This book is a practical manual and an outline of what is taught in the courses. It is intended to help the young spinal surgeon to understand the correct use of AO implants. The indi- tions given will aid the correct use of each procedure. . It must be strongly emphasized that surgery of the spine is technically de manding. The techniques described in this book should only be undertaken by surgeons who are trained and experienced in spinal surgery. Certain techniques, in particular pedicle screw fIxation and cages, have not yet been fully approved by the FDA in the United States. However, throughout the rest of the world, the use of pedicle screws has become a standard technique for the spine surgeon, since it has been shown to improve fIxation techniques and allow segmental correction of the spine. The use of cages has become more and more popular, specifIcally as a tool of minimally invasive spinal surgery.

Book excerpt: In this book Eve Rosenhaft examines the involvement of Communists in political violence during the years of Hitler's rise to power in Germany (1929-33). Specifically, she aims to account for their participation in `street-fighting' or 'gang-fighting' with National Socialist storm-troopers. The origins of this conflict are examined at two levels. First Dr Rosenhaft analyses the official policy of the Communist Party towards fascism and Nazism, and the special anti-fascist and self-defence organizations which it developed. Among the aspects of Communist policy that are explored are the relation between the international confrontation between Communists and Social Democrats as claimants to lead the left, and the implications of this dispute in German politics; the ideological difficulties in the implementation of Communist policy in a period of economic dislocation; and the organizational problems posed by the fight against fascism. Dr Rosenhaft then explores the attitudes and experience of the Communist rank and file engaged in the struggle against fascism, concentrating on the city of Berlin, where a fierce contest for control of the streets was waged.

Most historians attribute German exceptionalism to the failure or absence of bourgeois revolution in German history and the failure of the bourgeoisie to conquer the pre-industrial traditions of authoritarianism. However, this study finds that there was a bourgeois revolution in Germany, though not the traditional type. This so-called silent bourgeois revolution brought about the emergence and consolidation of the capitalist system based on the sanctity and disposability of private property and on production to meet individual needs through a system of exchange dominated by the market. In this connection, this book proposes a redefinition of the concept of bourgeois revolution to denote a broader pattern of material, institutional, legal, and intellectual changes whose cumulative effect was all the more powerful for coming to be seen as natural.
