Book excerpt: Given the lack of an appropriate vaccine or an effective treatment for AIDS, more attention has been focused on controlling the threat of the disease, particularly through efforts to increase widespread behavioral change. HIV antibody testing has been advocated as an important intervention to influence people to practice safer sex to decrease the risk of infection with HIV (CDC, 2003; Wilson & Jaccard, 1993). The theory of reasoned action and planned behavior (Ajzen, 1975. 1991) was used to explore the factors that influence HIV testing decisions among 186 sexually active heterosexual college students. According to the TPB, the most important determinant of an individuals' HIV testing is his/her behavioral intention to be tested for HIV. The present research operationalized all components of the TPB model and several extensions including attitudes (perceived benefits & HIV testing Beliefs), subjective norm (college norm and friend/family/sexual partner norm), perceived behavioral control, and perceived barriers. The role of anticipated regret was explored as well as the relationship between returning for test results and several model variables. Results revealed that despite a relatively high knowledge level, students are continuing to engage in a multitude of HIV risk behaviors that are not translating into an increased likelihood of being tested for HIV. The hypothesized relations among the study independent variables and HIV testing intentions and behavior were partially supported in the regression analyses. HIV testing intentions was significantly related to perceived benefits and anticipated regret, whereas HIV testing behavior was significantly related to college norm, family/friend/sexual partner norm, HIV testing beliefs, and perceived behavioral control. As noted in the literature review, the present research was the first of its kind to apply the full TRA/TPB model to HIV testing intentions in college students. Overall, the TPB model variables (attitudes [perceived benefits, testing beliefs], subjective norms [college norms, family/friend/sexual partners' norms], perceived behavioral control and anticipated regret, accounted for a significant amount of the variance in HIV testing intentions, approximately 30%. This was particularly the case with non tested students. These findings offers support for the use of the full TRA/TPB model variables in research on variables that may influence decisions to be tested for those who have never been tested. The general findings of this research are that the TRA/TPB is a useful and valuable model to use as a framework to inform HIV testing interventions.

Social science researchers and HIV educators have investigated the roles of intentions to prevent HIV infections mainly in the context of safer sex behaviors. Surprisingly, however, HIV testing intentions have not been extensively studied in light of various forms of HIV tests that are likely soon to be approved for widespread use . The present study evaluated the utility of Ajzen and Fishbein' s (1980) Theory of Reasoned Action in predicting college students' intentions to take an HIV test. Attitudes toward HIV testing and their perceptions of social pressure toward HIV tests were investigated as predictors of testing intentions. In addition, college students' preferences for HIV testing procedures were surveyed in order to investigate whether the characteristics of the new HIV tests were seen as favorable. Responses from 190 undergraduate and graduate students at Portland State University (Portland, Oregon) were analyzed by multiple linear regression analysis. Results strongly supported the impact of the subjective norm toward HIV testing as a determinant of testing intentions. On the other hand, testing attitudes failed to predict testing intentions unlike past studies on HIV preventive behavioral intentions. Regarding the procedural preferences, the majority of respondents preferred the procedures of conventional HIV tests except that most respondents preferred not to have post-test counseling after an HIV-negative result; in addition, respondents were equally divided in their preference for blood or saliva fluid sampling. Implications of these findings are discussed for possible interventions.

Book excerpt: This study investigates, using the Theory of Reasoned Action and its extensions, factors that determine intention to use condoms among tertiary institution students in Botswana. The data used was obtained from a questionnaire survey of 785 students. The findings partially support existing theory, for the three explanatory factors of intention were significant without controlling for the effect of each other. However, contrary to empirical findings in advanced countries, subjective norm (societal factor) played a larger role in explaining intention than attitude, although the evidence is mixed. These findings suggest that in Western cultures where individualist freedom is emphasised, behavioural intention would generally be determined not only by personal attitudes, but more so by the attitudes of others. Further, the relatively undeveloped economic environment might impose behavioural contraints. In several African countries these broader social-environmental issues pose critical and sometimes binding influence on the behaviour of many individuals. It follows that a study of behavioural intentions in the African scene which places greater emphasis on measuring attitudinal factors may be misleading because of under-specification of the causal social-environmental (perceived societal and perceived control) factors.

