There is a clear clinical need for better asthma management programs in the pediatric primary care setting (Akinbami et al., 2011; Sleath et al., 2011). The current system for asthma management in primary care offices is not improving outcomes for pediatric asthma patients in the United States or the state of Montana. Barriers to providing comprehensive evidence-based pediatric asthma care exist in the primary care setting nationwide (US DHHS, 2010; & Meyer, 2011). The creation of an evidence-based pediatric asthma toolkit for the primary care provider based on the EPR-3 Asthma Diagnosis and Management Guidelines (NIH, NHLBI, 2007) is an attempt to alleviate prevalent barriers primary care providers face in pediatric asthma care. A toolkit was assembled based on literature reviewed, interviews with experts in the field, and suggestions from primary care providers after reviewing the tool. The toolkit was found to be evidence-based, appropriate for use in the primary care setting, efficient for use in a busy primary care setting, and desired for use by primary care providers in the state based on informal interviews and reviews. It is suggested that the Pediatric Asthma Toolkit for the Primary Care Provider be tested using a formal research study for its' effectiveness in improving pediatric asthma care in the primary care setting as well as improving asthma outcomes in the state of Montana. More research needs to be done in the U.S. to evaluate the effectiveness and use of asthma toolkits by primary care providers. The toolkit will need to be updated when new research is published on asthma care as well as when new national asthma guidelines are issued. The toolkit will also need to be updated with current available pharmaceuticals and devices for asthma. This professional project is in alignment with the national Asthma Control Goals for Healthy People 2020 as well as the National Asthma Education and Prevention Program recommendations for implementation of the EPR-3 Guidelines. It is also in alignment with the Montana Asthma Control Program's goals to create partnerships throughout the state and improve asthma outcomes for Montana.

An estimated 5 million U.S. children have asthma. Too many of these children are unnecessarily impaired. Much of the money spent on asthma is for high-cost health care services to treat acute periods of illness. Many asthma attacks could be avoided--and much suffering prevented and many medical costs saved--if more children received good-quality, ongoing asthma care and if the 11 policy recommendations presented in this report were implemented in a oordinated fashion. A national call to action, the policy recommendations span public and private interests and compel integration of public health activities across local, state, and federal levels. This report summarizes the findings of an effort funded by the Robert Wood Johnson Foundation as part of the Pediatric Asthma Initiative, whose purpose is to address current gaps in national childhood asthma care. It is the first national initiative that simultaneously addresses treatment, policy, and financing issues for children with asthma at the patient, provider, and institutional levels. The purpose of RAND's effort was to:--identify a range of policy actions in both the public and private sectors that could improve childhood asthma outcomes nationwide--select a subset of policies to create a blueprint for national policy in this area--outline alternatives to implement these policies that build on prior efforts.The effort developed a comprehensive policy framework that maps the identified strategies to one overall policy objective: to promote the development and maintenance of asthma-friendly communities--communities in which children with asthma are swiftly diagnosed, receive appropriate and ongoing treatment, and are not exposed to environmental factors that exacerbate their condition. This report is intended as a working guide for coordinating the activities of both public and private organizations at the federal, state, and local community levels.

Book excerpt: "It is imperative that emergency department providers who care for pediatric patients presenting with an asthma exacerbation be experts in the evaluation and treatment of this special population. Providers need to be able to recognize a patient presenting in acute distress, provide a rapid triage assessment, determine the patient's level of distress and immediately initiate the appropriate medical treatment based on the severity of symptoms. The project objective was to address the need for improved emergency department (ED) assessment and treatment of the pediatric asthma patients presenting with an acute exacerbation. The objective was met by offering providers formal education and implementing an evidence-based pediatric asthma assessment tool and treatment protocol. ED providers including nurses, physicians and mid-level providers participating in the project were provided classroom education and hands on skills training based on a standardized evidence-based asthma treatment protocol that was developed to improve management of pediatric asthma exacerbations in the ED. An asthma skills assessment and knowledge test was given to each participant before and after the training session. At the completion of the training session, an evidence based pediatric asthma protocol was implemented in the department. A total of 26 providers participated in the project. The mean score on the pre Pediatric Asthma Knowledge Assessment skills assessment was 80.12% with a post mean score of 92.82. Based on a paired t-test, the results were significantly significant. The mean score on the Pediatric Asthma Skills Assessment was 89.24% with a post mean score of 92.92. While the results of the post Pediatric Asthma Skills Assessment improved, the improvement was not significantly significant. A retrospective chart review of all pediatric asthma patients, treated by a project participant, showed an 85.71% rate of compliance with order set initiation. This exceeded the internal organizational benchmark, which was set at 75%. Based on the findings, staff's knowledge related to the management of pediatric asthma exacerbations in the ED showed statistically significant improvement after participating in a formal education session. There was also significant compliance with the initiation of the pediatric asthma order set. Although the improvement was not significantly significant, it exceeded the organization internal benchmark." -- Abstract.

Book excerpt: The Expert Panel Report -3 (EPR3) NAEPP 2007 evidence-based clinical asthma guidelines were developed to provide evidence-based high-quality patient care that leads to improved outcomes. A literature review showed that healthcare providers do not routinely follow the asthma guidelines. The purpose of this project was to develop and implement an evidence-based asthma electronic health record (EHR) template in a pediatric office to improve provider compliance to the guidelines resulting in improved outcomes for children with asthma. The study was conducted over a period of four months from January - April 2016. An EHR asthma protocol template and training for providers using a PACE program (physician asthma care education) on current guidelines was provided. A retrospective EHR audit measuring provider's compliance was performed. Pre/post aggregate data for documentation specific to asthma was collected and analyzed using the chi square method. The outcome objectives from this quality improvement study focused on provider compliance and asthma control. Results indicated the EHR template significantly improved provider documentation in compliance with 7 of the 8 areas measured.

