L'objectif secondaire était d'évaluer l'intérêt d'un dépistage systématique de ce syndrome dans cette population. Méthode : étude prospective sur 102 patients ayant été hospitalisés en unité de soins intensifs de cardiologie pour un syndrome coronarien aigu. Un syndrome d'apnée obstructive du sommeil a été recherché chez tous les patients par un enregistrement polygraphique plus ou moins polysomnographique sur la période de février 2013 à février 2014. Pour l'objectif secondaire, l'ensemble de la population a été suivi sur un an. Les événements cardio-vasculaires indésirables ont été recensés: le décès d'origine cardiovasculaire, l'accident vasculaire cérébral, la récidive coronarienne, une angioplastie non programmée, une hospitalisation pour une insuffisance cardiaque congestive. Résultats : Le syndrome d'apnée obstructive du sommeil a été diagnostiqué chez 33 patients, soit une prévalence de 41,25%. L'âge moyen de ces patients était de 65 ans, avec une prédominance masculine. On note une prédominance de patients hypertendus dans cette population par rapport aux patients sans syndrome d'apnée obstructive du sommeil (60,61% vs 31,91%, p=0,0109). On retrouve une association entre le syndrome d'apnée obstructive du sommeil et la sévérité de l'atteinte coronarienne: 39,40% de lésions tritronculaires chez les patients amnésiques vs 17,02% chez les patients non amnésiques, p=0,0252. Pour l'objectif secondaire, nous n'avons pas retrouvé plus d'événements cardiovasculaires indésirables chez les patients apnéiques non traités par rapport aux patients apnéiques traités ou chez les patients non apnéiques. Conclusion : notre étude retrouve une forte prévalence du syndrome d'apnée obstructive du sommeil chez les patients coronariens, par rapport à celle dans la population générale. Bien que nous n'ayons pas retrouvé d'évolution péjorative au niveau cardiovasculaire chez le patient coronarien apnéique non traité et d'amélioration de la morbi-mortalité par le traitement, une forte association a été mise en évidence entre le syndrome d'apnée obstructive du sommeil et le syndrome coronarien aigu. Cela conforte l'idée d'un dépistage systématique chez le patient coronarien.
