Book excerpt: Contexte : La lombalgie chronique commune est une affection fréquente dont l'évolution est la plupart du temps bénigne. Toutefois, la récidive fait partie de l'évolution naturelle de la maladie et les coûts générés en raison du handicap et des arrêts de travail sont très importants pour la société. Le but de cette étude est d'expliciter les pratiques de prise en charge de la lombalgie chronique par les médecins généralistes. Méthode : Nous avons réalisé une étude qualitative au cours de laquelle 12 médecins généralistes ont été interrogés lors d'entretiens individuels semi-dirigés enregistrés. Nous avons ensuite réalisé une analyse thématique à l'aide du logiciel NVivo®. Résultats : Les recommandations HAS ne semblent pas être d'une grande aide pour cette affection et sont peu utilisées. Il s'agit d'une affection pour laquelle il arrive que les médecins généralistes se sentent démunis. Plusieurs freins ont été mis en évidence concernant les médicaments, la prise en charge de la douleur radiculaire, l'activité physique, le maintien de l'emploi et le recours à la multidisciplinarité qui n'est pas aussi simple que les médecins généralistes le souhaitent. L'urgence de remettre le patient en mouvement est une évidence et la rééducation l'enjeu principal de la prise en charge avec mise en œuvre d'une multidisciplinarité. Afin de remettre les patients au travail, les médecins généralistes prescrivent la ceinture lombaire. La douleur qui semble la plus difficile à soulager semble être la radiculalgie. C'est souvent en raison de la présence de cette douleur que les médecins adressent leurs patients en centre anti-douleur et aux rhumatologues.Conclusion : Les médecins généralistes délivrent à leurs patients une prise en charge selon un modèle biopsychosocial et multidisciplinaire et donc conforme aux données actuelles de la science. Il est cependant nécessaire de développer des pistes d'amélioration grâce à des études quantitatives.

Sa prise en charge actuellement préconisée est multimodale, bio psycho sociale. Des recommandations pour la pratique sont publiées en 2000 par l'ANAES et en 2008 par la HAS. Nous décrivons les pratiques des médecins généralistes sur le terrain, pour mettre en évidence les limites d'applicabilité de ces recommandations en soins primaires, et proposer des pistes d'amélioration. Matériel et méthode : étude transversale descriptive randomisée. Enquête téléphonique auprès de 100 médecins généralistes du Languedoc Roussillon. Résultats :les praticiens se conforment aux recommandations pour : l'interrogatoire, l'examen clinique, les modalités de suivi, l'éducation thérapeutique, les objectifs de la prise en charge, la prescription médicamenteuse, la prescription de thérapies physiques. Les difficultés d'application sont: l'utilisation des échelles d'évaluation de la douleur et les questionnaires évaluant son retentissement, la prescription des examens d'imagerie à bon escient, la prescription de psychothérapie par un tiers et sa mise en oeuvre par le généraliste au cabinet. Discussion : les raisons de ces difficultés sont : le manque de formation, l'absence d'adhésion du praticien à la conduite préconisée, la non-faisabilité de la directive dans le cadre du cabinet, la difficulté liée aux spécificités du patient, le manque de ressources en termes de professionnels et de structures disponibles, le manque de temps, le manque de reconnaissance du travail fourni. Aussi, le contenu mériterait d'être actualisé. Conclusion : les limites sont reconnues par les instances à même d'y remédier, mais ne figurent pas parmi les priorités actuelles

Book excerpt: Résumé: Introduction : La lombalgie chronique, définie par une douleur habituelle de la région lombaire évoluant depuis plus de 3 mois, a des conséquences socio-professionnelles majeures et entraîne une consommation importante de soins et d’absentéisme au travail. L’intensité de la douleur représenterait moins de 10 % de la variance du handicap alors que 35 % de cette même variance serait liée aux facteurs psycho-sociaux. A l’origine du parcours de soins, le médecin généraliste a un rôle central lors de l’évaluation initiale en identifiant les patients douloureux à fort potentiel de chronicisation. L’objectif principal est de dresser un état des lieux des pratiques des médecins généralistes dans l’évaluation du profil psychosocial des patients atteints de lombalgie chronique afin d'identifier les paramètres à améliorer. Matériel et Méthode : Il s’agit d’une étude observationnelle, descriptive et transversale, réalisée à l’aide d’un questionnaire rédigé à partir des recommandations de la Haute Autorité de Santé (HAS). Il a été envoyé par courrier électronique à un échantillon de 200 médecins généralistes thésés de l’aire toulonnaise entre Février et Mai 2016. Résultats : La recherche d’une fragilité psychologique (anxiété et/ou symptômes dépressifs) est effective dans 98,9% (dont 55,9% systématique et 43% occasionnelle). L’orientation des patients vers le psychologue ou le psychiatre existe pour 64,5% des professionnels, mais est occasionnelle, toutefois l’amélioration des patients traités est constatée. Les médecins évaluent systématiquement le contexte social à 68,8% et contexte professionnel à 96,7%. 73,1% considèrent avoir un rôle actif dans la prévention des patients exerçant une profession à risque de lombalgie. La communication avec le médecin du travail est faible. Les médecins pensent majoritairement (89,2%) qu’une meilleure coordination améliorerait la prise en charge. Discussion : Le médecin généraliste a un rôle fédérateur dans la prise en charge de la lombalgie chronique, en tant que médecin de premiers recours et coordinateur des soins. L’évaluation du contexte socio-professionnel est effective, la prise en compte d’une fragilité psychologique reste à développer. L’éducation thérapeutique délivrée par le médecin généraliste cible essentiellement l’importance de l’activité physique et des gestes de posture à adopter au quotidien. Une grille de lecture, mise à disposition des médecins généralistes, clairement définie, intégrant les paramètres psychosociaux permettrait d’effectuer un bilan opposable à la médecine du travail. Conclusion : La lombalgie chronique est une pathologie complexe qui nécessite de prendre en compte tous les aspects de la douleur chronique, qui ne peut se faire en une seule consultation. La reconsidération du médecin du travail en tant que collaborateur du médecin généraliste en médecine de premiers recours permettrait d’en optimiser la prise en charge.
