Book excerpt: Introduction: Over the past five years, California made significant strides in reducing the number of uninsured children from an estimated 778,000 in 2003 to 683,000 children in 2007, a 12 percent decline. This improvement in children's coverage was due in part to Medi-Cal and Healthy Families expansions, the introduction of Healthy Kids programs in several counties, and effective outreach and enrollment efforts. However, gains in children's coverage have slowed in recent years. Children's Health Coverage Facts and Figures provides an overview of trends in children's coverage and insurance programs in the state. Key findings include: The proportion * of children without health insurance continued to decline through 2007, though the pace of improvement has slowed. * Nearly 80 percent of California's uninsured children are eligible for coverage under either Medi-Cal, Healthy Families, or Healthy Kids. * Medi-Cal and Healthy Families are key sources of coverage for children in low-income households that together have closed the coverage gap among families with incomes up to 250 percent of the federal poverty level. * Healthy Kids programs are also important for children's coverage. Twenty-four counties operate Healthy Kids programs and four others rely on California Kids. * Children are less likely to have employment-based coverage than adults and are more likely to be enrolled in public programs in California. The future of children's health insurance coverage in California is uncertain. On the one hand, California's economic downturn and budget crisis will likely make it difficult to increase the number of insured children. On the other, Congress and the President reauthorized the Children's Health Insurance Program (CHIP), which provides new policy options for expanding coverage. In addition, Congress and the Obama Administration are working on national health care reform, which if enacted, would likely benefit children in California. In this presentation, children are defined as people between the ages of 0 and 18, unless otherwise noted.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Available in PDF, EPUB and Kindle. Book excerpt: Why is there such a deep partisan division within the United States regarding how health care should be organized and financed—and how can we encourage politicians to band together again for the good of everyone? For decades, Democratic and Republican political leaders have disagreed about the fundamental goals of American health policy. The modern-day consequences of this disagreement—particularly in the Republicans' campaign to erode the coverage and equity gains of the Affordable Care Act—can be seen in the tragic and disparate impact of COVID-19 on the country. In Crossing the American Health Care Chasm, Donald A. Barr, MD, PhD, details the breakdown in political relations in the United States. Why, he asks, has health policy—which used to be a place where the two sides could find common ground—become the nexus of fiery political conflict? From Harry S. Truman's failed attempt to enact a plan for national health insurance to the recent efforts of President Donald J. Trump, Barr's historical analysis also touches on every presidential administration in between. Tracing the bipartisanship that developed over the four decades following the passage of Medicare and Medicaid in 1965, Barr explains why this spirit of cooperation has given way to such a seemingly unbridgeable ideological chasm. Exploring how political conflict affects health care organization, financing, and delivery, Barr also offers a detailed analysis of the multiple attempts on the part of congressional Republicans and the Trump administration either to weaken or to repeal the ACA. Crossing the American Health Care Chasm offers a series of steps that policy makers can take to improve the national health care situation and provide a basis for ongoing bipartisanship as we continue to confront the policy challenges facing our country. Ultimately, Barr argues, this divide is more dangerous than ever at a time when health care costs continue to skyrocket, the number of uninsured Americans is rising, many state governments are chipping away at Medicaid, and the GOP has not let up in its efforts to dismantle the ACA. This book will be of profound interest both to those responsible for carrying out national health care policy and to those who study health policy from an academic perspective.

Book excerpt: "California employers and consumers are being asked to pay more for health care through soaring premiums and increasing co-payments, but they should not expect to receive comparable increases in benefits or quality care. Some health plans are citing factors, such as rising prescription drug costs and utilization, as the primary reasons why California's health care premiums are soaring. However, closer scrutiny does not entirely support this explanation. This study examines the factors underlying soaring premiums and why premiums will likely continue to outpace health care cost increases, how these rising premiums might affect the availability of employer-based insurance and ... how the documented health care changes and challenges affect the nation's largest minority community--Hispanics."--P. 1.
