Participants included 68 primary caregivers seeking services for a child aged fifteen months to 12 years suspected of or diagnosed with an ASD. Assessment tools included: 1. Parenting Stress Index-Fourth Edition-Short-Form (PSI-4-SF; Abidin, 2012), 2. Parenting Sense of Competence Scale (PSOC; Johnston and Mash, 1989), and 3. Relationship Assessment Scale (Hendrick, 1988). Independent samples t-tests were conducted to determine differences with a nonclinical control population (60 participants with a typically-developed child) regarding perceptions of stress, competence and relationship satisfaction. ASD caregivers reported experiencing significantly higher stress than Control caregivers (p=.000)..A hierarchical multiple regression analysis was used to assess whether perceived competence and relationship satisfaction variables were predictors of overall stress in the clinical sample. Book excerpt: Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder that is expressed quite differently from person to person due to the spectrum of symptoms an individual may experience. Developmentally, children who have ASD manage issues with communication and restricted or repetitive behaviors that impact their activities of daily living and/or overall functioning (American Psychiatric Association, 2013). The ongoing care and assistance for children with ASD from their caregivers is vital to help them achieve and maintain optimal quality of life. Given the pervasive needs of children with ASD, caregivers have a unique role in providing for their child, as compared to caregivers of children without disabilities. The present study explored the caregiver in a family with a child with ASD. Applying a family systems perspective, Bishop's (2005a, 2005b) Disability Centrality Model constructs (i.e. role impact, relationship satisfaction, mastery) were used to explain caregiver and family quality of life. In addition, family quality of life was measured in such a way to incorporate Bishop's idea of centrality, or the importance an individual places on certain areas of life. A three-step statistical analysis was completed in order to better measure this conceptualization. This study included 204 caregivers who met the criteria for the study (i.e., caregiver of a child with ASD under the age of 18; self-reported reading level of 6th grade or above). Hayes' regression based path-analytic framework, PROCESS v2.16 was used to determine the explained variance of caregiver and family quality of life. In addition, correlational and moderation analyses were tested. Results suggested that the caregiver's perceived quality of life is positively correlated with the caregiver's perceived family quality of life. In addition, results indicated that the proposed path analyses explained 52.3% of the variance in caregiver quality of life, 44.4% variance for the weighted family quality of life model, and between 23.1-45.2% variance in the five-individual family quality of life-satisfaction domains (i.e, family interaction, parenting, emotional well-being, physical/material well-being, disability-related support). Finally, to better understand the impact of importance on quality of life, a three-step statistical analysis found that importance was significant when used as a weighted measurement of family quality of life that took the importance and satisfaction in consideration in their relation to one another; however, importance was not significant as a moderator between the relationship of caregiver relationship satisfaction and family quality of life. The findings in this study expand the current literature on caregivers of children with ASD, caregiver quality of life, and family quality of life. In addition, this study demonstrated the viability of strengths-based models of caregiver and family quality of life using Disability Centrality Model constructs through a family systems perspective for a sample of caregivers of children with ASD. Future implications for rehabilitation research and practice are discussed.

Book excerpt: Background: Caregivers of children with Autism Spectrum Disorder (ASD) have been found to have poor mental health outcomes and higher stress compared to the general population, parents of typically developing children, and parents of children with other chronic health care conditions. Emotional support for caregivers has been shown to buffer the stress and negative effects that some caregivers experience. Caregivers living in rural areas may face additional barriers to obtaining support for themselves due to the lack of local support resources. Purpose: The purpose of this dissertation study is to examine geographic (urban, suburban, and rural setting) disparities in overall mental health and stress for caregivers and the amount of emotional support sources received by caregivers of children with ASD. In addition, the impact emotional support has on caregiver overall mental health and stress for caregivers of children with ASD is explored. Method: A cross-sectional secondary analysis of the 2016-2019 National Survey of Children’s Health (NSCH) data was used to conduct this dissertation study. Complex survey data analyses techniques with bivariable and multivariable linear regression models were employed for the statistical analyses. Results: No geographic disparities were found in caregiver overall mental health, caregiver stress, or emotional support received when controlling for possible confounding variables. Informal emotional support was found to significantly decrease caregiver stress for caregivers of children with ASD when controlling for covariates. In addition, formal emotional support was found to increase stress for caregivers, but this relationship was no longer significant when controlling for covariates. Overall, near or over a quarter of the subpopulation sample in each geographic area reported having no informal support sources and over half reported having no formal support sources. Conclusion: Despite hypotheses based on previous research, geographic location of residence did not predict outcome variables in parents of children with ASD. However, a concerning percentage of the sample in this study reported having no informal or formal emotional support. This is troubling as informal emotional support appears to be beneficial in decreasing caregiver stress for caregivers of children with ASD. Recommendation: Future research is needed to explore the characteristics and types of emotional support found beneficial to caregivers of children with ASD in varying geographic areas. Barriers to receiving emotional support need to be identified and intervention studies are needed to target the lack of emotional support accessed by caregivers of children with ASD. When providing formal emotional support to caregivers of children with ASD, health care providers need to be aware of the possibility of adding stress to caregivers.

