Book excerpt: Adolescents living with type 1 diabetes (T1D) begin to adopt a more collaborative approach to managing T1D with their parents before becoming independent in self-management. However, adolescents can encounter difficulties with T1D management and ambiguity in relation to the division of responsibilities for T1D management with their parents. This study aimed to explore: (1) how adolescents living with T1D experience self-management of T1D and share self-management responsibilities with parents and (2) the relationship between adolescent communication with parents about T1D self-management and demographic, clinical and psychosocial variables. A meta-synthesis of qualitative literature on child and adolescent experiences of self-management of T1D was conducted. The findings of the meta-synthesis indicated that balancing the demands of self-management with other aspects of life and sense of control over T1D are important for self-management of T1D. Following this a mixed methods study involving two phases was conducted; semi-structured interviews with adolescents aged 11 to 17 years (n = 28) and a cross-sectional survey with adolescents aged 11 to 17 years (n = 113). In interviews, adolescents highlighted their experiences of gaining independence in self-management in the overarching theme 'Navigating increasing responsibility for self-management during adolescence: "it changed gradually... and I am almost fully doing it [self-managing] now". The following themes contributed to the overarching theme: (1) Changing levels of involvement in self-management, (2) Talking about self-management with parents, (3) Taking ownership of self-management and (4) Environmental and contextual influences affecting self-management. The findings from the quantitative phase of this study indicated that parent-adolescent communication and T1D-specific family conflict are associated with T1D self-efficacy, division of family responsibility for T1D management, activation and self-management of T1D. However, the findings also indicated that T1D-specific family conflict and parent-adolescent communication are differentially related to distinct aspects of self-management and QOL. Overall, this study identified that parent-adolescent communication and family context characteristics relate to and contribute to self-management and T1D-specific QOL in adolescents living with T1D. This research identified what factors contribute to helping or hindering adolescent engagement with self-management of T1D and specifically that adolescents' perceptions of their communication with their parents about T1D management are important. For the first time, the findings provide a nuanced understanding of adolescent perspectives on self-management of T1D and communication with parents about T1D self-management. Promoting self-efficacy through targeting communication strategies employed by adolescents and their parents may result in more optimal sharing of responsibilities, improved self-management and better QOL for adolescents living with T1D.

Adolescents do not manage their T1D by themselves but rather in collaboration with parents who often help in health tasks and navigating healthcare provider interactions. Although parental support is critical for the adolescent's development of disease-management skills, it requires parents maintain a delicate balance of allowing adolescent autonomy and providing parent support. When parental support shifts to parental overprotection, i.e., restricting and limiting an adolescent's activity engagement, it may impair adolescent's skill development and mastery necessary to complete diabetes-management tasks independently, including healthcare provider communication. In particular, parental overprotection may impact the adolescent's interpersonal effectiveness skills, in that adolescents may not learn how to be mindful and attentive to relationships or build a repertoire of mindfulness and self-compassion that would support effective healthcare provider communication and more independent diabetes management. Thus, adolescent experiences of interpersonal effectiveness skills, parental overprotection, and healthcare provider communication may have long-term implications for diabetes outcomes as the adolescent transitions to adulthood. Adolescents with type 1 diabetes might then benefit from an intervention targeted at interpersonal effectiveness skills to both reduce parental overprotection and improve healthcare provider communication. Adolescent participants (n=25, mean age=16.25, 56% female) completed a baseline survey for a pilot intervention study aimed at increasing mindfulness and decreasing diabetes-related stress. Adolescents reported on mindfulness, self-compassion, parental overprotection, and health care provider communication. The present study found that increased adolescent perception of parental overprotection was associated with decreased interpersonal effectiveness skills (mindfulness and self-compassion) and all were associated with increased difficulty with communicating with healthcare providers about diabetes. In addition, the potential utility of a mindfulness intervention for improving interpersonal effectiveness skills, decreasing parental overprotection, and improving adolescent healthcare provider communication was examined. The results also indicate potentially meaningful change scores for the four variables of interests between treatment and waitlist groups with very small to large effect sizes that may accrue into meaningful changes across time. Further, among adolescents who were highly engaged in the intervention program (n=11), mindfulness, self-compassion, parental overprotection, and healthcare provider communication again showed potentially meaningful change scores with small to medium effect sizes that may have long-term impact across time. These findings indicate that a mindfulness-based intervention has potential utility, especially for participants who were highly engaged in the program. Parental overprotection and interpersonal effectiveness skills may be key factors to target in interventions to support the transition of diabetes-management behaviors from parent to adolescent as the adolescent ages into young adulthood, particularly with regard to adolescents learning to communicate with their healthcare providers.