The number of adolescents living with HIV has risen by 28% between 2005 and 2015 in this region. The HIV/AIDS-related knowledge, attitudes, behavior and HIV testing status study was done among young people aged 15 to 24 years in Myanmar, 2016. Objectives: To assess the knowledge, attitude, behavior, and HIV testing status among young people of Myanmar. Methods: The study was a cross-sectional study; it was a nationally representative survey of women and men age 15-24 year throughout Myanmar, data based on 2015-16 Myanmar Demographic and Health Survey (MDHS). A total of 3,728 women and 1,458 men were interviewed to examine knowledge, awareness, and behavior regarding HIV/AIDS and other sexually transmitted infections (STIs). The survey used computer-assisted field editing (CAFE) procedures. SAS software 9.4 used for data management and analyses. Results: The young people in the study group have the high awareness about HIV, but limited knowledge about HIV/ AIDS prevention which varied across residence and education level. The stigma and discrimination of HIV/AIDS still prevalent among young people. The prevalence of HIV testing was low among young men. Conclusion: HIV related knowledge, attitude and behavior among young people is the key area to focus on young people living in the rural area with low education and income in Myanmar. The policymakers and HIV program managers should focus on the most vulnerable groups on HIV prevention by awareness raising campaign and by creating an enabling environment for HIV counseling and testing. Book excerpt: The current study utilized the Information Motivation Behavioral Skills (IMB) model (Fisher & Fisher, 1992) as a framework to investigate HIV risk behaviors among a sample of university students in Trinidad. The Caribbean ranks second in the world in terms of HIV rates, and although a relatively small island, the rates of HIV infection in Trinidad are of grave proportions. This research examined their existing level of HIV/AIDS knowledge, their motivation to practice safer sex, and their perceived behavioral skills at engaging in HIV/AIDS preventive behaviors. Further, this study also sought to test the fit of the IMB model to the data for this sample of Trinidadian university students. A sample of 90 sexually active participants were selected for purposes of this study and were recruited from various classes at the University of the West Indies located in Trinidad. The findings revealed that the IMB model did fit the data for this sample. However, the information variable was found to not be related to any of the model variables. Motivation on the other hand was found to be the strongest predictor of some of the preventive behaviors. Cultural examinations revealed that there were moderately high levels of HIV/AIDS knowledge, low condom use and HIV testing, and somewhat positive attitudes toward preventive acts among the participants. Exploratory analyses also revealed significant gender differences. The implications of this study for its contribution to the continued effort to understand the reasons behind the growing prevalence of HIV/AIDS are also discussed.

Methods: Sexually active 13-22 year-olds (N=200) completed a theory-based survey and were offered HIV testing. Those who agreed chose one of three HIV tests. Chi-square and logistic regression were performed to determine factors associated with testing. Results: Half of participants (49.5%) agreed to HIV testing: 51% chose rapid oral, 30% venipuncture, and 19% rapid fingerstick. Male gender (OR 3.52; 95% CI 1.35-9.21), parental completion of high school (OR 2.69; 95% CI 1.15-6.31), intention to test for HIV if offered by clinician (OR 6.78; 95% CI 2.01-22.92), and higher perceived likelihood of current HIV infection (OR 10.0; 95% CI 2.9-33.3) were independently associated with testing. Conclusions: Intention to test if offered by a clinician and perceived likelihood of current HIV infection appear to be important factors in decision to test.

Book excerpt: Substantial empirical research has documented that HIV prevention and decision-making are heavily influenced by the knowledge and practice of values and beliefs regarding infection and behavior. Most HIV research is still concentrated among high-risk populations (IDU and FSW), leaving out women in long-term heterosexual marriage who have been considered "safe" and less at risk of infection. In addition, researchers have called for more comparative and cross-cultural studies focused on the interplay of health education, behaviors, context, and HIV risk perception. The objective of this dissertation is to advance understanding of the factors sexually experienced women in Postsocialist Georgia consider important when they assess HIV perceived risk of which they are made aware by messages emanating from the social environment. A conceptual framework that integrates concepts from traditional social psychological theory and the constructs of context-specific factors to guide research intervention is applied. Combined quantitative and qualitative approaches are used to achieve a better understanding of perceived HIV risk and its association with different factors. The testing of relationships from two national Reproductive Health Survey samples (1999 and 2005) demonstrate strong positive associations among increased HIV transmission knowledge, belief in accidental transmission, HIV testing practice, and HIV perceived risk, while controlling for sociodemographic factors. Characteristics associated with social norms and economic factors (including stigmatizing attitudes about the rights of PWAs, constrained attitudes concerning sexual control, and experienced migration) demonstrate a strong significant link with assessment of HIV risk perception. Qualitative research with women strengthens the argument of superficial health education and helps to explain variations in perceived risk assessment. Through the interviews, HIV prevention practice is examined in relation to a myriad of cognitive components. This study finds health knowledge, misconceptions, stigmatizing attitudes, and beliefs in sexual and gender norms among the major factors constraining successful HIV/AIDS prevention practices. An effective strategy for HIV/AIDS prevention will require enhancement of research, more emphasis on an integrated approach to target education efforts, training providers in information diffusion approaches, and promoting a general communication campaign.

Book excerpt: Although the number of HIV infected peaked in the late 1980's, HIV remains a major concern within the African American community (CDC, 2008). African Americans are disproportionately affected, comprising 14% of the U.S. population but representing 44% of new HIV infections in 2009 (CDC, 2011). It is vital to identify barriers to positive health behaviors like consistent condom use and HIV testing. This study focus on factors impacting attitudes towards HIV testing, including mistrust of the healthcare system, measured by support for HIV conspiracy theories (Thomas & Quinn, 1991). It also examined the prevalence of HIV conspiracy beliefs among African American college students, along with their perceptions of racism. The first goal of this study was to determine if perceived racism and HIV conspiracy theories are predictors of HIV testing attitudes. The second goal was to examine if perceived racism moderates the relationship between conspiracy beliefs and HIV testing attitudes. The findings indicated that higher levels of HIV conspiracy beliefs were associated with more negative attitudes towards HIV testing. The association between perceived racism and testing attitudes was marginally significant, while the interaction between perceived racism and testing was not significant. Implications for research and HIV interventions are discussed.
