Book excerpt: Knee replacement surgery is often performed in cases where the pain due to knee osteoarthritis can no longer be effectively controlled by conservative measures. The surgery-which replaces the articular surfaces of the tibia, femur and sometimes the patella with a mix of metal and polyethylene-is one of the most commonly performed lower limb surgeries. Despite patient reports of reduced pain and increased participation in activities of daily living, knee function following total knee arthroplasty (TKA) does not return to normal. Reductions in both passive and active knee range of motion, alterations in magnitude and timing of muscle activity, and changes in knee joint kinetics have all been reported. Comparatively little is known about why knee function is altered following TKA. In an attempt to address this gap in the literature, this thesis was designed to examine the biomechanical factors thought to contribute to reduced knee extensor moments following TKA. In particular, the contribution of alterations in passive knee moments, muscle moment arm lengths, and muscle activation magnitude and timing were examined in detail to determine which factors contributed to reduced knee extensor moment following TKA.

In spite of many articles in scientific journals, an outstanding monograph, and several excellent textbook chapters, the patella is still an enigma in many respects. The etiology of patellar pain is controversial, and there is no completely satisfying explanation for its cause or its relationship to chondromalacia. Curiously, neither the widespread use of arthroscopy nor the advent of newer diagnostic tests such as CT scanning and magnetic resonance imaging have cast much light. Without a better understanding of why patellar disorders occur it is not surprising that there is no consensus on how to fix them. Arthros copy has contributed little except to the patient's psyche. The currently most popular surgical treatment for recurrent dislocation of the patella was first described 50 years ago. One concrete advance, albeit a small one, is a better understanding of the role of anatomical abnormalities and patellofemoral dysplasia in patellar instabilities. However, the survivorship of hip, knee, shoulder, and other implants is limited. This includes new implant designs and materials, bearings concepts and tribology, kinematical concepts, surgical techniques, and anti-inflammatory and infection prevention strategies. A careful evaluation of new materials and concepts is required in order to fully assess the strengths and weaknesses and to improve the quality and outcomes of joint replacements. Therefore, extensive research and clinical trials are essential. The main aspects that are addressed in this Special Issue are related to new material, design and manufacturing considerations of implants, implant wear and its potential clinical consequence, implant fixation, infection-related material aspects, and taper-related research topics. This Special Issue gives an overview of the ongoing research in those fields.
