report outlines various approaches to accident causation research and indicates the relevant data needs. The extent and type of exposure information required is addressed. A detailed critique of the major research efforts in this field is provided. This critique centers mainly on the work done by Indiana University, Cornell Aeronautical Laboratory, Inc., Operations Research, Inc., and the University of Miami. Finally, short and long-range recommendations for NHTSA-sponsored research are presented. These include in general utilizing the NASS system to the fullest extent possible. Several specific studies recommended include: examination of data needs; survey of the possible existence of this required data; exploration of various alternative approaches to data collection (e.g. simulation); establishing appropriate quantifiers (e.g., what defines “following too closely”); determining exposure requirements; upgrading the statistical techniques utilized in this field; ascertaining the effectiveness of various countermeasures; and examining alternative information retrieval systems. In addition, several small open-ended research contracts for just plain “thinking about” accident causation methodology were recommended.

Available in PDF, EPUB and Kindle. Book excerpt: This new edition of a standard in the field is the most complete treatment available on modern methods of accident investigation. The investigation process is divided into three phases: preparation and planning, analytical methods and reporting, and corrective actions designed to prevent recurrence. Techniques discussed are general and can be applied to a wide range of industrial accidents. Topics covered include investigation concepts, the pitfalls of government intervention, legal aspects, multilinear events sequencing, and management oversight and risk tree (MORT). There is new material on the electronic and computer industries and on S-T-E-P accident investigation. A new chapter, ``A Generic Approach to Mishap Investigation,'' puts the entire process in perspective.

Book excerpt: The National Transportation Safety Board (NTSB) bears a significant share of the responsibility for ensuring the safety of domestic and international air travel. The NTSB relies on teamwork to resolve accidents; the parties that participate in an investigation may include manufacturers and operatiors, as well as the Federal Aviation Administration. This arrangement works well under most circumstances, despite inherent conflicts of interest may jeopardize, or be perceived to jeopardize, the intergity of the NTSB investigation. The NTSB's ability to lead investigations and to form expert teams is aldo seriously threatened by a lack of training, equipment, and facilities; by poor control of information; and inadequate aids to project management.
