This global burden starkly illustrates the inequality between the developed and the developing world. While the majority of people living in developed countries receive timely treatment, those living in developing countries are not as fortunate and their survival rates are much lower--not only due to delays in diagnosis, but also to a lack of personnel, a paucity of treatment facilities, and the unavailability of many medications. Routine screening--a mainstay in the developed world--could greatly increase the likelihood of identifying individuals with early stage cancers and thus reduce the number of people who present with advanced disease. This book represents a critical addition to the literature of global health studies. Focusing on cervical, breast, and oral cancers, these case studies highlight innovative strategies in cancer screening in a diverse array of developing countries. The authors discuss common issues and share how obstacles--medical, economic, legal, social, and psychological--were addressed or overcome in specific settings. Each chapter offers an empirical discussion of the nature and scope of a screening program, the methodology used, and its findings, along with a candid discussion of challenges and limitations and suggestions for future efforts.

This book identifies feasible, affordable steps for LMCs and their international partners to begin to reduce the cancer burden for current and future generations. Stemming the growth of cigarette smoking tops the list to prevent cancer and all the other major chronic diseases. Other priorities include infant vaccination against the hepatitis B virus to prevent liver cancers and vaccination to prevent cervical cancer. Developing and increasing capacity for cancer screening and treatment of highly curable cancers (including most childhood malignancies) can be accomplished using "resource-level appropriateness" as a guide. And there are ways to make inexpensive oral morphine available to ease the pain of the many who will still die from cancer.

Book excerpt: Though cancer was once considered to be a problem primarily in wealthy nations, low- and middle-income countries now bear a majority share of the global cancer burden, and cancer often surpasses the burden of infectious diseases in these countries. Effective low-cost cancer control options are available for some malignancies, with the World Health Organization estimating that these interventions could facilitate the prevention of approximately one-third of cancer deaths worldwide. But these interventions remain inaccessible for many people in the world, especially those residing in low-resource communities that are characterized by a lack of funds â€" on an individual or societal basis â€" to cover health infrastructure and care costs. Few guidelines and strategies for cancer control consider the appropriateness and feasibility of interventions in low-resource settings, and may undermine the effectiveness of these efforts. For example, interventions that are designed for high-resource settings may not account for important considerations in low-resource settings, such as resource constraints, infrastructure requirements, or whether a community has the capacity to deliver downstream cancer care. Patients in resource-constrained communities continue to face delayed diagnoses of cancer, potentially resulting in the diagnosis of later stage cancers and worsened patient outcomes. In addition, social stigmas, geopolitical issues, and cultural norms may limit access to cancer care in certain communities. Recognizing the challenges of providing cancer care in these settings, the National Academies of Sciences, Engineering, and Medicine developed a workshop series examining cancer care in low-resource communities. This report summarizes the presentations and discussions from the first workshop, which focused on cancer prevention and early detection.

Book excerpt: Telemedicine networks to support healthcare workers in resource-limited settings (often for humanitarian purposes) have evolved over the last decade or so in a largely autonomous way. Communication between them has been informal and relatively limited in scope. This situation could be improved by developing a comprehensive approach to the collection and dissemination of information. A recent review identified seven telemedicine networks, each of which had been in operation for at least five years and which provided store-and-forward telemedicine services to doctors in low- and middle-income countries. These networks provide clinically useful services and improved healthcare access. However, like much of telemedicine, the formal evidence for their cost-effectiveness remains weak. Topics of current research interest therefore include the cost-effectiveness of telemedicine in resource-limited settings. Outcomes data (and methods for gathering it) such as patient quality of life following a telemedicine episode, the knowledge-gain of healthcare staff involved in telemedicine, and staff recruitment and retention in rural areas are also of interest. Finally, there is little published information about the performance of these telemedicine networks (and methods for measuring it), about how best to manage them, and about how to share resources between them. A collection of articles reporting the current evidence supporting the use of telemedicine in resource-limited settings would build the evidence base and should provide a focus for future research. It would also serve to raise the profile of this potentially important research field.