Book excerpt: Asthma is a common chronic disease of childhood, affecting more than six million children (NHLBI, 2007). Several national guidelines regarding asthma diagnosis and treatment are available. Two of the most accepted are the National Heart, Lung, Blood Institute (2007) and the Department of Defense and Veteran's Affairs (DoD/VA, 2009) clinical practice guideline (CPG). These guidelines assist Health Care Providers (HCP's) in delivering the most current and accepted standards of practice for patients with asthma. However, compliance is often difficult for HCPs as well as patients. The purpose of this evidence-based project was to implement the DoD/VA asthma CPG at a Military Treatment Facility pediatric clinic. Compliance with the asthma CPG was evaluated in 5 specific areas: emergency department visits, acute care visits, hospitalization rate, use of a written asthma action plan and prescription of inhaled corticosteroid for any severity of persistent asthma. The Stetler Model of Research Utilization (2001) was used to facilitate this project. After literature review and an organizational assessment, this author utilized a combined approach of provider education and extrinsic tools to implement the CPG. A 3-month retrospective chart review was completed prior to and three months after CPG implementation. The two groups had similar ages, sex, and sponsor status. Following implementation of the CPG, there was a 6% decrease in the percentage of ED visits. However, there was a slight increase in rates of acute care visits and hospitalizations (2% and 6%, respectively). A small (2%) increase was also noted in the use of ICS. Unfortunately, post-CPG implementation data collection coincided with the winter months, which may have been a confounder. There was a larger increase in use of written asthma action plan post-CPG implementation (14 to 44%). Using the z-test of proportions, only the increase in the use of asthma action plan was statistically significant. Although there was not significant improvement in the majority of outcomes measured, the primary goal of this project was implementation of the asthma CPG. No one method of CPG implementation has been proven to be more successful than another method according to the literature. Through the process of this evidence-based practice project, Providers have voiced an increased awareness of the DoD/VA CPG. Although not formally measured, this provides a basis for future in depth study and analysis. This project will be sustained via ongoing process improvement monitoring and reporting.

Book excerpt: Following a 2006 study noting that physician compliance with the NHLBI guidelines in the South Bronx needed improvement, particularly among primary care providers (PCPs) in private practice, a re-examination was expanded to other areas of the Bronx and to San Diego to analyze the awareness of the 2007 Expert Panel Report 3 (EPR-3). A 10-point questionnaire highlighting specific areas of focus in EPR-3 was administered to 52 community-based, 49 hospital-based PCPs and 33 residents practicing in inner-city neighborhoods in the Bronx, and to 36 community-based, 12 hospital-based PCPs and 8 residents practicing in urban areas of San Diego. Comparison of total scores between community-based with hospital-based PCPs and residents in the Bronx showed a trend (p=0.06) towards lower rates of compliance among community-based practitioners, particularly for prescribing a spacer with an inhaler (p=0.0004). Preferred prescription of pills over inhalers, and low rates of referral to community health professionals for asthma self-management education were observed in all three groups in this area. In San Diego, residents demonstrated a lower overall compliance (p=.004), particularly for preferred prescription of pills over inhalers and acknowledgement of ethnocultural differences in treatment of asthma. Low rates of referral to community health professionals for asthma self-management were observed in all groups; however, the effect was most prominent in the residents in comparison to private practitioners.

Book excerpt: These guidelines are the product of a new Pediatric Asthma Initiative aimed at integrating environmental management of asthma into pediatric health care. This document outlines competencies in environmental health relevant to pediatric asthma that should be mastered by primary health care providers, and outlines the environmental interventions that should be communicated to patients. These environmental management guidelines were developed for pediatricians, family physicians, internists, pediatric nurse practitioners, pediatric nurses, and physician assistants. In addition, these guidelines should be integrated into respiratory therapists' and licensed case/care (LICSW) management professionals' education and training. The guidelines contain three components: (1) Competencies: An outline of the knowledge and skills that health care providers and health professional students should master and demonstrate in order to incorporate management of environmental asthma triggers into pediatric practice; (2) Environmental History Form: A quick, easy, user-friendly document that can be utilized as an intake tool by the health care provider to help determine pediatric patients' environmental asthma triggers; and (3) Environmental Intervention Guidelines: Follow-up questions and intervention solutions to environmental asthma triggers. Although environmental factors may play a role in the prevalence of asthma in the population, these guidelines are not directed at the primary prevention of pediatric asthma on a general scale. They are aimed instead at educating health care professionals on how to advise families about environmental interventions that can reduce or eliminate triggers for children who are already diagnosed with asthma. These guidelines are intended to be used with children (0-18 years) already diagnosed with asthma. Referral to a specialist is advised if the diagnosis of asthma is in doubt. Sources of guidelines for making the diagnosis of asthma include the NAEPP (National Asthma Education and Prevention Program) Guidelines and resources from Kaiser Permanente, the American Academy of Allergy Asthma & Immunology, and the American Academy of Pediatrics. Integrating behavioral health into primary care medical settings has been shown to: (1) produce healthier patients; (2) produce medical savings; (3) produce higher patient satisfaction; (4) leverage the primary care physician’s time so that they can be more productive; and (5) increase physician satisfaction. For these reasons this is an emerging paradigm with a lot of interest and momentum. For example, the President’s New Freedom Commission on Mental Health has recently endorsed redesigning the mental health system so that much of this is integrated into primary care medicine.
