The way in which the final assembly of different components of the breast antigens is achieved and their functions are now within our grasp as a result of new understanding of molecular structure of these breast antigens. In addition. newer immunoassays aiming at the earliest detection of the disease are also described that integrate with promising attempts at imaging and radioimmunotherapy to set the stage for new oncological possibilities in breast cancer treatment. All these areas of intense involvement of scientists with diverse specialties are presented in this volume. which proves the need for multidisciplinary approaches to increase our chances for success in this field of medical research.

Understanding the genetic information for the expression of these antigens has been recently advanced leading to preparation of molecularly engineered reagents for use in vaccination, serum assays, and immunizations for novel antibody production. In the anti-breast cancer antibody field the availability of molecular engineering approaches to humanize murine antibodies has induced intense interest in the creation of less immunogenic antibody forms that are now available for clinical testing. Clinical studies using anti-breast murine antibody continue to be carried out and are presented at this meeting establishing a base line for safety and efficaciousness in imaging and immunotherapy that it is hoped will be superseded by the humanized forms. Basic immunology and immunochemistry studies in breast cancer are also included in this workshop that demonstrate the fast pace at which this research is advancing in many laboratories worldwide.

These meetings have acted in a way as a clearing house and have permitted their attendees to derive predictions that have helped shape future research and fine-tune objectives. But above all, the re-evaluation of past research at these Workshops and the renewed excitement brought to them by new information has helped generate a momentum and enthusiasm that assures for the future large scientific gains.

In the last two years since our first International Workshop on Monoclonal Antibodies and Breast Cancer there has been an enormous surge in the number and quality of applications for these new reagents. Solid achievements have been made in identification and quantitation of estrogen and progesterone receptors. in histopathological diagnostic procedures. in serum diagnosis. and we are witnessing now the first attempts to treat breast cancer with immunoconjugates. Cytosolic estrogen receptors can now be quantitated with monoclonal antibodies. and also their cellular distribution can be directly assessed histologically. In addition. monoclonal antibodies to progesterone receptors have been generated that show promise in having similar uses as those to the estrogen receptor. In the field of diagnosis. the use of monoclonal antibodies has permitted the development of serological approaches for early diagnosis by identifying and measuring breast epithelial antigens in serum. and of histological approaches for establishing criteria for breast cancer dissemination and prognosis. With great expectations we are all following developments in the area of breast cancer treatment using conjugates of anti-breast epithelial monoclonal antibodies which are now confirming earlier reports in affecting breast tumor control. The recent creation of newer monoclonal antibodies and conjugates await experimentation and clinical trials to determine their value.

Book excerpt: Breast cancer, being a disease with very high prevalence in the female population, has permanently attracted the attention of biologists, biochemists, pathologists, surgeons, and oncologists. In the U.S. alone approximately 120,000 new cases are diagnosed and 37,000 deaths a year are recorded as an outcome. Early diagnosis, clear cut prognosis and the hope of new therapeutic approaches has spurred the enthusiasm of researchers, who with the advent of newer methodologies, tried to employ it in different approaches to the breast cancer patient. In spite of the efforts devoted towards the creation of new approaches, reagents and procedures taking advantage of special characteristics of the breast tissue were slow to come. In general, most diagnostic and prognostic techniques were those shared and applied to other carcinomas. In the early seventies we discovered the first sets of antigens associated with breast epithelial tissue both in the mouse and the human. The recognition of such group of molecules, present in the milk fat globule membrane, gave impetus to several studies in these areas. These mammary cell surface antigens proved useful in several diagnostic approaches such as radioimaging, histochemical and serum assay procedures. However, the apparently inherent limitations of polyclonal antibodies reduced the impact of these novel approaches.

In this issue, the better-characterized glycoprotein antigens and novel molecules in angiogenesis are examined as new targets of breast cancer vaccines or immunotherapy. Continued effort in new antigen identification will be critical to cancer control. Finally, a reality check is warranted. Most breast cancer cells are still elusive to immune intervention.The mechanisms of such evasion are under intense investigation and much progress has been made. Alteration in antigen processing machinery is a major route of tumor evasion.

The report herein focuses on successful syntheses of mucin related 0-linked glycopeptides, in particular the TF and 2,3-STF antigens. TF is observed on a variety of epithelial tumors and 2,3-STF is observed on a large number of breast tumors.

While TAA-based immunotherapeutic treatments for the eradication of cancer are being pursued, few TAAs have been defined. We have applied a serologically based strategy called SEREX to the identification of TAAs in the murine colon carcinoma cell line, MC38, and in human breast tumor tissue. We have identified two positive clones in MC38, one as a murine endogenous leukemia proviral envelope (env) and LTR sequence, and the other as the murine ATRX sequence. Northern blot analysis demonstrated high levels of expression of env in MC38 cells, whereas expression was absent in normal colonic epithelium. Southern blot analysis revealed several mutations at the DNA level. Northern blot analysis of ATRX demonstrated comparable levels of expression in normal versus MC38 cells, and sequence analysis indicated no mutations. Polynucleotide immunization strategies with these antigens are being pursued. A potential TAA from breast tissue has been isolated and is being characterized. These results demonstrate the potential of SEREX screening to identify tumor antigens which may be utilized as novel targets for cancer immunotherapy.