This paper examined the role that division of responsibility (adolescent, parent, and shared) for diabetes management tasks plays in adolescent health in this population across two studies. Diabetes responsibility was measured using two methods: surveys (Study 1) and a novel coding scheme applied to parent-adolescent interactions (Study 2). Results of study 1 showed that Latinx mothers reported more shared and less adolescent responsibility in surveys than non-Hispanic White mothers. Mother- and adolescent-reported shared responsibility were generally associated with higher levels of diabetes adherence, while reports of individual (parent and adolescent) responsibility were generally associated with lower levels of diabetes adherence. Results of Study 2 showed that descriptions of shared, adolescent, and parent responsibility in conflict discussions were not associated with adolescent health. Cultural values (familism and independence/self-reliance) were not associated with particular patterns of diabetes responsibility, but cultural values were associated with health outcomes. Specifically, independence/self-reliance tended to be associated with poorer adolescent health (lower quality of life and increased anxiety), while adolescent familism was associated with higher levels of adolescent-reported adherence. The importance of multi-method studies and implications for interventions are discussed.

Book excerpt: Type 1 Diabetes (T1DM) is a life-long chronic illness often diagnosed during childhood or adolescence which requires individuals to adhere to a complex treatment regimen. Unfortunately, adherence is especially poor during adolescence. One factor which may influence adherence is treatment responsibility, or determining whether the parent or adolescent assumes treatment responsibility. Although there are no clear guidelines regarding when to transition responsibility, knowing whether the adolescent has the cognitive capacity or executive functions (EF) to successfully manage their treatment is essential. Parental stress has also been associated with treatment responsibility, suggesting that greater levels of parent responsibility are associated with greater levels of reported parental stress. However, most studies have failed to examine the effects of multiple forms of stress (i.e., general, parenting, and disease-related) on treatment responsibility. Further, little is known regarding how parent perception of adolescent general and disease related EF and various forms of parent stress are related to treatment responsibility. The present aimed to assess the contribution of various forms of stress and parent perception of general and diabetes related executive functioning on the transition of responsibility from parent to adolescent in adolescents with T1DM. The findings from the study underscore the role of parent reported diabetes distress as it relates to the transition of responsibility. Specifically, diabetes distress was a significant predictor of greater parent responsibility above and beyond the role of general and disease related executive functioning. Nonetheless, results suggest that parents transition greater responsibility to their adolescent when they perceive their adolescent to have a greater ability to remember tasks associated with their illness. General and perceived stress did not influence the transition of responsibility from parent to adolescent. The present study expanded previous research by examining the role that various forms of stress and parent perception of adolescent's executive function play in their decision to transition responsibility for diabetes management among adolescents with T1DM. Despite limitations within the present study, the findings suggest clinical implications and future directions for research. Specifically, future interventions should target decreasing parent diabetes distress, while also focusing interventions on improving adolescent's diabetes related memory for managing their disease. Future research should include a smaller age range and should be longitudinal in nature to better understand how the dynamic nature of the associations among various forms stress, executive functioning, and treatment responsibility may have an impact on treatment adherence and glycemic control.

Glycemic control among patients with T1D often deteriorates during adolescence; yet little is known about the most effective way for providers to communicate with adolescents to prevent this decline. Given the importance of effective communication, examination of effective patient-provider communication strategies is needed. The current investigation used Motivational Interviewing (MI) as a framework to help characterize naturally-occurring adolescent patient-provider communication in medical encounters and examined the relations between provider communication and T1D self-management and control. Participants were five pediatric endocrine providers and 55 adolescents with T1D (49% female; 76% White; M age= 14.8 years, SD= 1.6). Mean T1D duration was 7.9 years (SD= 3.9) and mean baseline HbA1c was 8.58% (SD= 1.4). Adolescents and caregivers completed surveys related to diabetes self-management and psychosocial functioning at a routine endocrinology visit and again at one and three months post-baseline. Medical encounters were audio-recorded and coded. HbA1c was obtained via medical chart review at baseline, three, and six month appointments. The aim of this cross-sectional study is to expand the research and examine organizational factors that may be associated to HbA1c among Latinx adolescents (N=55) and their parents in Southern California. A secondary data analysis was conducted on the Adolescents and Young Adults with Type 1 Diabetes Assessment in the Center for Endocrinology, Diabetes, and Metabolism (Attack) Study. Participant inclusion criteria included, self-identified as Hispanic/Latino/Latinx between the ages 11-17, a diagnosis of T1D for at least 6 months, and adolescents and parents who answered the questions of interest. Results from hierarchical multiple linear regression suggested that intrapersonal (e.g., diabetes self-management behaviors [blood glucose checks]), interpersonal (e.g., family’s insurance [proxy for socio-economic status] and language preference of the parent), and organizational level factors (e.g., cultural competency [communication and discrimination] of the provider) were significantly associated with HbA1c. Therefore, these factors should be further examined as potential predictors of HbA1c levels among other groups of Latinx adolescents with T1D, as they may inform T1D management interventions.

