La question centrale de cette thèse est d'envisager si l'incapacité chronique des patients souffrant de la maladie d'Alzheimer à utiliser le langage articulé s'accompagne d'une détérioration du registre gestuel. Pour répondre à cette question, nous avons utilisé une tâche d'interaction sociale engagée à partir d'un matériel pictural. Cette situation se veut refléter le plus écologiquement possible des situations de communication au quotidien. Globalement, on observe que, si la production verbale des patients Alzheimer est déficitaire, leur production de gestes liés au discours n'est pas atteinte. Les résultats vont dans le sens d'une atteinte différenciée des capacités de communication verbales et gestuelles des patients et penchent en faveur de l'existence de deux systèmes de communication parallèles pouvant interagir entre eux à différents moments de l'élaboration du message. De surcroît, les résultats suggèrent que les patients présentant un niveau très sévère de la pathologie peuvent compenser l'atteinte lexicale par l'augmentation de la production de gestes déictiques, qui permettraient de rendre visible le référent verbal. Des perspectives fondamentales comme cliniques peuvent être envisagées afin que les gestes liés au discours et plus généralement la communication non verbale puissent constituer un levier thérapeutique dans la prise en soin des patients atteints d'une maladie d'Alzheimer et de leur entourage.

Book excerpt: Ce mémoire a pour but l'étude d'une prise en charge orthophonique innovante visant le " maintien et l'adaptation des fonctions de communication chez les personnes atteintes de maladies neuro-dégénératives ". Il s'agit d'utiliser l'improvisation théâtrale dans l'objectif de stimuler la communication des patients déments. Pour cela, nous avons mis en place, dans une institution, un groupe de théâtre comprenant 10 patientes atteintes d'une démence de type Alzheimer (DTA), à différents degrés de sévérité. A partir des séances filmées, nous avons basé une première analyse sur les corpus des improvisations de trois patientes de trois degrés de sévérité différents afin de dégager un profil de communication verbale et non-verbale à partir de données quantitatives et qualitatives, permettant de mettre en exergue des constantes selon le degré d'atteinte de la démence. Parallèlement, nous avons axé notre observation sur le groupe. La pratique de l'improvisation ne permet pas d'augmenter la quantité d'énoncés produits mais on constate que les interactions sont toujours cohérentes. On remarque une modification du répertoire non-verbal conjointement à l'avancée de la maladie. Pour l'une de nos patientes, les capacités d'expression non-verbale lui permettent d'entrer dans l'improvisation, malgré des difficultés massives de production langagière. L'accent mis sur le travail de l'expression des émotions nous permet de constater que la tristesse s'avère particulièrement difficile à jouer et que ce type de travail dépend beaucoup de la personnalité et de la culture de chacune. On ressent une forte cohésion au sein du groupe et un impact visible de cette prise en charge sur le bien-être des patientes ainsi que sur le regard de l'entourage.
