Pourtant le sentiment de culpabilité a déjà été repéré, comme un obstacle à lademande d'aide de l'aidant, mais ce facteur a été peu exploré dans un axe de recherche. Cette rechercherepose sur l'hypothèse que le sentiment de culpabilité de l'aidant familial, représenterait le principal frein,susceptible d'empêcher sa demande d'aide et de soutien, face à la maladie d'Alzheimer ou à la maladieapparentée de son proche, lorsque le patient vit à domicile. Une sous-hypothèse vise à situer différemment lesentiment de culpabilité de l'aidant familial, selon sa position de conjoint, ou plus largement de descendant(enfant, belle-fille, gendre...), dans la relation d'aide. Pour tester cette hypothèse trente huit entretiens semidirectifsont été réalisés, et complétés par la passation des échelles d'attachement (RSQ), du caregiver(CRA), et de dépression (Beck).Cette recherche vise à expliciter les fondements, et les mécanismes du sentiment de culpabilité des aidantsfamiliaux, en l'articulant à la problématique de perte, qui est coeur de la maladie d'Alzheimer ou desmaladies apparentées. Elle apporte donc un éclairage nouveau sur le travail psychique de l'aidant familial,qui s'écarte de son seul abord sous l'angle du fardeau et de l'épuisement, pour l'envisager à la lumière dutravail du pré-deuil, qui apparaît comme la clé de voûte de la relation d'aide. Par conséquent, la recherchesuivra le cycle de la dépendance du patient, pour dégager à chacun de ses stades, les incidences de la pertedans l'espace psychique et intersubjectif chez l'aidant familial, selon la nature des liens d'attachementdéveloppés avec le patient, mais aussi avec le groupe familial. A partir de là, nous tenterons de relier leregistre principal d'élaboration de la perte, à un profil d'aidant singulier dans la relation d'aide, afin d'éclairerles liens entre ses manifestations de culpabilité, et sa demande d'aide.

Le rôle du médecin généraliste est important dans l’aide et l’accompagnement qu’il peut fournir à ces familles. Dans ce contexte, nous avons souhaité réaliser une étude qualitative avec une série de huit entretiens semi - dirigés afin de découvrir le quotidien de l’aidant qui doit apprendre à gérer la maladie. Les raisons du soutien sont multiples, les aidants évoquent notamment l’attachement et le respect du lien familial. Le proche vit avec le malade et doit faire face à tous les changements qu’entraîne la démence. Les symptômes sont multiples et les troubles du comportement fréquents. La maladie redéfinit le quotidien et désorganisent la famille qui doit en permanence s’adapter à l’arrivée progressive de nouveaux symptômes. Le vécu de la maladie est lourd et difficile mais le maintien à domicile est souhaité pour plusieurs raisons. Il est moins onéreux que le placement et résulte de la volonté de l’aidant de prendre en charge complétement son parent ou son conjoint. La culpabilité d’institutionnaliser l’autre est fréquente et l’image de la maison de retraite reste assez négative. La confiance dans les traitements est faible et les aides utilisées sont très diverses. Le rôle du médecin généraliste reste assez marginal alors qu’il est souvent en première ligne pour aider ces personnes. L’enjeu d’une prise en charge adaptée est de diminuer le fardeau quotidien et de prévenir l’épuisement de l’aidant.

Matériel et méthodes : étude qualitative par les entretiens semi directifs auprès de vingt-sept conjoints-aidants de malades d'Alzheimer non institutionnalisés de Sarthe et Maine-et-Loire. Une analyse thématique a été faite par l'étude du contenu sémantique. Résultats : l'affection était le socle du couple. L'impact des différents conflits et épreuves ayant jalonnés leur vie de couple était relativisé. La satisfaction globale de la vie de couple était préservée. Communication conjugale et vie sociale tenaient des rôles importants pour la bonne santé du couple. La maladie d'Alzheimer se mêlait aux autres pathologies limitantes. L'angoisse de l'avenir était ressentie chez certaines femmes. Le lien conjugal perdurait par un reforcement de la communication non verbale. Etonnamment, la MA n'était pas le cataclysme attendu dans l'histoire du couple. Le mariage induisait une notion de devoir envers l'autre, socle permettant le dépassement de cette épreuve. S'y rattachait un risque de culpabilité en cas de difficulté. Le conjoint donnait un sens à la MA et l'intégrait dans ses représentations. Conclusion : pour mieux aider le conjoint dans son rôle d'aidant, le praticien doit étudier l'histoire du couple en profondeur, établir les liens sociaux et familiaux, évaluer les systèmes de représentations et déterminer l'impact d'autres pathologies. Une consultation semestrielle au domicile peut être proposée.
