Fast Monte Carlo algorithms are accurate in general but their clinical implementation needs validation. Calculations of electron beam dose distributions performed using the fast Monte Carlo system XVMC and the well-benchmarked general-purpose Monte Carlo code EGSnrc were compared with measurements. Irradiations were performed using the 9 MeV and 15 MeV beams from the Clinac 18 accelerator with standard conditions. Percent depth doses and lateral profiles were measured with thermoluminescent dosimeter and electron diode respectively. The accelerator was modelled using EGS4/BEAM, and using an experiment-based beam model. All measurements were corrected by EGSnrc calculated stopping power ratios. Overall, the agreement between measurement and calculation is excellent. Small remaining discrepancies can be attributed to the non-equivalence between physical and simulated lung material, precision in energy tuning, beam model parameters optimisation and detector fluence perturbation effects." --

Book excerpt: IntroductionIn PT, dose recomputation using an independent Monte Carlo (MC) dose engine is often suggested as an important feature of a modern quality assurance strategy. However, an accurate beam data library (BDL) must be commissioned in the MC engine to ensure fair comparisons with the treatment planning system (TPS).We propose a method to automatically commission the fast MC code MCsquare, recently released open source.Materials and methodsThe BDL of MCsquare requires energy-dependent parameters that describe 1) optical properties (beam divergence, spot size, correlation), 2) the energy spectrum, and 3) the number of protons per MU for absolute dose computation. This BDL is optimized from measurements acquired during TPS commissioning.Optical parameters are retrieved by fitting beam divergence and spot sizes according to the Courant-Snyder theory, considering measured spot fluences as 2D Gaussians. The mean energy and its standard deviation are optimized to reproduce measured depth dose profiles, using a Nelder-Mead method.Finally, the obtained BDL is validated by comparing simulations of various plans (QA SOBPs) to corresponding phantom measurements.ResultsRegarding the validation, 92% of the simulated plans lead to a range difference below 1 mm while 93% show an accuracy in absolute dose better than 1%.ConclusionsWe propose an automatic way to process measurements data in order to commission MC algorithms. The method provides good results and is independent of the treatment machine.

Book excerpt: The purpose of this thesis is to validate the Monte Carlo algorithm for electron radiotherapy in the Eclipse[TM] treatment planning system (TPS), and to compare the accuracy of the Electron Monte Carlo algorithm (eMC) to the Pencil Beam algorithm (PB) in Eclipse[TM] Dose distributions from GafChromic[TM] EBT3 film measurements were compared to dose distributions from eMC and PB treatment plans. Measurements were obtained with 6MeV, 9MeV, and 12MeV electron beams at various depths. A 1 cm thick solid water template with holes for bone-like and lung-like plugs was used to create assorted configurations and heterogeneities. Dose distributions from eMC plans agreed better with the film measurements based on gamma analysis. Gamma values for eMC were between 83%-99%, whereas gamma values for PB treatment plans were as low as 38.66%. Our results show that using the eMC algorithm will improve dose accuracy in regions with heterogeneities and should be considered over PB.

However, there is no guidance on how to commission Monte Carlo Dose Calculation (MCDC) algorithms for Pencil Beam Scanning (PBS) using GBD. Methods: A GBD framework was developed to commission MCDC algorithms of RayStation 8A and Eclipse AcurosPT v13.7.20 in water and heterogeneous phantoms. Measurements included Bragg peaks and profiles of PBS single-spots and PBS field outputs for Varian ProBeam. The phase parameters, numbers of protons per MU were obtained from in-air measurement and PBS outputs of 100 cm2 square fields at 2 cm depth, while spot profiles and more PBS fields at more depths were used to validate TPS.Results: The maximum differences of phase parameters spot sigma and divergence between MCDC algorithms are below 4 u00b5m and 0.26 mrad in air, respectively. Comparing TPS to measurement at depths, both MC algorithms predict the spot sigma within 0.5 mm, the resolution of measurement device. AcurosPT is found to underestimate numbers of protons per MU by ~2% and requires user adjustment to match measurement, while RayStation is within 1% of measurement using Auto model. Site-specific gamma criteria vary from 2 mm/2% to 5 mm/5% with comparing TPS to exit dose measurement.Conclusion: The proposed GBD framework can detect potential issues during PBS beam data collection or TPS commissioning processes. Therefore, it can shorten commissioning time and improve dosimetric accuracies. Secondary MCDC can be used to identify the root sources of disagreement between primary MCDC and measurement.

