Book excerpt: Presently, and in the past, the predominant investigative emphasis among research workers in arteriosclerosis has been on plasma and arterial lipids. Recent data from a number of laboratories suggest that arterial mesenchyme is of considerable importance in the pathogenesis and fate of arteriosclerotic lesions. The significance of some of these observations made it clear that there was need for intensified research on the connective tissue components of the arteriosclerotic lesion and that arteriosclerosis research workers could benefit from a more comprehensive view of the subject. Because of their experience in the field of arteriosclerosis and their interest in stimulating new directions for research on the lesion, the Committee on Coronary Artery Lesions and Myocardial Infarctions of the Council on Arteriosclerosis, American Heart Association, planned an International Workshop on Arterial Mesenchyme and Arteriosclerosis. The Workshop brought together scientists expert in connective tissue research and research on arteriosclerosis who presented the current status of knowledge in their areas of expertise. The Workshop was held April 2-3, 1973 at the Royal Orleans Hotel, New Orleans, Louisiana and was attended by more than 170 people. The twenty papers and discussions presented in this volume summarize the proceedings of the Workshop and represent a comprehensive review of the role of arterial mesenchyme in arteriosclerosis.

It describes structural and functional properties of the cerebral circulation that are unique to the brain, an organ with high metabolic demands and the need for tight water and ion homeostasis. Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.

Book excerpt: A vast literature has been concerned with arteriosclerosis and yet, many aspects of pathogenesis and of the mechanism of development of the arteriosclerotic vascular lesion remain only poorly understood. In recent years, our knowledge of the earliest stages of arteriosclerosis have greatly improved. By now, we have learned to relate morphologic changes to disturbances in function. It has been of particular impor tance that components of the arterial wall could be analyzed in regard to dysfunction, for example, in the endothelium or the vascular smooth muscle. The interaction of the different morphological components of the vascular wall could thus be much bet ter understood. Likewise, the interaction between the arterial wall and the flowing blood could be much better described, including the intimate relationship between platelets and the endothelium, the coagulation system and the endothelium, the granulocytes and the endothelial cell layer, as well as processes of migration of blood cells into the subendothelial space. The recognition of functional and morphological disturbance has attained clinical significance not only because the arteriosclerotic diseases have quantitatively reached the dimensions of an epidemic, that is, of a magnitude never been witnessed. It is also because of the development of new drugs that interfere with the atherogenic process and thereby prevent the development of the disease or halt its progression. The Conference was held under the auspices of the International Society of Cardiology, the International Cardiology Foundation and the European Atherosclerosis Group. The aim of the Conference was to achieve a synthesis of present knowledge concerning arterioscle rosis. Therefore, workers were brought together from several coun tries and from various disciplines that do not ordinarily intercom municate for free exchange of data and ideas. Six broad subject areas were introduced by single papers; three of them are included in this volume. In the discussion which followed each formal pres entation, the participants attempted to reconcile disparate data and interpretations and to reach .a clear identification of important areas of ignorance and of crucial questions for future research. The format of the proceedings does not follow precisely that of the Conference itself. The formal papers are included, somewhat ab breviated, and excerpts of the discussion have been gathered under a series of topics arranged in logical sequence. Therefore, the quoted statements do not necessarily appear in order or in the place in the program where they were made. Principal issues, syntheses and unanswered questions are interspersed among the topics as edi torial comments.

Book excerpt: The major cause of death in the Western world is some form of vascular disease; and principal among these forms is atherosclerotic heart disease (ASHD). Although much is known about the etiology and treatment of ASHD, there is, as yet, no specific means of prognosis of an impending coronary episode. There are, however, several indications of susceptibility to coronary disease, generally known as risk factors, the foremost of which is hyperlipidemia. Hyperlipidemia is more commonly designated as hypercholesteremia or triglyceridemia, depending upon which moiety is elevated, but since lipids are transported in the blood as members of a lipoprotein complex, the most descriptive general term would be hyperlipoproteinemia. This volume represents an effort to elucidate the origins and metabolic behavior of lipoproteins and their components, to describe aspects of the morphology, biochemistry and experimental induction of ASHD, and to describe modalities of treatment. The contributions to this book include descriptions of cholesterol synthesis and metabolism, as well as the metabolism of bile acids, the principal products of cholesterol metabolism. There are also chapters on the mechanisms of hyperlipidemia and on lipoprotein metabolism. The induction of experimental atherosclerosis and the aortic structural changes caused by this disease are discussed.

For this reason a fairly large research effort has gone into the investigation of LDL over the past few years. The purpose of this book is to collect and summarize in one place most of the pub lished information on LDL through 1975. To this end more than 1500 references are cited in the papers that make up this volume. The A, B, C apolipoprotein classification system was adopted for use throughout this work. In addition to the A, B, C, and "D" families of apolipoproteins, apoE is used to designate the "arginine-rich" apolipo protein. This classification system is used because it is far less cumber some than other proposed classification schemes for apolipoproteins.
