Assisted Independent Living (AIL) programmes were designed to keep the elderly in as independent a living environment as possible despite increasing functioning disabilities and frailties that often accompany advancing age. Originally published in 1982, this book defines sheltered housing, traces its development in Western society and analyses its success under several variations in Great Britain. The British analysis focuses on those aspects of the sheltered housing programme that had wider relevance to the development of AIL housing policy in Europe and North America.

The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.

Book excerpt: The Government has not fully thought through the implications of its social care reforms and may leave local authorities open to a deluge of disputes and legal challenges. MPs and Peers warn that without greater integration with health and housing, and a focus on prevention and early intervention, the care and support system will be unsustainable. The Committee also calls for a nationwide campaign to educate people about the need to pay for their own care, saying that adult care and support are poorly understood. Key recommendations include: a new power to mandate joint budgets and commissioning across health, care and housing, such as support for the frail elderly, making it simpler for NHS and local Councils to pool budgets; fast-tracking of care and support assessments for terminally-ill people; new legal rights for young carers to protect them from inappropriate caring responsibilities and ensure they get the support they need; an obligation on the Secretary of State to take into account the draft Bill's well-being principle when designing and setting a national eligibility threshold; independent resolution of disputes over decisions about care and support - and costs that count towards the cap - through a Care and Support Tribunal. In addition, the Committee makes a number of recommendations to improve health research and the education and training of NHS workers. The Committee also warns that restricting support and care to those with the highest levels of need will simply shunt costs into acute NHS care and undermines interventions to prevent and postpone the need for formal care and support.
