Book excerpt: Diabetes mellitus (DM) is a chronic metabolic disorder manifested by an enhanced blood glucose levels. DM is categorized into type 1 diabetes and Type 2 diabetes. Metformin is an anti-diabetic medication that is related to the biguanide class. Metformin is the first treatment for Type 2 DM because of its efficacy, safety and minimal risk of hypoglycemia. Sulfonyl ureas namely glipizide and glyburide activate insulin secretion from pancreatic beta cells. Meglitinides, like nateglinide and repaglinide also activate insulin secretion, but their action is short - lived compare to sulfonyl ureas. Thiazolidine diones (TZDs) along with pioglitazone and rosiglitazone activate insulin sensitivity particularly in peripheral tissues. They stimulate peroxisome proliferator - activated receptor gamma (PPAR - gamma ) and give rise to an improved glucose utilization and decreased insulin resistance. Dipeptidyl peptidase - 4 (DPP - 4) inhibitors namely saxa gliptin and sita gliptin continue the action of incretin hormones. Inretin activate insulin release and stop glucagon secretion and contribute to glucose control. Sodium - glucose c0-transporter - 2 (SGLT 2) inhibitors along with canagliflozin and dapagliflozin influence kidneys to decrease glucose reabsorption. Alpha-glucosidase inhibitors namely acarbose, decrease digestion and absorption of carbohydrates in the intestine. Finally it is concluded that the management of diabetes is associated with multifaceted approach, often combining lifestyle modifications and pharmacotherapy.

Book excerpt: Acute and chronic hypoxia results in unconsciousness. Brain death happens immediately without proper treatment. Hypoxia activates Juxta glomerular (JG) apparatus of kidneys and enhances the secretion of erythroietin which in turn activates the red bone marrow to enhance the RBC count. Hypoxia enhances the rate and force of contraction of heart, cardiac output (COP) as well as blood pressure. Hypoxia enhances the respiratory because of the stimulation of chemoreceptor reflex.

Book excerpt: Antiarrhythmic drugs play a key role regarding abnormal heart rhythms. Anti-arrhythmic drugs are divided into different classes dependent on their primary mechanism of action. These drugs are regarded as into four main categories such as class I (sodium channel blockers), class II(beta _ blockers), class III (potassium channel blockers) and class IV (calcium channel blockers). Class I anti arrhythmics specifically act on sodium channels, which play a role in the initiation and conduction of action potentials particularly in Cardiac cells. Class II anti arrhythmics act by decreasing the effects of adrenaline on the heart. Class III antiarrhythmics affect potassium channels and carry on the repolarization phase of the Cardiac action potential. Potential class IV antiarthmics stop calcium influx during depolarization and result in acdecrement in the heart rate and the forcec of contraction. Finally it is concluded that anti arrhythmic drugs play a critical role in managing various vtyoes of arrhythmias.

Book excerpt: Alzheimers disease is a neuro degenerative disorder. Medications namely done penile, galantamine and rivastigmine increase the levels of acetyl choline, a neurotransmitter that plays a critical role particularly in memory and learning. By stopping the enzymes that break down acetyl choline, these drugs increase communication between nerve cells in the brain. NMDA receptor antagonist that is meantime is also used in the treatment of Alzheimers disease.

Meanwhile, the practice of anesthesiology has expanded to the preoperative environment and numerous locations outside the OR. Anesthetic Pharmacology: Basic Principles and Clinical Practice, 2nd edition, is an outstanding therapeutic resource in anesthesia and critical care: Section 1 introduces the principles of drug action, Section 2 presents the molecular, cellular and integrated physiology of the target organ/functional system and Section 3 reviews the pharmacology and toxicology of anesthetic drugs. The new Section 4, Therapeutics of Clinical Practice, provides integrated and comparative pharmacology and the practical application of drugs in daily clinical practice. Edited by three highly acclaimed academic anesthetic pharmacologists, with contributions from an international team of experts, and illustrated in full colour, this is a sophisticated, user-friendly resource for all practitioners providing care in the perioperative period.

Book excerpt: This textbook provides a concise and thorough introduction to clinical appraisal pharmacology from which students can gain a rapid understanding of individual topics within the discipline, and from which they can review and revise certain topics before exams. This is achieved through self-contained topics, with concise text on the left and a diagram on the right to elaborate and reinforce the text. The text has a strong clinical focus and, in the main, sections will relate to particular body systems or the therapy of a disease-class (eg, cancer). Pharmacological principles and mechanisms are covered, but the main focus is on the therapeutic use of the drugs. Sections will cover basic principles of pharmacology, specialized pharmacology, complementary pharmacology, and the vitamins. The text will be written primarily for pharmacy students, but will also be of interest to clinical medical students. The strong clinical focus should also appeal to practitioners.
