Dieses wird nicht nur in der Sozialen Arbeit und im Gesundheitswesen angewandt, sondern auch in der Pflege, der Rehabilitation, der Psychiatrie, bei medizinischen Behandlungen, in der Altenpflege, der Behindertenhilfe, der Familienhilfe, der Kinder- und Jugendhilfe, aber auch der Straffälligen- und Bewährungshilfe sowie der Suchtmittelabhängigen- und Wohnlosenhilfe. Case Management kann als koordinierter und vernetzter Hilfeprozess betrachtet werden, der versucht durch Aktivierung der persönlichen, sozialen und materiellen Ressourcen die Abhängigkeit der Klienten von Hilfesystemen zu verringern. Es geht darum, die Selbstbestimmung der Klienten zu fördern.