For instance, parenting can enrich and give focus to parents' lives; generate stress or calm; and create any number of emotions, including feelings of happiness, sadness, fulfillment, and anger. Parenting of young children today takes place in the context of significant ongoing developments. These include: a rapidly growing body of science on early childhood, increases in funding for programs and services for families, changing demographics of the U.S. population, and greater diversity of family structure. Additionally, parenting is increasingly being shaped by technology and increased access to information about parenting. Parenting Matters identifies parenting knowledge, attitudes, and practices associated with positive developmental outcomes in children ages 0-8; universal/preventive and targeted strategies used in a variety of settings that have been effective with parents of young children and that support the identified knowledge, attitudes, and practices; and barriers to and facilitators for parents' use of practices that lead to healthy child outcomes as well as their participation in effective programs and services. This report makes recommendations directed at an array of stakeholders, for promoting the wide-scale adoption of effective programs and services for parents and on areas that warrant further research to inform policy and practice. It is meant to serve as a roadmap for the future of parenting policy, research, and practice in the United States.

Book excerpt: The purpose of this research effort among adolescents ages 11 to 15 years was to gain a greater understanding, from the adolescent's perspective, of their experiences in living with diabetes and associated challenges and management issues they faced. The aim was to build a theoretical paradigm that captured the main categories and moving processes in the experiences of adolescents and provided much needed information and hypotheses to support interventional design. Classical grounded theory with constant comparative analysis using gerund coding was utilized. Fifteen individual interviews were conducted. The substantive theory that was developed was "normalizing". Normalizing is defined as the ability of the adolescents to integrate diabetes into the background of their daily life by creating routines to make diabetes `part of me'. The conceptual codes developed that describe normalizing for adolescents with diabetes included: a) `recognizing life is changing', b) `taking action to prevent a crisis', c) `disclosing to engage support', d) `taking on the burden of care', e) `accepting the `new normal'', and f) `hoping for a normal future'. For adolescents with diabetes, the normal developmental tasks of adolescence are closely related to each step of this normalizing process and help to explain why adolescents struggle with diabetes during this phase of adolescence. Needle fear, moving through transition to self-care, conflict with parents, and the interactions, reactions and emotions of others were key processes that affect the ability to normalize. Strategies used by adolescents in managing their diabetes include maintaining motivation, building trust, and learning to cope. The supportive behaviors of parents, peers, teachers, and others were helpful. When researchers and clinicians view this difficult stage of adolescent diabetes care as an attempt to normalize their life, it places a very different and positive perspective on this process with a focus on wellness and maintaining a normal life rather than a focus on illness. This creates many new opportunities to provide education and interventions to not only help the adolescents normalize but also assist their parents, peers, schools, and communities to help them normalize their life as well.

Mary Dixon-Woods and colleagues (Mary Dixon-Woods, Booth, & Sutton, 2007) identified 42 qualitative evidence synthesis papers published in health care literature between 1990 and 2004. An ongoing update by Hannes and Macaitis (2010)identified around 100 additional qualitative or mixed methods syntheses. Yet these generally lack a clear, detailed description of what was done and why (Greenhalgh et al, 2007; McInnes & Wimpenny, 2008). Choices are most commonly influenced by what others have successfully used in the past or by a particular school of thought (Atkins et al, 2008; Britten et al, 2002). This is a substantive limitation. This book brings balance to the options available to researchers, including approaches that have not had a substantial uptake among researchers. It provides arguments for when and why researchers or other parties of interest should opt for a certain approach to synthesis, which challenges they might face in adopting it and what the potential strengths and weaknesses are compared with other approaches. This book acts as a resource for readers who would otherwise have to piece together the methodology from a range of journal articles. In addition, it should stimulate further development and documentation of synthesis methodology in a field that is characterized by diversity.
