A small number of those bilingual individuals may present with stuttering, a disorder of speech fluency characterized by various speech and non-speech behaviors that interfere with the forward flow of speech. Although there is a large body of research in bilingualism, there is limited literature on stuttering in bilingual speakers. Furthermore, recent resources for speech-language pathologists (SLPs) lack the latest information about assessing and treating bilingual clients who stutter. My goal is to create a clinical handbook which incorporates the most current research to provide appropriate assessment and treatment guidelines for Spanish-English (SE) clients, as it is the fastest growing population in the U.S., takes into account cultural considerations and offers activities for SLPs and graduate students seeking to complete their master's degree in this field. This handbook will include research and techniques on how to implement the Lidcombe program along with the demands capacity theory for pre-school bilingual children who stutter. In addition, this handbook will include information on motivation, education, identification, modification which consists of fluency shaping and stuttering modification techniques, and desensitization for the bilingual school-age and adult client.

Book excerpt: As the number of bilinguals and multilinguals continues to grow globally, researchers and clinicians in the field of speech-language pathology are faced with a unique challenge and opportunity - the need to understand the experiences of bilingual and multilingual clients who stutter. Yet, research and clinical guidelines about assessment and treatment of bilinguals who stutter has been scarce. There is a critical need for research to explore the types of stuttering produced by bilinguals in each of their languages. This research study has two aims: (1) to compare and contrast stuttering characteristics in two languages (English and Spanish) and across bilingual participants and (2) to investigate the effect of language dominance and proficiency on stuttering patterns in each language and across individuals. Participants were two adults who stutter who were both bilingual Spanish-English speakers enrolled in an ongoing clinical research program at PSU. Data were collected from speech samples that were recorded during therapy sessions, and disfluency counts, iterations of stuttering, and physical concomitants were coded. Results demonstrated no statistically significant differences in the overall frequency and disfluency types across languages, the iterations of stuttering across languages, or the physical concomitants across languages. The results of this study provide evidence for correlation between concurrent treatment in both languages and similar stuttering patterns in both languages. They also support the utilization of one language in the evaluation of balanced bilingual adults who stutter.

Book excerpt: The purpose of this systematic review was to analyze the descriptions of bilingual Spanish-English (SE) participants provided in stuttering literature to determine whether critical factors were used to define bilingualism and appropriate criteria were used to classify stuttering. The method included a systematic search of published studies that included bilingual Spanish-English participants who stutter and reviewed the level of detail provided regarding language history, function, proficiency, stability, mode, accent, covert speech, and affective factors. The identification and diagnosis of stuttering was also analyzed using disfluency factors that were reported across the studies. Those factors included formal diagnosis, monolingual guidelines, self-report, parent/teacher concern, informal observation, family history, and disfluency types. Ten studies qualified to be included the bilingualism analysis, consisting of over 15 different speakers who were identified as bilinguals who stutter. Nine out of the 10 studies qualified to be included in the stuttering analysis of the systematic review. Of the 10 studies analyzing bilingualism, the most frequently reported language profile information involved language proficiency, language history, and language function. Affect, accent, mode, and language stability information were the least mentioned language factors in the studies. Of the nine studies included in the disfluency analysis, the most commonly reported disfluency profile information involved disfluency types, comparison to monolingual normative data, and informal observation. The least reported disfluency factors in the studies were formal diagnosis, parent/teacher concern, self-reported stutterer, and family history of stuttering. Results demonstrate that the definition of bilingualism and the classification of stuttering among bilinguals is lacking and inconsistent.
