Intimate labour as a theoretical construct provides a way to think about the kind of care doulas offer women across the reproductive spectrum. Doulas negotiate boundaries and often blur the divisions between communities and across public and private spheres in their practice of intimate labour. This book weaves together three main threads: doulas and mothers, doulas and their community, and finally, doulas and institutions. The lived experience of doulas illustrates the interlacing relationships among all three of these threads. A feminist analysis is offered using narrative to elucidate the various intersections where power structures meet and become determinate in framing the birth experience. Although women's stories are currently becoming a valuable source of data in theory formation and in understanding the current patriarchal control over birth, the scope of these stories is somewhat limited. One conspicuous near absence is stories written by the birth attendant. This perspective--from one who both is mediator and advocate while operating within and through the traditional structure of medical establishment--is one that is much needed if there is to be a sufficient interrogation of birth as a socially constructed event whose boundaries reproduce a patriarchal model of birth. the purpose of this project is to present women's birth events through the lens of the nurse-midwife. the "stories" presented here are based on actual events and demonstrate the complexity of the interrelationship between power, the institution, the medical community, and birth. the framework for analysis is birth events, as told by the nurse-midwife, and is informed by the work of Michel Foucault and his philosophy of power and the institution. In addition, select writings of feminist philosophers offer a foundation for the interpretation of the birth events. Locating birth experiences within a feminist and philosophical framework demonstrates the value and necessity of placing birth within a feminist and philosophical framework and not solely within a patriarchal medical framework. These birth "stories" emerge from participation in more than seven hundred births and are representative of birth in a tertiary care hospital in the United States at the end of the twentieth century. They represent the midwife's own struggle surrounding the current framework of birth.
