Book excerpt: Excerpt from Clinical Lectures on Stricture of the Urethra and Enlargement of the Prostate The immediate treatment of retention of urine arising from either of these causes is practically the same. It con sists in clearing out the lower bowel at once by an enema, placing the patient in a hot bath, and giving opium by the mouth, or morphia hypodermically. Should these measures not suffice, a soft olivary catheter of medium size, 8 or 9 of the English scale, should be passed slowly and gently, or a silver catheter if necessary. If there be any difficulty in introducing the catheter, the patient should be anaesthetised, when the spasmodic element will entirely disappear and the instrument, as a rule, pass readily. Organic stricture may be defined as a permanent diminution in the calibre of the urethra, due to the development of organized lymph or scar tissue in its walls. I had not intended issuing a new edition of this work in separate form, as, at the invitation of the Publishers, I am engaged in preparing a more or less comprehensive work on 'The Surgical Diseases of the Urinary Organs.' Owing, however, to professional demands on my time, this work cannot appear as early as I anticipated. In the meantime I find that there is a somewhat pressing demand for the lectures on the prostate. I have, therefore, decided on issuing forthwith the lectures on the prostatic portion of the work in separate form. Consequent on the introduction of my operation of total enucleation of the enlarged prostate, the original lectures, which were delivered in November, 1900, at the Medical Graduates' College and Polyclinic, have been entirely re-written and amplified. Excerpt: ...--Treatment--Longitudinal Wounds Of The Urethra. Injuries to the urethra not only expose the patient to the risk of retention and extravasation of urine, but are usually followed by a stricture of the worst form. In a seaport like this a very considerable proportion of the stricture cases are met with amongst its sailor population, and of these, a large number I have observed are traceable to injuries received in the course of their employment. Such, for instance, as blows on the perinasum, by falls over ropes and spars, where the urethra is violently stretched or contused against the pubic arch. A contusion of the perinaeum, without any obvious tearing of tissue, is quite sufficient to occasion retention, and require the use of the catheter; but inasmuch as the urethra escapes without laceration, the inconvenience is a temporary one, and the formation of a stricture need not be apprehended. There was a patient not long ago in one of my wards who was injured in this way. He had fallen a considerable height over some railings, alighting upon his perinaeum. On admission into the Infirmary some hours after, the perinaeum and scrotum were very much swollen and ecchymosed. Though he could not pass water, yet there was no difficulty in introducing a catheter into the bladder. Nothing like a rent could be felt, and no blood appeared to have escaped from the orifice of the urethra. It was a case of contusion with very considerable ecchymosis, and after the lapse of forty-eight hours no further catheterism was required. Had there been evidence that the urethra was torn, it might have been necessary, as I shall presently state, to adopt a very different treatment.
