Available in PDF, EPUB and Kindle. Book excerpt: Surgery as a medical discipline has from its beginnings appealed to the imagination of many. It is therefore not surprising to find that its colourful past has induced quite a few authors to take up their pens. The truth of this in the Netherlands is witnessed by a number of dissertations and monographs and especially by the numerous articles related to the history of surgery which have appeared in the medical weekly Nederlands Tijdschrift voor Geneeskunde, particularly during the two decades preceding the Second World War. The memorial volume, published in 1977 by the 'Nederlandse Vereniging voor Heelkunde' (Association of Surgeons of the Netherlands) has thoroughly covered the history of Dutch surgery since the tum of the century, but a chronological survey of the earlier events which led to these modem achievements is still wanting. This book has been written with a view to meeting this need. In it, Dutch surgery has by no means been taken as an isolated phenomenon, but considered in its context with European surgery as a whole. Foreign influences on the on surgery abroad are discussed Netherlands and, conversely, Dutch influences whilst contemporary medical thinking is set against a cultural and political back ground. It is hoped that this approach will allow the book to exceed the narrow boundaries of'campanilismo' and make it of interest to non-Dutch readers as well.

Book excerpt: After my appointment as head of the Orthopaedic Department in the Surgical Clinic of the Wilhel mina Gasthuis in Amsterdam, January 1953, I have had the privilege of a close and daily co-operation with my second-incharge M. J. Kingma. Amongst other problems, reconstructive surgery with the aid of bone-grafting captured the interest of both of us. In vol. XII, fasc. 3 (1960) of the Archivum of calf bone preserved by refrigeration was Chirurgicum Neerlandicum Kingma wrote: 'The use introduced into surgery in the Netherlands by Meiss in 1951. After the appearance of Meiss' reports on his favourable results, many other surgeons wished to employ this method and in order to cope with the numerous requests for material, a national bone transplantation service for calf bone was in stituted in 1952 under the patronage of the Netherlands Red Cross with Kingma as its medical direc tor'. In 1953 we considered homogenous graft to provide a better 'biological intermedium' than calf bone graft. Consequently we developed a bone-bank for homogenous grafts in our department from that date. After 18 years of experience we have been able to demonstrate clinically and radiologically that in some cases a 'biological fixation' can be maintained with the homogenous graft during the recon struction by 'graduated' function. Kingma controlled the results and arranged the data."

Available in PDF, EPUB and Kindle. Book excerpt: It was a long and troublesome history but an inspiring and excltmg one too-from the time of Zielonko, a Russian pathologist who in 1874 first transplanted muscles, to the present day microneurovascular muscle graft. Today we know that for functional recovery of muscle tissue vascularisation and neurotisation are of equal importance. With the establishment of microsurgery of small vessels and nerves, its application for myopias tic operations has gained increasing interest. It was my intention to bring together leading authorities from all over the world in the field of muscle transplantation. Open questions and unsolved problems had to be discussed between research workers in the basic medical sciences and our clinical colleagues; in this way progress can best be accomplished. This publication is the result of the "Vienna Muscle Symposium" (June 13-14, 1980). It gives an up-to-date viewpoint and should act as a stimulus to all who are interested in this particular field. I wish to express my sincere thanks to those who have contributed to this publication, to the two co-editors J. Holle and B. M. Carlson for their effective cooperation, and to Springer-Verlag Wien for their prompt and most efficient work.

Available in PDF, EPUB and Kindle. Book excerpt: Packed with original color illustrations and videos, Interdisciplinary Cleft Care: Global Perspectives draws from the rich national and international relationships between the Global Smile Foundation and world experts in cleft care to provide comprehensive, clear, and user-friendly content for all cleft care professionals. This text is designed to be an inclusive resource that addresses the educational needs of all cleft care providers, from novice learners looking to develop their area’s first multidisciplinary cleft team to seasoned specialists looking to improve their outcomes. While most other educational cleft care books on treatments and techniques are relevant in high-resource settings, they may not be applicable in low and middle-income countries where orofacial clefting is most common. Interdisciplinary Cleft Care: Global Perspectives details multiple treatment modalities and surgical techniques that can be applied in low, medium, and high-resource settings alike for all key challenges in cleft care. This book has a team-based framework with chapters that address pertinent topics in cleft care by national and international authorities on the subject from plastic surgery, facial plastic surgery, pediatric otolaryngology, oral maxillofacial surgery, speech-language pathology, dentistry, orthodontics, psychology, and more. This text also brings attention to special topics often ill addressed in traditional cleft care resources, including psychosocial issues, safety and preparedness in low-resource settings, and guidance for building a multidisciplinary cleft care team where needed. The book includes access to 60 high-quality surgical videos to support the techniques reviewed in the chapters.

Book excerpt: Refined Carbohydrate Foods and Disease: Some Implications of Dietary Fibre addresses the geographical distribution and historical emergence of the characteristically western diseases, which may be accounted for on a common dietary basis, particularly, on deficiency of dietary fiber. The book is divided into 10 parts, each with number of chapters focusing on relationship between disease and the environment; refined carbohydrate foods; various diseases of the large intestine; and other diseases that are associated with constipation, straining at stool, and refined carbohydrate foods. The book shows the role of refined carbohydrate foods as a significant cause of diseases, because it removes fiber, which has many unsuspected physiological functions. This reference is deemed to be useful to forge stronger links between epidemiologists and experimental workers.
