His papers were placed -in my hands by his son, but I found so little that had not been used by him, that I felt that any further investigation of the subject must be prefaced by research in another direction. The statistics of amputations, upon which Sir James founded most of his argument, have not been disputed as far as those derived from hospitals are concerned. Those which were contributed by private practitioners were, how ever, manifestly open to the objection that the records might be imperfect; and, at least, that they were not sufficiently extended to be convincing. Book excerpt: The main topic of my dissertation is empirical analysis in health care quality delivery and improvement. This dissertation is comprised of three essays, shown in chapters 2, 3, and 4, respectively. In the first essay, I study the determinants of meaningful use achievement of U.S. hospitals. Using archival hospital-level data, I observe that hospitals where the CIO has oversight of health information management (HIM) are more likely to achieve meaningful use. I also observe that standalone hospitals are less likely to achieve meaningful use, relative to their counterparts that belong to a health network or system. My results also indicate that there is no association between IT spending and hospital achievement of meaningful use. My research represents one of the first studies on the role of hospital operational characteristics and their association with achievement of meaningful use. In the second essay, I study the effect of intra- and post-operative factors on patient health care outcomes of coronary artery bypass grafting (CABG) surgery. Significant variation exists in the operative mortality of CABG patients. Based on a unique data set provided by the Texas Quality Initiative (TQI), I develop a predictive model to identify intra- and post-operative factors that are significantly associated with operative mortality risk of CABG surgery. Patients who undergo hybrid coronary surgical and interventional procedures, receive anti-fibrinolytic, experience none aortic occlusions with beating heart, unplanned procedures, or in-hospital post-operative reoperative(s) are more likely to face higher operative mortality risk. In the third essay, I study the readmission patterns of CABG patients. Utilizing (re)admission records across hospitals and health systems over time in North Texas, I estimate (i) propensity of 30-day readmission after CABG surgery; (ii) frequency of 30-day readmission for patients; and (iii) the length of time between any consecutive readmissions. I find that patient demographic characteristics, insurance type, length of stay during CABG surgery hospitalization, and comorbidities are important predictors of readmission risk. Moreover, other variables, indicating whether a patient is readmitted within 30 days of discharge from the CABG surgery admission, total charges of a hospitalization, and discharge destination, also significantly affect patient readmission rates.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.
