Zum Schluss sollen die Ergebnisse kurz zusammengefasst und die Intentionen des Case Management im Krankenhaus dargestellt werden. Ein wichtiges Instrument der Sozialen Arbeit ist das Case Management. Es ist für die Betreuung von Einzelfällen unentbehrlich, da es die Zusammenarbeit mit anderen Berufsfeldern erleichtert, was bei der Arbeit im Krankenhaus eine große Rolle spielt. Heutzutage ist unser Gesundheitswesen gekennzeichnet durch eine bunte Versorgungsstruktur und Trennung von sozialen, medizinischen und pflegerischen Diensten. Dies hat in der ambulanten Versorgung zu einer mangelhaften Abstimmung zwischen den einzelnen Bereichen und zu einer Unübersichtlichkeit geführt. Um dieser Unübersichtlichkeit entgegenzuwirken, sind in den letzten Jahren die Bewegungen des Case Management entstanden.

In dem Kapitel „Entwicklung der Krankenhaussituation in Deutschland“ beschreibe ich die demographische Entwicklung und die Einführung eines neuen Vergütungssystems (Diagnosis Related Groups System), welche meines Erachtens die Hauptursachen der Veränderungen der heutigen Krankenhauslandschaft darstellen. Das nächste Kapitel „Spannungsverhältnis zwischen Wirtschaftlichkeit und Qualität“ widme ich Qualtiätsdimensionen und zwei von mir ausgewählten Qualitätsinstrumenten bzw. –verfahren, namentlich klinische Behandlungspfade und das Zertifizierungsverfahren nach KTQ. Damit möchte ich zeigen, wie sich die Krankenhäuser durch diese Verfahren verbessern wollen bzw. können, wobei ich gleichzeitig auf die Schwachstellen dieser Verfahren aufmerksam mache. Weiterhin wird hier die Prozessoptimierung als eine der wichtigsten Aufgaben der Krankenhäuser in Deutschland beschrieben. In dem Kapitel „Lösungsansatz Case Management“ beschreibe ich die historische Entwicklung von Case Management, seine Ablaufphasen und seine klassischen Funktionen. Schließlich lege ich besonderes Augenmerk auf die Funktionen des Case Managers im Krankenhaus und die Aufgaben, welche er zu bewältigen hat. Im letzten Kapitel „Chancen und Einsatz des Case Managements im Krankenhaus“ möchte ich die Potentiale des Case Managements aufzeigen und der Frage einer sinnvollen Positionierung des Casemanagers innerhalb der Krankenhaushierarchie nachgehen.

Beyond the Checklist argues that lives could be saved and patient care enhanced by adapting the relevant lessons of aviation safety and teamwork. In response to a series of human-error caused crashes, the airline industry developed the system of job training and information sharing known as Crew Resource Management (CRM). Under the new industry-wide system of CRM, pilots, flight attendants, and ground crews now communicate and cooperate in ways that have greatly reduced the hazards of commercial air travel. The coauthors of this book sought out the aviation professionals who made this transformation possible. Beyond the Checklist gives us an inside look at CRM training and shows how airline staff interaction that once suffered from the same dysfunction that too often undermines real teamwork in health care today has dramatically improved. Drawing on the experience of doctors, nurses, medical educators, and administrators, this book demonstrates how CRM can be adapted, more widely and effectively, to health care delivery.
