Book excerpt: L'analyse de la fonction systolique en échocardiographie représente un challenge majeur depuis une vingtaine d'années dans le but d'améliorer la détection et la quantification des anomalies de la contraction myocardique. L'analyse des déformations myocardiques consiste à étudier le pourcentage d'étirement ou de raccourcissement myocardiques au cours du cycle cardiaque. Après le grand pas en avant dans ce domaine, consécutif à l'avènement du Doppler tissulaire il y a un quinzaine d'années, ces 4 dernières années ont vu naître une nouvelle technique d'analyse bidimensionnelle des déformations, porteuse de grands espoirs car s'affranchissant de la limitation liée à l'angle d'incidence Doppler et basée sur le suivi des marqueurs acoustiques de la paroi myocardique : le « speckle tracking echocardiography » ou « 2D strain ». Cette thèse rapporte tout d'abord une série de travaux orientés sur la validation de ce nouvel outil. Nous avons ensuite appliqué expérimentalement cette technique pour évaluer sa capacité à détecter l'ischémie myocardique lors d'un examen de stress sous dobutamine en comparaison à l'analyse de perfusion myocardique en échographie de contraste. Enfin, nous exposons plusieurs expériences cliniques qui témoignent de quelques applications potentielles de cette technique dans la prise en charge des patients porteurs de cardiopathies diverses : myocardiopathie hypertrophique, fibrillation auriculaire paroxystique et recherche de viabilité sous faibles doses de dobutamine chez les patients porteurs d'une cardiopathie ischémique.

Le dépistage de la maladie coronaire athéromateuse des vaisseaux épicardiques est non invasif et basé sur l’analyse de la cinétique du myocarde au cours d’un stress mais il n’a pas une sensibilité suffisante pour détecter une anomalie isolée de la microcirculation. Le test au froid au cours d’une échocardiographie pourrait donc être utilisé pour étudier la fonction endothéliale de façon non invasive. Notre étude caractérise la cinétique des marqueurs hémodynamiques et celle de la déformation myocardique à l’aide d’une échographie associée à une technique d’analyse des champs de déplacements (speckle tracking), lors d’un test de provocation au froid dans une population normale. Nous montrons que premièrement, la réponse physiologique lors d’un test au froid se caractérise essentiellement par une élévation significative de la post charge avec une majoration de 35% de la PAm et peu de modification de la FC; et deuxièmement, une réponse biphasique de la mécanique cardiaque avec initialement une brève élévation des valeurs de strain longitudinal et radial petit axe, suivie d’une diminution progressive. Ces variations de strains sont corrélées aux variations hémodynamiques engendrées par le test au froid (FC, PAm et DC). De nouvelles études seraient nécessaires pour analyser les variations du strain, au cours d’un CPT, chez des patients porteurs d’une dysfonction endothéliale.

La fonction ventriculaire droite est un marqueur pronostique puissant dans cette pathologie. Le manque de robustesse des outils échocardiographiques actuels pour l'étude du ventricule droit nécessite de multiplier les paramètres mesurés. L'objectif de notre étude est de déterminer les performances diagnostiques de la déformation myocardique en mode 2D-speckle tracking pour le diagnostic de la dysfonction ventriculaire droite chez les patients présentant une cardiopathie dilatée. Méthode : 76 patients présentant une cardiopathie dilatée (volume télédiastolique ventriculaire gauche ≥ 75ml/m2) avec dysfonction ventriculaire gauche (fraction d'éjection ≤ 45%) ont bénéficié d'une échographie cardiaque complète avec étude de la déformation myocardique. La faisabilité, la reproductibilité et les performances diagnostiques de la déformation myocardique ont été évaluée et comparées aux autres méthodes échographiques d'évaluation du ventricule droit. L'angioscintigraphie des cavités cardiaques était la technique de référence. Résultats : La déformation myocardique moyenne du ventricule droit était de -21,0 +/- 5,7% en l'absence de dysfonction ventriculaire droite et de -15,8 +/- 5,1% en cas de dysfonction (p = 0,0001). Un seuil à -16% faisait le diagnostic avec une sensibilité à 60,5% et une spécificité à 87,5%. La faisabilité de la technique était de 92,1% sur la paroi libre du ventricule droit et de 90,6% sur la paroi inférieure. La reproductibilité de la technique était bonne (coefficients de variation intra et inter observateur à 5,5% et 10,7% respectivement sur la paroi latérale, et 8,8% et 10,9% sur la paroi inférieure). Faiblement corrélée au TAPSE (R = -0,39) , à la FRSVD (R = -0,37), à l'onde S latérale (R = -0,29), ou à l'accélération isovolumique (R = -0,13) et plus performante, la déformation myocardique pourrait être un outil complémentaire pour le diagnostic précoce de dysfonction ventriculaire droite. Conclusion : Dans la cardiopathie dilatée, la déformation myocardique ventriculaire droite apparaît comme une technique prometteuse pour diagnostiquer la dysfonction du ventricule droit, évaluer le pronostic et guider la stratégie thérapeutique.
