Assessing cost-of-illness reveals the major determinants of costs to society. Weaknesses in current methodology include a lack of comprehensiveness, insufficient precision, the absence of information on reliability and validity and the omission of key stakeholders' perspectives. The objectives of this thesis were to assess the reliability of patient self-reports of health services utilization and to utilize a prospective model to generate a comprehensive and precise estimate of the cost of asthma per patient from multiple perspectives. Methods. The Pharmacy Medication Monitoring Program (PMMP) collects longitudinal data from patients recruited in community pharmacies. Respiratory patients reported medication consumption, health services use, out-of-pocket costs, quality-of-life and other information during 3 telephone interviews conducted at 1, 3 and 6 months after enrollment. Self-reports of health services utilization were compared to Ontario Health Insurance Plan (OHIP) records in 83 subjects and the cost of asthma was assessed in 1,279 patients. Results. A gradient agreement between self-reports and OHIP records was observed, ranging from substantial for admissions to slight for emergency room visits. Age, disease severity and occupation were significant determinants of total costs. The annual cost to society among adults ranged from $1,633 per mildly ill patient (95\% CI \$1,282, $1,984) to \$4,223 per severely ill patient (95% CI $3,849, \$4,598). The costs in children under 4 years were higher than older children but lower than adults. Indirect costs were responsible for 43% of total costs. The largest components of direct costs were hospitalizations, 47%, and medications, 40%. Conclusions. Interventions aimed at reducing productivity losses and admissions may result in savings to society, the health care system and the patient. Estimates stratified by age, severity and occupation can serve as benchmarks and prove useful for case mix and capitation planning. The quality of clinical and health policy decision-making may be enhanced by cost-of-illness estimates that are comprehensive, precise and expressed from multiple perspectives. The PMMP is a potentially useful model for outcomes research and health economic assessments.

Book excerpt: Asthma is the most common chronic respiratory disease and is characterized by inflammation of the airways in response to stimuli. Evidence additionally suggests that asthma is more prevalent and more severe in Black American and Hispanic populations, influenced by factors such as chronic stress, access to healthcare, and medical racism. We aimed to quantify the prevalence, severity, and nonfatal burden of asthma by race/ethnicity, utilizing a Bayesian meta-regression analytical method. Methods: We ran models of asthma prevalence by race/ethnicity, year, sex, and age, utilizing data from the National Health and Nutrition Examination Survey (NHANES) and the National Health Interview Survey (NHIS). We separately quantified the severity of asthma using NHANES data on functional health loss due to wheeze, mapping this information to health states and disability weights sourced from the Global Burden of Disease study. This information was used to calculate Years Lived With Disability, a metric of nonfatal disease burden. We conducted a Das Gupta decomposition analysis to assess whether differences in nonfatal burden between different race/ethnicity groups were driven by prevalence or severity. Non-Hispanic Black women had the highest age-standardized asthma prevalence rates (6.1%, 95% Uncertainty Interval [UI] 5.7 to 6.3). Black women also had the highest age-standardized mean disability weight of all race/ethnicity and sex combinations (0.049, 95% UI 0.040 to 0.060), while Non-Hispanic White men have the least severe asthma on average. An estimated 66% (37.2 to 80.8) of the difference in age-standardized YLDs between Black and White women was attributable to severity. This study indicates large racial disparities in the nonfatal burden of asthma in the United States, with the largest burden in Non-Hispanic Black women. Concerted policy action should be taken to increase access to high-quality healthcare in this group to reduce observed disparities.

Key tables and figures from the full report are included for easy reference. Asthma is a chronic inflammatory disease of the airways. In the United States, asthma affects more than 22 million persons. It is one of the most common chronic diseases of childhood, affecting more than 6 million children (current asthma prevalence, National Health Interview Survey (NHIS), National Center for Health Statistics, Centers for Disease Control and Prevention, 2005) (NHIS 2005). There have been important gains since the release of the first National Asthma Education and Prevention Program (NAEPP) clinical practice guidelines in 1991. For example, the number of deaths due to asthma has declined, even in the face of an increasing prevalence of the disease (NHIS 2005); fewer patients who have asthma report limitations to activities; and an increasing proportion of people who have asthma receive formal patient education (Department of Health and Human Services, Healthy People 2010 midcourse review). Hospitalization rates have remained relatively stable over the last decade, with lower rates in some age groups but higher rates among young children 0–4 years of age. There is some indication that improved recognition of asthma among young children contributes to these rates. However, the burden of avoidable hospitalizations remains. Collectively, people who have asthma have more than 497,000 hospitalizations annually (NHIS 2005). Furthermore, ethnic and racial disparities in asthma burden persist, with significant impact on African American and Puerto Rican populations. The challenge remains to help all people who have asthma, particularly those at high risk, receive quality asthma care. Advances in science have led to an increased understanding of asthma and its mechanisms as well as improved treatment approaches. To help health care professionals bridge the gap between current knowledge and practice, the NAEPP of the National Heart, Lung, and Blood Institute (NHLBI) has previously convened three Expert Panels to prepare guidelines for the diagnosis and management of asthma. The NAEPP Coordinating Committee (CC), under the leadership of Claude Lenfant, M.D., Director of the NHLBI, convened the first Expert Panel in 1989. The charge to that Panel was to develop a report that would provide a general approach to diagnosing and managing asthma based on current science. Published in 1991, the “Expert Panel Report: Guidelines for the Diagnosis and Management of Asthma” (EPR 1991) organized the recommendations for the treatment of asthma around four components of effective asthma management: Use of objective measures of lung function to assess the severity of asthma and to monitor the course of therapy; Environmental control measures to avoid or eliminate factors that precipitate asthma symptoms or exacerbations; Patient education that fosters a partnership among the patient, his or her family, and clinicians; Comprehensive pharmacologic therapy for long-term management designed to reverse and prevent the airway inflammation characteristic of asthma as well as pharmacologic therapy to manage asthma exacerbations. The NAEPP recognizes that the value of clinical practice guidelines lies in their presentation of the best and most current evidence available. This report presents recommendations for the diagnosis and management of asthma that will help clinicians and patients make appropriate decisions about asthma care. ~

Many of these hazards are subject to regulation, or may be considered for regulation, in order to reduce exposures and prevent human health risks. However, the available information on willingness-to-pay (WTP) to avoid asthma or reduce its severity is scarce, incomplete and does not provide estimates compatible with welfare economic theory that can be used in cost-benefit analysis. This paper is part of the series of large scale willingness to pay (WTP) studies resulting from the Surveys to elicit Willingness to pay to Avoid Chemicals related negative Health Effects (SWACHE) project that intends to improve the basis for doing cost-benefit analyses of chemicals management options and environmental policies in general. The present paper offers values suitable for use in cost-benefit analyses of the WTP for reduced severity of asthma attacks in adults and children and in reduced probability of getting asthma for these two population groups, all in the context of reducing chemical exposures, and covering populations in seven OECD countries: Canada, Czech Republic, France, Poland, Sweden, the United Kingdom and the United States. The context for such WTP elicitations was a set of household products that contain fewer hazardous chemicals than what is currently available in supermarkets but are more expensive.

A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure.
