The author then goes on to his main topic, the causes of disease, especially focusing on the current belief of the "malarious" or "miasmatic" origin of disease. He firmly states that, after many years of observation and medical practice, he is convinced that the greater incidence of sickness in marshy, lowland areas is not due to miasm, an exhalation from decaying vegetable matter. He explains his conclusion by detailing the conditions of scientific hypotheses and inductive reasoning and then applying them to an assembly of medical facts and reports on miasma.
