Book excerpt: In the early days of cardiac surgery, but also in the following four decades, median sternotomy and cardiopulmonary bypass were nearly always included in cardiac surgical procedures. Less invasive surgical techniques were developed with the aim of reducing perioperative trauma without compromising the surgical result, and things became more complex for cardiac surgeons. They now often had to consider the surgical access of choice and whether cardiopulmonary bypass should be used or not. Since the mid 1990s, not only have several novel minimally invasive surgical techniques been presented, but also further refinements have been recommended from time to time. This work comes to fill a gap in the field of coronary artery bypass grafting and conduit harvesting in cardiac surgery, by gathering the mature version of such new, less invasive techniques combining safety, effectiveness, simplicity, sometimes even reducing procedural costs and that always for the patient’s and for the surgeon’s sake.

Available in PDF, EPUB and Kindle. Book excerpt: This book is unique in describing the practical considerations of endoscopic cardiac surgery. It covers this topic from basic concepts such as patient selection to imaging and outlines the different approaches to particular endoscopic cardiac surgical procedures. As there is an increasing acceptance that cardiac surgery needs to be provided to older and sicker patients and more asymptomatic patients, the option of a sternotomy is becoming less appealing. The endoscope has changed the patient pathway in many surgical specialities, but until recently cardiac surgery has been slow to adopt this approach. With improvements in imaging and better imaging technology, there is a steady increase in accepted procedures and trained surgeons offering this approach. Endoscopic Cardiac Surgery: Tips, Tricks and Traps captures the experience of endoscopic cardiac surgeons from around the world and is a concise yet thorough reference book for those starting a program in this discipline. With over 3 hours of associated video material, it will be an essential primer for the discipline, therefore making it a valuable reference for trainees in cardiac surgery and experienced surgeons wishing to add this approach to their armamentarium. With the endless pursuit of better outcomes, modern coronary artery bypass grafting (CABG) has become technically more complex in ways that are well beyond the training of the average cardiac surgeon. The old concept of "one-technique-fits-all" has been abandoned in favour of a specialized approach tailored to the individual patient. In fact, in recent years, there is a growing movement towards establishing coronary surgery as a super-specialization of cardiac surgery. Technical Aspects of Modern Coronary Artery Bypass Surgery aims to expand on both the basics and complexities of the technical aspects of coronary surgery. It serves as an up to date resource that illustrates and details the advancement and techniques in this field which may soon become a separate super-specialty. With a particular emphasis on illustrations, the book will be an essential reference book for both established surgeons that have no experience in advanced CABG, and the new generation of CABG surgeons. A complete and concise resource on all aspects of coronary surgery In-depth illustrative review of various coronary techniques Covers both current recommendations and well-established practices in the field

Available in PDF, EPUB and Kindle. Book excerpt: Minimally invasive cardiac surgery(MICS) is an integral component of every future cardiac surgeon’s training. There continues to be a growing global demand towards less invasive surgical techniques. Both cardiologist and cardiac surgeon form "heart teams" to provide patients with novel, minimally invasive procedures, with all their benefits. Less invasive techniques are often complex and require special knowhow and skills. This book offers an innovative approach to learning, utilizing QR code technology, which refers the reader to essential audio-visual material, which, along with the didactic text, focuses on practical aspects of minimally invasive cardiac surgery. In modern Heart Teams, and with the advent of the hybrid era, surgeons will only be able to survive if they have state-of-the-art skills in less invasive technologies, which can be incorporated in the hybrid theatre and/or trans-catheter arena. This text accompanies the surgeon along this path, and provides clinical advice and practical solutions, beyond the necessary basic knowledge. Which courses to visit, which videos to watch, which centres to join for serious training? How best to exploit public and multimedia? How to consent a patient into a MICS procedure? How to set up a MICS program or practice? In the era of value driven outcomes, and a shift towards shorter and better patient journeys, MICS is a skill that no heart surgeon can be without. Minimally Invasive Cardiac Surgery: A Practical Guide is a teaching resource, reference book and manual written by surgeons who both operate and teach the procedures described within. Provides access to online resources via QR codes Includes links to videos and the e-version of the text Acts as a gateway to a huge choice of minimally invasive cardiac surgery materials

Book excerpt: This book is a valuable resource on the subject of off-pump coronary artery bypass (OPCAB) surgery, which has recently been developed as a promising technique for reducing surgical damage to patients. Because of the increasing number of older patients who are candidates for a coronary artery bypass graft (CABG) operation, less-invasive surgeries are becoming a priority all over the world. Despite some challenges, such as its technical difficulties, emerging questions, and controversies involved, OPCAB surgery in CABG is now the gold standard in Japan. This book, written by OPCAB experts, examines the procedure established and its chief benefits. Its coverage ranges from a careful evaluation of the evidence for and against OPCAB surgery to preoperative and intraoperative management of patients undergoing coronary bypass surgery. It also contains a description of advanced techniques for every aspect of the procedure. The authors[share essential insights into the latest trends in cardiovascular surgery for professionals and residents, presenting detailed descriptions of the exposure of the heart, stabilization of each coronary artery target, construction of distal anastomoses, and other important factors. The book offers an invaluable reference guide for cardiac surgeons, cardiologists, anesthesiologists, and other medical staff in operating rooms around the world.

Available in PDF, EPUB and Kindle. Book excerpt: The surgery of the heart or the great vessels, performed to correct congenital heart disease or treat valvular disease or complications arising due to ischemic heart disease, is termed as cardiac surgery. The scope of cardiac surgery is wide. The surgery that is performed on the heart after a large incision to the chest to open the rib cage is called an open-heart surgery. Depending on the procedure, it may entail the opening of the heart itself. A modern development in cardiology is the procedure of beating-heart surgery, in which the heart continues to beat even during the surgery. Coronary artery bypass grafting is performed with the objective of preventing clot formation, and subsequent heart attack. As opposed to the highly invasive open-heart surgery, minimally invasive surgery requires smaller incisions and can be robot-assisted. Another important area of cardiac surgery is heart transplantation. This book is compiled in such a manner, that it will provide in-depth knowledge about the theory and practice of cardiac surgery. The aim of this book is to present researches that have transformed such surgical techniques and aided their advancement. For someone with an interest and eye for detail, this book covers the most significant topics in this field.

Book excerpt: In 1962, Thomas Kuhne coined the term “paradigm shift” while arguing that human knowledge advances by quantum leaps with interspersed smaller steps. Preparation for the major advance is generally not a concerted effort by thought leaders. Rather, a few (or one) visionaries gain insights into a process and are able to definitely demonstrate the accuracy of their worldview. Often, the epiphany does not occur during the intellectual lifetime of the discoverers. Medicine has had numerous such “paradigm shifts” including the compelling reworking of Galen’s concepts of the body. Of note, the scientific world of the time explained the new views by arguing that the human body must have changed between the time of ancient Greece and modern Europe. The inauguration of cardiac surgery itself required profound shifts in medicine’s view of physiology. Yet, over the ensuing 40 years, the field was fine tuned so we could provide greater than 95% success rates in elective surgery with low cost and short h- pital stays. In some parts of the world, the procedures were viewed as commodities and prices dropped as providers were unable to differentiate the quality of their work. As patients and their physicians became more demanding, the desire to make the procedures “minimally invasive” grew. In effect, what we were really searching for was a life saving procedure that also preserved quality of life. In short, “minimally invasive” has really been a code phrase for procedures that disrupt our quality of life the least.
