Book excerpt: Les troubles des conduites alimentaires (TCA) concernent essentiellement l'anorexie mentale et la boulimie, situées à des pôles opposés d'une même activité, celle qui consiste à s'alimenter. Entre ces deux affections oscille une troisième entité, mixte, appelée anorexie de type boulimique. Cet état intermédiaire, qui possède certaines caractéristiques de l'une et de l'autre, est marqué par des périodes anorectiques entrecoupées de crises boulimiques. Chez la personne anorectique-boulimique, l'ambivalence prédomine ; elle aime et elle n'aime pas les aliments, elle veut vivre tout en échappant à la matérialité du corps. Dans 20 à 50 % des cas, des sujets féminins en majorité, nous observons le passage d'une phase restrictive débutant souvent à l'adolescence à une phase d'alternance avec des crises de boulimie suivies de vomissements. Les raisons de cette évolution mal comprise et redoutée ne sont pas connues. S'il existe peu de travaux scientifiques à ce sujet, des chercheurs tentent de préciser les mécanismes psychopathologiques, neurobiologiques et environnementaux impliqués ainsi que les facteurs de risque afin d'améliorer la qualité de la prise en charge des patients. En effet, malgré un pronostic de guérison équivalent chez les jeunes femmes souffrant d'anorexie restrictive et celles présentant une anorexie-boulimie, les risques de complications parfois fatales (suicide) sont plus élevés dans le deuxième cas. A la lumière des données de la littérature, l'analyse de situations cliniques concernant des prises en charge de patientes souffrant d'anorexie de type boulimique en psychiatrie ambulatoire nous permet de mettre en évidence plusieurs facteurs prédictifs de cette évolution. Le passage à l'anorexie boulimique serait de deux ordres. D'une part, il serait directement lié aux carences nutritionnelles et aux remaniements neurobiologiques (sérotoninergiques et dopaminergiques ), soit consécutifs à la phase de restriction alimentaire, soit sous l'influence du patrimoine génétique. D'autre part, il serait indirectement modulé par des facteurs psychologiques individuels et environnementaux dont les comorbidités psychiatriques (TOC et dépression surtout), l'intolérance à la frustration, l'impulsivité, le trouble de personnalité de type limite, les antécédents d'abus ou de maltraitance physique. Comment anticiper une telle évolution afin d'en prévenir les complications? Comment dépister cette forme mixte, souvent découverte tardivement? Telles sont les questions que pose notre expérience clinique quotidienne et qui doivent faire l' objet d'études pour guider la prise en charge