Book excerpt: In order to deliver MERT plans efficiently, we have designed and manufactured a prototype EMLC based on the results of Monte Carlo simulations and a Varian 25 x 25 Cm to the 2 electron applicator. The performance of the EMLC was experimentally evaluated and the results were compared with those of Monte Carlo simulations and a PMLC. We have also developed a complete system of Monte Carlo-based inverse treatment planning system for MERT plans. The system incorporates the Monte Carlo simulated electron pencil beams into the inverse planning process. Additionally, we have characterized the EMLC using a multiple-source beam model for fast Monte Carlo simulation, incorporated the EMLC into the EGS/BEAM simulations as an MLC component module, and created MERT plans for breast cancers. We have also studied the effect of beam delivery on optimized modulated electron radiation therapy plans in the final dose calculation. Furthermore, we have developed aperture-based MERT techniques for fast delivery of MERT plans. We believe that MERT provides a viable alternative modality to conventional Opposed tangential fields approach and photon IMRT.

Taking advantage of recent advances in low-cost computer hardware, modern multiprocessor architectures and optimized Monte Carlo transport algorithms, PEREGRINE performs mm-resolution Monte Carlo calculations in times that are reasonable for clinical use. PEREGRINE has been developed to simulate radiation therapy for several source types, including photons, electrons, neutrons and protons, for both teletherapy and brachytherapy. However the work described in this paper is limited to linear accelerator-based megavoltage photon therapy. Here we assess the accuracy, reliability, and added value of 3D Monte Carlo transport for photon therapy treatment planning. Comparisons with clinical measurements in homogeneous and heterogeneous phantoms demonstrate PEREGRINE's accuracy. Studies with variable tissue composition demonstrate the importance of material assignment on the overall dose distribution. Detailed analysis of Monte Carlo results provides new information for radiation research by expanding the set of observables.

The most promising avenue towards this goal is the Monte Carlo technique, whereby particle trajectories are simulated from a modelled radiation source into a patient volume. Following the trend of incorporating fast Monte Carlo algorithms in clinical electron beam dosimetry, Elekta CMS inc. commercialized an electron Monte Carlo (eMC) module implemented in the treatment planning system XiO. The algorithm is based on the XVMC code and allows for fast calculations in voxelized heterogeneous media. This thesis reports its validation as a first step towards clinical implementation for an Elekta SL25 linear accelerator. Simulations with the eMC module were performed for 12 MeV beams in different CT scanned phantoms and beam setups. Concurrently, GafChromic® EBT3 film measurements were performed and the two dose distributions were compared, using mainly three-dimensional gamma agreements. Film measurements were analysed through a novel method using the triple channel dosimetry function of Film QA Pro, thus yielding absolute dose readings with an uncertainty of ±1.5% over the range of [75, 700] cGy. The results indicate that beam models generated by CMS should be validated prior to any more complex investigation as they strongly impact on the accuracy of dose distributions. Provided that the beam models are valid, the comparisons are very promising : a 2%/3 mm agreement was found between measurements and calculations at different depths in a three-dimensional heterogeneous phantom." --

Book excerpt: Purpose: To assist 4D treatment planning for spot-scanned-delivered protons in the presence of respiration by developing Monte-Carlo based 4D packages for dose calculation and robust optimization.Methods: A GPU-based platform was created to calculate 4D dose from an input treatment plan to assess the planu2019s stability under patient breathing as characterized by 4DCT. The 4D calculation began with a beam-delivery simulation which divided the planned spots across the breathing phases. An in-house Monte Carlo dose engine was then used to calculate sub-doses on each of the breathing phases. The sub-doses were then summed onto the end-exhalation phase (EE) using deformable-image-registration based accumulation techniques. The beam-delivery simulation and dose-accumulation algorithms were incorporated into a robust optimization engine. The doses from potential spots were calculated and deformed to the EE before being accumulated as a weighted sum, with the weights dictated by beam-delivery simulations. The 4D doses were affected by beam-delivery uncertainties due to variable breathing and delivery parameters; these uncertainties were included in the optimization algorithm as robust scenarios in addition to range and setup uncertainties.Results: The uncertainties from the beam-delivery simulation led to moderate variations in the dose-volume histograms of clinical target volumes (CTVs) as seen in Figure 1 from the calculator. The optimizer converges, as in Figure 2, even with updating the phase weighting periodically to account for updated spot weights.Conclusions: A 4D dose calculator and robust optimizer were developed based on Monte Carlo calculated dose, realistic simulations of the proton delivery, and deformable-image-registration based dose accumulation.
