This document describes a comprehensive survey of air-taxi operators and pilots in Alaska in which company and pilot demographics, flight practices, and attitudes about safety were examined. It provides information about current practices and how industry views potential safety measures, which is critical to designing effective prevention strategies. The National Institute for Occupational Safety and Health, as the national agency responsible for occupational safety and health research, is committed to continuing to reduce the number of fatal occupational aviation crashes in Alaska. Book excerpt: During the 1980s, the National Transportation Safety Board investigated several aviation, highway, and marine accidents that involved operator fatigue. Following completion of these investigations, the Safety Board in 1989 issued three recommendations to the U.S. Department of Transportation (DOT) addressing needed research, education, and revisions to hours-of-service regulations. In the 10 years that have passed, the Safety Board has issued more than 70 additional recommendations to the DOT, States, industry, and industry associations to reduce the incidence of fatigue-related accidents. In response to the three 1989 recommendations, the DOT and the modal administrations have, in general, acted and responded positively to those addressing research and education; little action, however, has occurred with respect to revising the hours-of-service regulations. Nevertheless, the Safety Board believes that support has grown in recent years to make substantive changes to these regulations. This report provides an update on the activities and efforts by the DOT and the modal administrations to address operator fatigue and, consequently, the progress that has been made in the past 10 years to implement the actions called for in the three intermodal recommendations and other fatigue-related recommendations. The report also provides some background information on current hours-of-service regulations, fatigue, and the effects of fatigue on transportation safety. As a result of this safety report, the National Transportation Safety Board issued new safety recommendations to the U.S. Department of Transportation, the Federal Aviation Administration, the Federal Highway Administration, the Federal Railroad Administration, the Research and Special Programs Administration, and the United States Coast Guard. The Safety Board also reiterated two recommendations to the Federal Aviation Administration.

Book excerpt: Aviation is a basic mode of transportation in Alaska. Alaska has six times as many pilots per capita and 16 times as many aircraft per capita when compared to the rest of the United States. There are approximately 736 registered landing areas, in addition to the thousands of lakes and gravel bars where pilots land. Hazards can exist on the ground when aircraft are taking off or landing, and also in the air when sharing congested airspace. During 2011 there were three mid-air collisions in Alaska. One of these collisions occurred in the Matanuska-Susitna (Mat Su) Valley between two seaplanes, fatally injuring four people. In response, the Mat Su Mid-Air Collision Avoidance Working Group was formed with representatives from industry, the military, the Federal Aviation Administration (FAA), Aircraft Owners and Pilots Association (AOPA), Alaska Airmen's Association, the Alaskan Aviation Safety Foundation, the Alaska Air Carriers Association and the National Institute for Occupational Safety and Health (NIOSH). Mapping of Common Traffic Advisory Frequencies (CTAFs), which are the frequencies used by pilots to communicate while operating near an airport without a control tower, in the Mat Su Valley showed a substantial amount of overlap; pilots in an area could be on separate, but correct frequencies and therefore unable to communicate with each other. In addition, there were disparities between various sources of radio frequency information, including the FAA's Alaska supplement, flight charts, and individual airport listings. As a partner in the Mat Su Mid-Air Collision Avoidance Working Group, NIOSH analyzed data from FAA and Aviation Safety Reporting System databases and found very little overlap in reports of near misses and near mid-air collisions from Alaska. Alaska pilots were surveyed and locations of high traffic areas in the Mat Su Valley were identified. Radar data from air traffic control was reviewed to determine common traffic routes. Several scenarios were developed to clarify communication confusion; these scenarios were reviewed and evaluated by local pilots, commercial operators, flight instructors and airport operators.--NIOSHTIC-2