The ability to comply with requests may be especially important for children with ASD, who are already pre-disposed to core deficits in social-communication that may make caregiver-child interactions more challenging. Although compliance to caregivers' requests has been studied in children with autism, studies have largely neglected exploring these constructs amongst very young children with ASD. There is also a scarcity of research exploring the effect of early caregiver-mediated intervention on compliance in very young children with autism, an area that warrants more study given the importance of children's compliance for future outcomes. This study is a secondary data analysis of existing data collected as part of a larger randomized controlled efficacy trial of a naturalistic, parent-mediated social-communication intervention for children with (ASD). The aims of this study are to characterize the use of caregiver requests and children's compliance at entry to the study, explore the effects of treatment on caregiver's use of requests and children's compliance to caregivers' requests over time. Caregiver's requests and children's compliance were coded using videos of dyads completing a clean-up task at three timepoints: entry, exit and 3-month follow-up. Caregiver-mediated intervention (JASPER) increased children's compliance with caregivers' direct requests, but no effect was detected for the effect of treatment on children's compliance with caregivers' indirect requests and caregivers' total requests over time. No treatment effect was found for caregivers' use of direct and indirect requests for participants in either condition. Implications for the contributions of caregiver-mediated treatment and caregivers' use of requests on the development of compliance in very young children with ASD are discussed.

Book excerpt: Given the unique demands of raising a child with autism spectrum disorder (ASD), it is necessary to understand the impact on the quality of life (QoL) of their caregivers. Though QoL is considered an important outcome in health services research (Moons et al., 2006; Oliveira, Carvalho, & Esteves 2016), there is a lack of knowledge about QoL in caregivers of children with ASD or caregivers of children for whom there are ASD concerns, but no formal diagnosis. The objectives of the current study were to (1) Examine how having a young child with ASD or ASD concerns affects caregiver QoL, relative to children with other developmental concerns and typical development; (2) Examine profiles across QoL domains within each caregiver group to determine which QoL domains were most impacted relative to other domains; (3) Examine to what extent three levels of determinants contribute to caregiver QoL: family demographic (child age, family income, caregiver employment status, number of children in the home), psychological (parenting stress), and health care (family-centered care); and 4) (Exploratory) Examine the extent to which family centered care moderates the relation between parenting stress and QoL. The sample was drawn from a larger NIH study examining screening and referral practices for children with ASD in primary care provider (PCP) practices and early intervention (EI) agencies in Washington State. The sample was comprised of three groups: caregivers of children with ASD or ASD concerns (ASD/ASD Concerns; n = 42), caregivers of children with other developmental concerns (Other Concerns; n = 34), and caregivers of children with typical development (No Concerns; n = 112). Caregivers completed a measure of family demographics, parenting stress, family-centered care, and QoL at their time of entry in the study. Results indicated that caregivers in the ASD/ASD Concerns group reported significantly lower Psychological and Social QoL than caregivers in the Other Concerns group and the No Concerns group, and significantly lower Environmental QoL and Physical QoL than caregivers in the No Concerns group (Objective 1). For caregivers in the ASD/ASD Concerns group, Social QoL was significantly lower than Environmental and Physical QoL and Psychological QoL was significantly lower than Environmental QoL (Objective 2). Income, parenting stress, and family-centered care were significant predictors for all four QoL domains, while the number of children residing in the home was a significant predictor for Physical QoL only. There was also a significant group by stress interaction for Environmental QoL, such that there was a stronger negative relationship between parenting stress and Environmental QoL for caregivers in the ASD/ASD Concerns group compared to caregivers in the No Concerns group (Objective 3). Family-centered care did not moderate the relationship between parenting stress and QoL as predicted (Objective 4). Overall, these findings suggest that future research is needed to determine how to provide additional support to all caregivers, particularly low-income families and caregivers who are experiencing high levels of parenting stress and low levels of family-centered care.

Book excerpt: Objective: The purpose of the present study was to examine risk and protective factors associated with the life satisfaction of caregivers and typically developing (TD) siblings of children with autism spectrum disorder (ASD). Method: Participants for this study included 106 families of children with ASD ages 3 to 17 years old, with at least a primary caregiver and a TD sibling ages 11 to 17 years serving as respondents. Of these 106 families, 68 families had a secondary caregiver who also participated in the study. Caregivers reported on the ASD symptom severity and behavioral and emotional problems of their child with ASD and their TD child. Additionally, caregivers self-reported on their distress, perceived social support, and life satisfaction. TD siblings self-reported on their perceived social support and life satisfaction, as well as their own behavioral and emotional problems. Results: PROCESS, a computational tool for SPSS that allows for the examination of indirect effects via bootstrapping techniques (Hayes, 2013, 2018), was used to examine six parallel mediation models. Results revealed no direct effect for characteristics of the child with ASD (i.e., ASD symptom severity, behavioral and emotional problems) on life satisfaction for any family member. Additionally, no indirect effect through social support was found for any family member and no indirect effect of caregiver distress was found for secondary caregivers or TD siblings. However, results indicated an indirect effect of characteristics of the child with ASD on life satisfaction through caregiver distress for primary caregivers. Conclusions: Results of this study provide insight into the functioning of family members of children with ASD and provide important clinical implications for working with the families, as well as suggestions for future research.

Book excerpt: Caregivers raising a child with autism experience increased parental burden, though many have positive experiences as well. Perceived negative impact, a form of parental burden, is the degree to which a caregiver reports negative financial, social, and emotional experiences associated with having a child with a disability. This longitudinal study defined parental perceived negative impact trajectory classes across time, determined predictors, and explored the relationship between functional adult outcomes and impact class. Participants (n = 209) were comprised of caregivers whose child received an ASD diagnosis or had developmental delay. Latent class growth modeling defined three trajectory classes (e.g., low [n = 68, 32.54%], medium [n = 98, 46.89%], and high [n = 43, 20.57%]). Regressions revealed the low impact trajectory class had more caregivers who were racial minorities, less educated, and more socially supported. Membership in the high negative impact class was associated with increased childhood hyperactivity, irritability, autism symptomology, and poor adaptive skills. The low negative impact trajectory class was associated with young adults achieving more functional outcomes. Overall, negative impact declined over time all for all classes, though never completely subsided. Possible implications for clinical practices are discussed.
