Book excerpt: Background: Periodontal disease is a complex, multifactorial infection in which the interactions between bacteria and the host response contribute to the destruction of the periodontium. Routine clinical measures cannot accurately identify those sites at risk, as current measures are retrospective. This problem has led to the search for a biomarker of periodontal disease activity. The identification of these biomarkers may lead to the earliest possible identification of the disease process allowing the most effective preventive procedures to be used. Aims: To investigate the presence and quantities of the soluble scams - sE-Selectin, sICAM-1 and sVCAM-1 as well as the cytokines IL-1 a̲nd IFN? in gingival crevicular fluid (GCF) of patients with adult periodontal disease and patients without periodontal disease (controls). Also, to study the relationship between the levels of these sCAMs and cytokines in the healthy and diseased sites within and between the groups before and after treatment. Materials and Method: Each site selected for GCF sampling was classified clinically as either healthy, gingivitis or periodontitis. The volume of GCF collected from each site was measured using a Periotron 8000. ̣The presence and quantity of the five inflammatory mediators in the GCF was investigated using enzyme linked immunosorbent assays. Results: The results obtained in this study show that multiple molecules can be assayed in small GCF volumes collected from individual crevicular locations. Statistically significant results were obtained when the cytokines and scam levels from healthy and diseased patients were compared. Conclusions: These results indicate that there are differences in the levels of these inflammatory mediators in periodontal health and disease. Changes in the levels of these cytokines and scams may be a sensitive indicator to differentiate healthy sites from those with periodontal disease. Further work is required to establishment if these biomarkers will enable the identification of those sites most at risk for disease progression and thereby play a preventive role in the pathogenesis of periodontal disease.

Abstract: Abstract Abstract The aim of this study was to evaluate the adjunctive effect of a low-power laser in non-surgical periodontal treatment of patients with moderate to advanced chronic periodontitis. The study was performed as an intra-individual, longitudinal trial of 9 months duration with a single-blinded, controlled and randomized design. A total of 14 patients, 13 females and 1 male, aged from 33 to 57 years (mean = 43.6) having at least two sites per quadrant with probing pocket depth (PPD) >= 5mm were recruited. Matched sites with PPD of 5mm or above and comparable radiographically detected bone defect were selected on each side of the mouth in each patient. Clinical parameters, including presence of supragingival plaque (PI%), bleeding on probing (BOP%), probing pocket depth (PPD) and probing attachment level (PAL) were recorded at baseline, 3 months, 6 months and 9 months after treatment. Gingival crevicular fluid (GCF) samples and standardized intra-oral radiographs for evaluation of alveolar bone changes by digital subtraction radiography were also taken at the baseline and after 1 month, 3 months, 6 months and 9 months. After initial examination, full-mouth supra- and sub-gingival debridement was performed and oral hygiene instructions were given. Test sites were selected randomly by a coin toss and irradiated by a low-power laser (He-Ne) operating at a wavelength of 632nm and an output power of 0.2mW while control sites received no additional treatment. Laser irradiation was applied directly to the buccal or palatal aspects of the selected sites, each time for ten minutes for a total of eight times in the first 3-month period by a trained operator.
